Neuropeptide Y (NPY) is a 36-amino acid polypeptide that is widely distributed in the central nervous system and periphery. Pharmacological studies have suggested that there are at least three receptor subtypes, Y1, Y2, and Y3. Cloning of the Y1 subtype has been reported previously. Here we report the isolation by expression cloning of a cDNA encoding a human NPY receptor displaying a pharmacology typical of a Y2 receptor. COS-7 cells transfected with the cDNA express high affinity binding sites for NPY, peptide YY, and NPY13-36, whereas [Leu31,Pro34]NPY binds with lower affinity. The receptor is 381 amino acids in length and has seven putative transmembrane regions typical of G-protein-coupled receptors. Comparison of the amino acid sequence of this Y2 receptor to that of the human Y1 receptor indicates that the two receptors are 31% identical at the amino acid level. Northern blot analyses reveal a single 4-kilobase mRNA species and indicate that the messenger RNA is present in many areas of the central nervous system. NPY induced calcium mobilization and inhibited forskolin-stimulated cAMP accumulation in Chinese hamster ovary cells that stably express the Y2 receptor cDNA, indicating that the recombinant Y2 receptor is functionally coupled to second messenger systems.
Bacillaene, a novel polyene antibiotic, was discovered and isolated from fermentation broths of a strain of Bacillus subtilis. The novel antibiotic has a nominal molecular weight of 580 and an empirical formula of C35H48O7. Bacillaene is active against a broad spectrum of bacteria in agar-plate diffusion assays. Studies in vitro indicate that the antibiotic inhibits prokaryotic protein synthesis but not eukaryotic protein synthesis. Cell survival studies performed with strains of Escherichia coli indicate that the antibiotic is a bacteriostatic agent.
In the past 5 years, a new approach was developed to evaluate the patient with sporadic symptoms as is the patient with syncope--the long-term ambulatory loop electrocardiogram monitoring. This method allow to assure the heart rate and rhythm at the moment of the symptoms hardly to be able with the other diagnostic tools disposable. The long-term ambulatory loop electrocardiogram monitoring was not to replace the actual tools, but complement them. Newer devices are smaller and too light, and have the ability of registry some minutes before and after the symptom. In the near future, the devices will be able to detected and store arrhythmias and to possess an easy and accessible management. With continued and profitable work that has been done in this area, a large proportion of patients with the diagnosis of "syncope of unknown etiology" can have a diagnostic and specify therapeutic and we will be able to assert with more strictness if the symptoms related by the patient are or not of cardiovascular origin.

The present work is an attempt to provide a general overview on the current methods and applications of heart rate variability (HRV), starting from the underlying mechanisms of cardiac rhythmic automaticity and the influence on heart rate of biological modulators, namely autonomous nervous system. Different evaluation methods for HRV are compared in what concerns the duration of the records under analysis and the mathematical approach (spectral/non-spectral). Finally, clinical applications of HRV are mentioned, some of them the Department is particularly interested in, namely coronary heart disease, heart failure and diabetic neuropathy. The authors conclude, minding some aspects susceptible of discussion in the future of this technique.
Primary fallopian tube carcinoma is a rare neoplasm. It may be postulated that postoperative loco-regional radiotherapy is a valuable adjunctive in the overall management of the tubal carcinoma. However, we can't advocate the efficacy of this approach authentically due to a very small number of cases. A prospective controlled and randomized trial with larger patient number may yield some definite conclusions regarding it's optimal management.
Five cases of primary lymphoma of the brain are reported and the literature reviewed. The diverse clinical spectrum at presentation and variable response to therapy are few of its unique features. The literature is reviewed briefly and the importance of diagnosis at an early stage and prompt institution of therapy in form of radiation therapy and chemotherapy when indicated is stressed.
Primary melanoma of the meninges, a rare CNS tumor, is presented. Criteria for diagnosing a primary CNS melanoma are elucidated. Literature is reviewed in this context. The histogenesis of tumor, problem of occult primary melanoma and the role of CT scan and CSF cytology in early diagnosis have also been highlighted.
OBJECTIVES: Characterize power spectrum pattern of the heart rate variability and assessment of the relative contributions of sympathetic and parasympathetic cardiac nervous system control in athletes. STUDY DESIGN: Prospective study of athletes and sedentary healthy control group sex and age matched. SUBJECT AND METHODS: 8 athletes, 3 female and 5 male, swimming (4), canoeing (2) and cycling (2), aged 17.4 +/- 3.6 years, and 8 sedentary healthy controls. ECG signals were recorded after a period of 15 minutes in supine rest with controlled breathing at 15 cycles/min. Signal acquisition was done at 300 samples/sec. From 512 consecutive heart beats, we calculated the average, the standard deviation, the maximum and the minimum values and the rate between the longest and the shortest R-R interval (E/I). We also calculated, after computing the fast Fourier transform, the total spectrum power, the low frequency component (LF, from 0.01 to 0.15 Hz) and the high frequency component (HF, greater than 0.15 Hz) and its ratio (LF/HF). RESULTS: The average R-R interval was 921 +/- 154.2 msecs and 673.2 +/- 98 msecs, the standard deviation was 72.5 and 29.4 msecs and the ratio E/I 1.63 +/- 0.14 and 1.28 +/- 0.08, respectively for athletes and control group. Differences between groups were significant (p less than 0.01) for all parameters, with higher variability in the athletes. Both spectral bands (LF and HF) and higher power in athletes (LF = 0.54 +/- 0.23 and HF = 0.76 +/- 0.14) than in the control group (LF = 0.14 +/- 0.10 and HF = 0.18 +/- 0.15) (p less than 0.001). There were no significant differences for LF/HF ratio, or normalized LF (LF%) or normalized HF (HF%) between groups. CONCLUSIONS: The present results indicates higher power of both spectral bands (LF and HF) and higher amplitude of the respective peaks in athletes when compared with healthy sedentary, with a clear predominance of the HF band in the total spectral power density, which suggest that the higher heart rate variability observed in athletes reveals the predominance of parasympathetic activity, without reduction of the sympathetic tone.
The signal-averaged ECG became an important noninvasive method for identifying life-threatening ventricular arrhythmias. It is most accurate in patients with coronary artery disease and can be helpful in predicting whether a patient with an unexplained syncope or nonsustained VT is at risk of developing sustained ventricular tachycardia or not. A patient with an entirely normal signal-averaged ECG is at very low risk of sustained VT. In contrast, an abnormal signal-averaged ECG should raise suspicion about VT, and prompt further studies, including electrophysiological testing. Signal-averaged ECG can also be used repeatedly with acceptable reproducibility. Technological advances and improvements in the frequency analysis of the averaged signal may increase the predictive accuracy of the technique and consequently enhance its clinical applications.
In 24 adults with hemoglobin SS followed at the Duke University Comprehensive Sickle Cell Center, we have studied the following nutritional parameters: reduced ascorbic acid; dehydroascorbic acid; alpha and beta carotenes; cryptoxanthin; and alpha and gamma tocopherols in whole blood, washed red blood cells, plasma, or serum. In the same population we also examined reduced glutathione (GSH) and oxidized glutathione (GSSG). Fifteen of these 24 patients also were interviewed for usual dietary intakes using a 28-day dietary history. Data obtained from patients with hemoglobin SS, sickle cell anemia (SCA) were compared to those found for seven healthy normal black adults of similar age. Plasma alpha tocopherol levels were significantly lower in SCA individuals than those of the controls (P less than 0.004). Alpha and gamma tocopherol levels in sickle RBCs were significantly higher than those from RBC suspensions of control subjects (P less than 0.007, and P less than 0.001, respectively). All serum values for carotenoids examined, specifically, beta carotene, alpha carotene, and cryptoxanthin were also markedly depressed when compared to those of healthy controls (P less than 0.001, P less than 0.002, and P less than 0.001, respectively). No other statistically significant differences were found between the two groups for any of the remaining variables, including dietary estimates. Dietary analyses suggest that dietary intakes of SCA individuals exceeded the recommended daily allowances (RDA) of all macro- and micronutrients measured, and intakes of most nutrients exceeded those of black controls interviewed. These results suggest that in individuals with SCA, several micronutrients vital to maintaining reducing capacity are present in diminished quantities in plasma/serum. These anomalies exist in SCA patients even though their intake of these micronutrients are similar to those of healthy black men and women.
This presentation is a randomized prospective study of 74 patients of squamous cell carcinoma of esophagus divided into two groups. First group was treated with radical radiation alone and the other with radical radiation and combination of bleomycin and 5-FU. Analysis at the end of two years showed that the patients treated with addition of chemotherapy had an overall survival of 23 percent as compared to nine percent in the radiation alone group (P less than 0.05).
Thirty seven cases of carcinoma of skin treated with electron-beam irradiation over a period of three years have been analysed retrospectively. Majority of lesions were in head and neck and basal-cell carcinoma was the commonest histological type. Of twenty-one patients followed, seventeen (81%) achieved complete remission and remained disease free for a period of one to three years. Cosmetic results were excellent. Role of electron-beam in the management of these superficial cancers and its advantage over the conventional radiation therapy methods have been highlighted. Radiation therapy utilising electron-beams form a very useful modality in the management of skin cancers, especially those arising in head and neck.

Immunoblot analysis of liver microsomes from nine patients demonstrated that each contained a cytochrome P-450 that reacted with an antibody directed against the ethanol-inducible rabbit liver cytochrome, P-450 3a. Two of the liver specimens exhibited high concentrations of the immunoreactive protein, high rates of aniline hydroxylation and N-nitrosodimethylamine demethylation, and extensive inhibition of activity in the presence of antibody to P-450 3a. One other liver specimen exhibited a very low rate of aniline hydroxylation with significantly less antibody inhibition. The variability witnessed was independent of the alcohol history of the individual patients, suggesting that the human cytochrome may be under some other environmental, dietary or genetic regulation. Its inducibility by ethanol was not directly studied in this investigation. However, we conclude that there is a cytochrome P-450 present in human liver which is immunochemically and catalytically similar to the ethanol-inducible P-450 of rabbit liver.
Spatial statistics methods have been used to analyse the nuclear pore pattern in rat ventral prostate nuclei isolated from adult animals. The observed results show that: (1) pores on prostate nuclear membranes are not randomly distributed; (2) the data sets obtained from different micrographs are consistent with the same statistical model thus suggesting the existence of a typical pore distribution.
From 1957 to 1980, 170 neuroblastoma were surgically approached: 3 cervical, 11 thoracic, 147 abdominal, 5 pelvic. The surgical data gathered from this large series leads us to assess that there are great differences between lateral and median neuroblastoma. Lateral ones can be surgically removed without major risk. Median ones arising from sympathetic nerves which depend on the aorta or on the visceral arteries (coeliac or mesenteric) cannot be removed totally without life-threatening risk. These data do not agree with Evans and d'Angio staging. Indeed, stage I and II are only observed in neuroblastomas developed on the lateral nerves of the sympathetic chain and can be removed. Stage III tumors can also be removed surgically when arising from a lateral site, even if they cross the median line. On the contrary all median perivascular neuroblastomas are to be staged III: total removal is not possible and the surgical risk is too high when compared with efficiency of chemotherapy. A special type, the distal perivascular neuroblastoma, is pointed out (5 cases). It is very important to precise exactly the initial localization of neuroblastoma, whatever be their metastatic extension. The new technics of investigation (C.T., ultrasonography, but also plain film and front and lateral views of intravenous pyelogram) allow to define precisely the median or lateral site of the tumor and also to choose between a non-surgical (median) or a surgical (lateral) treatment. This distinction seems in correlation with the two different ways of pathway of the embryonic neuroblast cells.
