[The possibility of determining the functional status of the hypertrophied myocardium in multiple valvular defects of the heart].

A comprehensive angiocardiographic study with computer-aided data processing were performed in 46 patients (mean age 34.2 years) with multivalvular diseases. Thirty two patients were diagnosed as having mitral and aortic defect, 14 had mitral, aortic and tricuspid defect. Three-valve disease, unlike two-valve disease, showed a "paradoxical" improvement in some parameters, such as end-diastolic volume, end-systolic volume, myocardial mass, ventricular performance rates. An analysis of circular intramyocardial tension, coronary sinus blood oxygen extraction and saturation indicated that myocardial function was less intensive in three-valve disease, suggesting the compensatory pattern of tricuspid valve involvement. However, this "compensation" results in a further decrease in cardiac output and cannot be regarded as a genuine compensation. In multivalvular disease, the integral intramyocardial tension and its ratio to coronary sinus blood oxygen saturation may serve as the only diagnostic criterion for myocardial dysfunction and a predictor in determining the outcome of surgical intervention.

[Intracardiac mechanisms of compensation of isolated aortic insufficiency].

A complex angiographic examination was performed in 28 patients with rheumatic aortic valvular disease. The major mechanism of compensation in aortic failure was found to be an increase in left ventricular end-diastolic volume at the expense of added regurgitating blood volume. It was shown that isolated aortic failure was a good compensated defect with high functional myocardial reserves and coronary circulation along with adequate left ventricular dilatation and normal-stress nature of left ventricular hypertrophy. Left ventricular dilation at a regurgitation fraction of less than 30% and intramyocardial tension increase were demonstrated to be unfavorable predictive signs and to be indicative of profound myocardial dysfunction.

[Aortocoronary bypass in progressive stenocardia].

The authors evaluate the efficacy of aortocoronary shunting in the treatment of 10 patients with progressive angina pectoris (unstable stenocardia) in follow-up periods of up to 12 months after the operation. In none of the patients did angina pectoris or myocardial infarction develop in the indicated postoperative periods.

[Aortocoronary shunting in patients with newly developed stenocardia].

Aorto-coronary bypass were performed in 10 patients aged from 34 to 55 years 7 to 30 days after the occurrence of angina pectoris for the first time. Coronarography demonstrated stenosis (of more than 75%) of the coronary arteries in all patients. Angina pectoris disappeared and did not recur in postoperative periods of up to 12 months in all cases. Shuntography was carried out 6 weeks after the operation in one patient: the shunt was potent.

[Aortocoronary shunting in the early period after myocardial infarction].

Fourteen patients aged 34-54 years underwent aortocoronary bypass surgery for early postinfarction angina within 30 days following myocardial infarction. In periods up to 1.5 years after surgery, angina pectoris was not detected in the patients; cases of recurrent myocardial infarction were not registered.

[Prognostication of the degree of the hypotensive effect of revascularization of both kidneys in patients with renovascular hypertension].

A method for prognosticating the degree of a hypotensive effect of revascularization of both kidneys was developed on a group of 22 patients with vasorenal hypertension caused by bilateral affections of the renal arteries. It was based on mathematical treatment by the method of multidimensional regressive analysis of some parameters of preoperative examination of the patient and allowed the postoperative arterial pressure value to be calculated in mm Hg.

[Isolated mitral valve insufficiency in comparison with mitral-tricuspid insufficiency: various mechanisms of compensating for the defect and functional status of the myocardium].

A combined analysis of ventricular contractility and intracardiac hemodynamic compensatory mechanisms was carried out, on the basis of angiocardiographic findings, in 37 patients with rheumatic mitral incompetence. Atrial fibrillation aggravates essentially the defect's hemodynamics, while added tricuspid incompetence is accompanied by a certain off-loading in the lesser circulation network. A grossly perversed phasic structure of intramyocardial stress was noted, apparently being a compensatory mechanism. Reduced specific coronary flow and diastolic perfusion gradient in intact coronary arteries are shown to be causes of clinical angina.

[Status of the coronary arteries and left ventricle of patients surviving myocardial infarction].

The condition of coronary arteries and left-ventricular function were assessed in 62 patients with a history of myocardial infarction 1 month to 4 years prior to coronarographic investigation. It was demonstrated that the detectability of an occlusion in the artery supplying blood to the infarcted area decreased with time elapsed since myocardial infarction, the correlation being more characteristic of the anterior interventricular branch of the left coronary artery. An analysis of ventriculograms showed that myocardial infarction due to a lesion of the left coronary artery results in a smaller left-ventricular dysfunction and is less commonly associated with circulatory insufficiency. The frequency of spontaneous thrombolysis is shown to be relatively high in postmyocardial-infarction patients.

[Concentration of endogenous prostanoids and cyclic nucleotides in patients with atrial fibrillation].

The variations in the content of endogenic prostanoids and cyclic nucleotides in the blood plasma of the coronary sinus and ascending aorta were studied in patients with the idiopathic stable form of atrial fibrillation before and after sinus rhythm recovery effected by electroimpulse therapy. Changes in the levels and ratios of these compounds in the coronary venous and arterial blood were found to be opposite in their trends. A significant elevation in TxB2 levels and the TxB2/6-keto-PGF1 ratio as well as increased cGMP concentrations in the plasma of the coronary venous blood appear to be suggestive of an unfavourable prognosis because patients with such changes developed atrial fibrillation recurrences soon after electroimpulse therapy.

[Effect of non-ionic roentgenocontrast preparation Omnipaque-350 on central hemodynamics and myocardial contraction].

The authors studied the effect of the non-ionic radiopaque substance Omnipaque-350 on the central hemodynamics and myocardial contractility. A comprehensive angiographic examination was conducted in 25 patients. Omnipaque-350, when administered intravascularly, was found to induce the same general reactions as ionic triiod-containing radiopaque. It did not exert direct myocardiodepressive effect; yet, upon intracoronary administration it caused uniform changes in myocardial contractility and elasticity which corresponded to transitory alterations in its perfusion. During left ventriculography, Omnipaque-350 as compared with Urografin-76, has less marked hypervolemic properties and a less significant impact on the central hemodynamics and the pump function of the heart.

[Clinical aspects and diagnosis of ischemic heart disease in patients with arterial hypertension].

Twenty-six patients with arterial hypertension and chest pain were examined using echocardiography, bicycle ergometry, pharmacological tests with the intravenous administration of dipiridamol and isoproterenol, coronary angiography and left ventriculography. With regard to the painful syndrome the patients were divided into 2 groups: Group 1 with angina, Group 2 with cardialgia. The majority of the group 1 patients showed extensive atherosclerosis of the coronary arteries. The results of bicycle ergometry and pharmacological tests in this group were positive in the majority of cases. The group 2 patients displayed minor atherosclerosis of the coronary arteries or intact arteries which correlated with negative results of the bicycle ergometry and pharmacological tests. Echocardiographic and ventriculographic examinations of the functional state of the left ventricle revealed circulation insufficiency in none of the patients.

[Idiopathic hypertrophic subaortic stenosis: functional condition of the left ventricle, central and pulmonary hemodynamics (an angiocardiographic study)].

Two-projectional left ventriculography, with pressure recording in the aorta, left ventricle, right chambers of the heart and the pulmonary artery, was performed in 26 patients with idiopathic hypertrophic subaortal stenosis free of intraventricular pressure gradient. Considerable left-ventricular hypertrophy associated with this condition was shown to be of "low-stress" origin and strictly concentric in nature, left ventricular systolic function keeping within normal range, and diastolic function being disordered as manifested in significantly increased filling pressure and disordered left ventricular relaxation. Disordered geometry and local contractility of the left ventricle reflected in hypercontraction of the outlet orifice, papillar muscles and posterior wall, combined with its reduced longitudinal contractility results in excessive tension of tendinous chordas and abnormal movement of the anterior mitral cusp towards the hypertrophic ventricular septum and marked mitral regurgitation.

[Mechanisms of regulation of high cardiac output in patients with hypertension (angiocardiographic study)].

To identify mechanism conditioning high cardiac output, an angiocardiographic study was conducted in 39 patients with essential hypertension. Increased stroke and cardiac output was found to be due to increased venous inflow, with inotropic condition of the hypertrophic myocardium being reduced in spite of enhanced general pumping function of the left ventricle. As arterial hypertension tended to stabilize and cardiac output declined, total peripheral resistance assumed the principal responsibility for its regulation, whereas at earlier stages of the disease it shows either normal, or even reduced levels, and venous inflow is the principal factor controlling cardiac and stroke output.
