Factor VIIIR:Ag plasma levels in patients with cervical and ovarian carcinoma.

The Authors measured Factor VIII Related Antigen (FVIIIR:Ag), Factor VIII Coagulant Activity (FVIII:C) and fibrinogen plasma levels in 21 patients with cervical carcinoma and in 14 ones with ovarian carcinoma. In the women with cervical cancer, FVIIIR:Ag was significantly higher than controls only in advanced stages. No significant variation of fibrinogen according to stage and no correlation between FVIIIR:Ag and fibrinogen plasma levels were observed. In the patients with ovarian cancer FVIIIR:Ag was significantly increased in stage III-IV and not significantly in stage I. In these subjects too there was a lack of correlation between FVIIIR:Ag and fibrinogen plasma levels. These results seem to indicate that FVIIIR:Ag increase in advanced stages is due to its major release in circulation resulting from invasion of vascular endothelium and not to an aspecific reactivity. These data could suggest a role of FVIIIR:Ag as an aspecific marker of vascular involvement by cervical and ovarian neoplasms.

Effects of the dopamine antagonist veralipride on hot flushes and luteinizing hormone secretion in postmenopausal women.

The effects on hot flushes of the dopamine antagonist Veralipride versus placebo were investigated in a randomized double-blind study of postmenopausal healthy women (N = 20 in each group). Cutaneous temperature recording and plasma LH pulsatility were studied in eight patients from each group. Veralipride administration (100 mg/day for 30 days) induced a significant (P less than .01) reduction in vasomotor symptoms and was more effective (P less than .05) than placebo. Treatment was followed by the expected increase (P less than .001) in plasma prolactin levels and by a significant decrease (P less than .05) in mean plasma LH. A significant reduction (P less than .01) was observed in objectively recorded hot flushes after Veralipride treatment, whereas there was no significant change in the characteristics of LH pulsatility. Infusion of the opioid antagonist naloxone (N = 5) induced a significant (P less than .01) increase in LH secretion after Veralipride administration. These results suggest that the endogenous opioid system may mediate the endocrine and clinical effects of long-term Veralipride treatment.

Analysis of failures in patients with cervical carcinoma treated by radical surgery and external pelvic irradiation.

In this paper we examine 68 patients who had undergone radical abdominal hysterectomy according to Wertheim-Meigs for Ib-II cervical carcinoma. Post-surgical stage was T1bN0M0 in 35 patients, T1bN1M0 in 3, T2aN0M0 in 7, T2aN1M0 in 6, T2bN0M0 in 13, T2bN1M0 in 4. All the patients, except 5 with ureteral fistulas, had high energy external pelvic irradiation, reaching a dose of 45 Gy. Up to December 1986, the neoplasia had relapsed in 10 patient (14.7%). According to post-surgical stage we observed 1/38 relapses in T1b patients (2.63%), and 9/30 relapses in T2 ones (30%). Otherwise, we observed 3 relapses in the 55 patients with negative lymph-nodes (5.4%), and 7 recurrences in the 13 patients with histologically proven positive lymph-nodes (53.8%). In these last patients 3/7 relapses were distant (42.8%). Nodes status seems to be the most important prognostic factor in patients with IB-II cervical carcinoma. Since lymph-nodal involvement could be indicative of a systemic spreading of disease, we think that an adjuvant polychemotherapy, in addition to surgery and radiotherapy, could improve the prognosis of patients with positive lymph-nodes.

Preoperative evaluation of CA 125 and CA 19-9 serum levels in patients with ovarian masses.

Serum CA 125 and CA 19-9 were presurgically measured in 40 patients with ovarian carcinoma and in 108 with benign ovarian pathologies. The sensitivity for ovarian carcinoma of CA 125 (cut-off value = 65 U/ml) and CA 19-9 (cut-off value = 40 U/ml) were 67.5% and 37.5% respectively. In particular serum CA 125 was elevated in 71.9% of non-mucinous and in 50% of mucinous carcinomas, while serum CA 19-9 was high in 25% of non-mucinous and in 87.5% of mucinous malignancies. The correlation of CA 19-9 with mucinous histotype was significant. Elevated serum levels of CA 125 and CA 19-9 were observed respectively in 14.7% and in 13.8% of benign adnexal masses. The percentages of elevated serum marker levels were significantly higher in patients with ovarian carcinoma than in women bearing benign ovarian pathology (P less than 0.001 for CA 125; P less than 0.01 for CA 19-9). Serum CA 125 and CA 19-9 alone cannot clarify the nature of an adnexal mass. However, the measurement of serum levels of these markers could give additional information to other diagnostic methods, such as ultrasonography, for discriminating benign from malignant ovarian pathologies.

Use of sulprostone for induction of preoperative cervical dilation or uterine evacuation: a comparison among the effects of different treatment schedules.

Sulprostone was administered to 144 pregnant women to induce either preoperative cervical dilation or uterine evacuation. A comparison was performed to select the drug treatment schedule showing the greatest effects with the lowest incidence of side effects. As for preoperative cervical dilation, the local (intracervical/intramural) injection of 50 + 50 micrograms and the intramuscular treatment with 500 micrograms of sulprostone showed the most favorable effects on the cervix after 12 and 6 hours, respectively. The cervix dilated 8 mm or more in 82.6% of subjects treated locally and in 85.7% of those treated intramuscularly. No patient experienced a serious side effect, but two displayed vomiting after receiving 500 micrograms of the drug. Uterine evacuation was induced by both repeated intramuscular injections (500 micrograms every 4 hours) and continuous intravenous infusion (2.8 micrograms/minute for 6 hours) of sulprostone in more than 90% of the patients with intact pregnancy, missed abortion, or fetal death. The efficacy of the drug was not influenced by gestational age or parity. Gastrointestinal symptoms were the only recorded side effects, with negligible incidence and intensity for both routes of sulprostone administration.

Prevention of puerperal lactation by a single oral administration of the new prolactin-inhibiting drug, cabergoline.

To evaluate the efficacy of a single oral administration of the new ergot derivative Cabergoline in the prevention of post-partum lactation, we compared the effects of three different doses of the drug with those of placebo in 32 puerperal women. In a controlled, double-blind trial, the subjects were randomly allocated to four treatment groups receiving either placebo or 400, 600, or 800 micrograms Cabergoline (N = 8 in each group) within 24 hours after delivery. Treatment efficacy was assessed clinically by physical examination before (day 0) and at one, two, three, four, and 14 days after treatment. Plasma prolactin (PRL) concentrations were measured in blood samples collected before and at one, two, three, and four days after treatment. Lactation was prevented in four of the eight subjects (50%) who received 400 micrograms Cabergoline and in all subjects who received 600 or 800 micrograms Cabergoline. By contrast, only one of the eight subjects (12.5%) receiving placebo showed no signs of spontaneous lactation within the 14 days after delivery. No effects of placebo administration on plasma PRL levels were observed. Plasma PRL concentrations were significantly reduced starting from one day after Cabergoline administration, however, and the amount of inhibition of PRL secretion induced by different doses of the drug was not statistically different. These preliminary data demonstrate that Cabergoline has a dose-related effect in the prevention of postpartum lactation, and milk secretion can be prevented completely by a single oral administration of 600 or 800 micrograms of the drug.

Prediction of 1-year outcome after complicated and uncomplicated myocardial infarction: Bayesian analysis of predischarge exercise test results in 300 patients.

After myocardial infarction (MI), the additive prognostic value of exercise variables to clinical variables has been questioned. The merits of a symptom-limited predischarge exercise test were therefore evaluated in clinically defined subgroups of patients. Exercise tests were consecutively performed by 208 survivors of uncomplicated MI (no heart failure, postinfarction angina, recurrent infarction, or late arrhythmias) and by 92 survivors of complicated MI. After uncomplicated MI (1-year mortality rate 4%), an achieved workload greater than 70% of age-predicted maximum identified 145 patients at very low risk (predictive value for survival 98%). After complicated MI (1-year mortality rate 13%), an exaggerated heart rate response was the best predictor of outcome, but had low (92%) predictive value of survival at 155 bpm. It is concluded that stress testing has only limited value after complicated MI. After uncomplicated MI, exercise variables are extremely helpful in identifying patients at very low risk in whom further investigations are not warranted.

Clinical and metabolic study of a new pill containing 20 mcg ethinylestradiol plus 0.150 mg desogestrel.

The clinical and metabolic effects of a short-term treatment with a combination contraceptive pill containing 0.150 mg desogestrel and 20 mcg ethinylestradiol were evaluated in a group of 17 healthy women. In spite of the low estrogen content, the pill exerted a good cycle control and the incidence of irregular bleedings was low. The minor side effects commonly associated with oral contraceptive (OC) use rarely occurred, and an improvement of premenstrual symptoms was reported during pill intake. As for the different biochemical parameters tested, the formulation induced a significant increase of fibrinopeptide A (FPA) plasma levels. However, the resulting increase of peptide was lower than that induced by pills containing 30 mcg ethinylestradiol. No significant modifications of plasma total cholesterol (T-CH) and low-density lipoprotein cholesterol (LDL-CH) were observed, while triglycerides (TG), high-density lipoprotein cholesterol (HDL-CH) concentrations and the HDL-CH/LDL-CH ratio significantly increased. A significant increase of apolipoproteins AI (Apo AI) and apolipoproteins AII (Apo AII) concentrations was also observed. Moreover, the pill did not alter fasting insulin and glucose levels and their response to an oral glucose tolerance test (OGTT). It may be concluded that this new formulation can be considered acceptable for clinical use, mainly in consideration of the minor or no changes in the biochemical parameters regarded as risk factors for venous and arterial diseases.

Vaginal treatment with a prostaglandin F2 alpha derivative (alfaprostol) for inducing labor in pregnancy at term.

The effectiveness and acceptability of Alfaprostol (an analog of PGF2 alpha) in inducing labor were assessed in 20 pregnant women at term. All subjects had no spontaneous uterine activity before treatment and the mean (M +/- SE) Bishop score was 2.45 +/- 0.21. The drug was administered by vaginal route at the dose of 10 mg every 3 hours. Regular uterine contractions appeared in all patients and delivery occurred in 85% of the patients after a mean time of 9h50min +/- 0h55min following the start of treatment. The mean dose of Alfaprostol utilized to achieve delivery was 29.4 +/- 2.0 mg. No major side effects were noted in the mothers and their fetuses at any time during treatment. Two patients exhibited vomiting. The Apgar score of all newborns at birth was 8 or more. These results suggest the usefulness of Alfaprostol to induce labor in pregnant women at term, as it has oxytocic activity without adverse effects on either the mother or the fetus.

Correlation of CA125 and CA19-9 serum levels with clinical course and second-look findings in patients with ovarian carcinoma.

CA125 and CA19-9 levels were serially evaluated in blood samples from 21 patients during and after integrated surgical and chemotherapeutic treatment for ovarian carcinoma. Serial measurement of CA125 and CA19-9 correlated with clinical course of disease in 89.7 and 72.7% of instances, respectively. The decrease of serum CA125 and/or CA19-9 in the normal range at the end of chemotherapy does not exclude the presence of residual disease, which can be accurately evaluated only by second-look laparotomy. Serum CA125 and/or CA19-9 can raise some months before clinical and ultrasonographic detection of recurrence. CA125 is the most reliable marker in ovarian carcinoma; however, the concomitant measurement of CA19-9 could offer some benefit in the monitoring of patients with this neoplasia.

Pharmacologic induction of multiple follicular development improves the success rate of artificial insemination with husband's semen in couples with male-related or unexplained infertility.

The use of artificial insemination with husband's semen (AIH) as treatment for couples with male-related or unexplained infertility is often disappointing. The aim of the present study was to evaluate whether the induction of multiple ovulation can increase the pregnancy rates in couples treated with AIH. Multiple follicular development was induced by means of clomiphene citrate (CC) plus purified follicle-stimulating hormone (FSH) in 17 couples undergoing AIH for male-related or unexplained infertility. In spite of the long duration of infertility (greater than 6 years), 10 couples (58.8%) achieved pregnancy within 6 months of treatment. This figure was significantly higher than that obtained in 120 couples who underwent AIH either during spontaneous cycles or after induction of follicular development with CC alone. These results suggest that the availability of multiple mature oocytes increases the conception rate during each cycle, as reported for in vitro fertilization (IVF) programs. It is therefore suggested that a trial of induction of multiple follicular development be performed in couples with male-related or unexplained infertility before their inclusion in protocols for invasive procedures, such as IVF or gamete intrafallopian transfer.

Value of the regurgitant volume to end diastolic volume ratio to predict the regression of left ventricular dimensions after valve replacement in aortic insufficiency.

The aim of this study was to assess the value of regurgitant stroke volume (RSV) to end-diastolic volume (EDV) ratio to predict the regression of left ventricular (LV) dimensions after uncomplicated valve replacement in 34 patients with severe pure aortic insufficiency. The RSV/EDV ratio was measured by contrast ventriculography and thermodilution techniques. LV end-diastolic diameter (EDD) was measured pre- and postoperatively by M-mode echocardiography (at a median interval of 3.3 years after valve replacement). LV/EDD decreased from 74 +/- 8 mm to 54 +/- 11 mm (P less than 0.001). Eleven patients had a persistent postoperative LV enlargement (median EDD 65 mm, range 56-100 mm) while, in 23 patients, EDD became normal (median 49 mm, range 40-55 mm). During follow-up, one patient with LV enlargement died of congestive heart failure. Preoperative RSV/EDV ratio was significantly higher in patients with normal postoperative EDD as compared to those with persistent LV enlargement (0.32 +/- 0.06 vs. 0.24 +/- 0.07, P less than 0.005). The best cutoff point of RSV/EDV to predict the normalization of LV dimensions was 0.28. Postoperative EDD remained abnormal in eight out of 16 patients (50%) with RSV/EDV ratio less than 0.29, while it remained enlarged in only three out of 18 patients (17%) with a preoperative RV/EDV ratio greater than 0.28. The other usual preoperative catheterization and echocardiographic variables were equally or less predictive than RSV/EDV ratio. In conclusion, despite the limitations due to the use of different techniques, we confirmed that the RSV/EDV ratio is a potentially useful variable for the assessment of the proper timing of valve replacement in patients with severe isolated aortic insufficiency.

Incidence and prognostic implications of repetitive ventricular complexes during pre-discharge bicycle ergometry after myocardial infarction.

The clinical significance of repetitive ventricular complexes (RVCs) during pre-discharge bicycle ergometry after acute myocardial infarction has been assessed in 408 consecutive patients. RVCs occurred in 32 patients (8%). When compared to patients without RVCs, those with RVCs had a larger infarction, a higher prevalence of heart failure and late sustained ventricular tachycardia or fibrillation, a lower radionuclide ejection fraction and more frequent RVCs during pre-discharge 24-hour electrocardiographic monitoring. During a follow-up 30 patients died, 17 suddenly. Mortality was 15% (N = 5) in patients with RVCs during exercise test and 7% (N = 25) in those without RVCs. Sudden death occurred in only one patient with RVCs. When multivariate analysis was applied to clinical and exercise test data, RVCs during exercise did not predict cardiac mortality independent of variables related to left ventricular function, such as a history of previous myocardial infarction, persistence of heart failure in the late hospital phase and the extent of blood pressure rise during exercise test.

Value of predischarge data for the prediction of exercise capacity after cardiac rehabilitation in patients with recent myocardial infarction.

The aim of this study was to assess whether data related to predischarge clinical examinations, resting radionuclide ventriculography and symptom-limited bicycle ergometry can predict the achievement of a normal exercise capacity after a rehabilitation program in patients with a recent myocardial infarction. The study population consists of 141 consecutive patients who completed a 3-month training program. Patients with heart failure and/or severe angina were excluded. The rehabilitation program included two training sessions weekly during the 3 months. Working capacity (WC) increased from 79 +/- 17% at hospital discharge to 105 +/- 21% of normal values after rehabilitation (P less than 0.001), by 33% on average. Ninety-five patients achieved a normal WC. Conventional predischarge clinical evaluation, resting left ventricular ejection fraction, exercise induced angina, or ST segment depression were not predictive of normal WC after rehabilitation. Predischarge WC was the single best predictor of a normal WC after rehabilitation compared to those with a persistently low WC (84 +/- 15% in patients with normal WC vs 69 +/- 14% in those with persistently low WC, P less than 0.001). Nevertheless, 49% of patients with a baseline WC of less than 80% achieved a normal WC after rehabilitation. No correlation was found between the change of WC after rehabilitation and predischarge WC or ejection fraction. Therefore, the selection of patients for cardiac rehabilitation after a myocardial infarction should be based primarily on clinical grounds. Exclusion based on exercise induced angina, ST segment depression or low resting ejection fraction at hospital discharge or at entry in the rehabilitation program is not justified.
