Defense Mechanism Test (DMT) and Kernberg's theory of personality organization related to adolescents in psychiatric care.

75 adolescent psychiatric patients were diagnosed with the perceptual projective test the Defense Mechanism Test (DMT) and also according to Kernberg's theory of personality organization (PO). The test protocols were scored in respect of 130 DMT variables and analyzed by means of partial least squares (PLS) discriminant analysis. The objective was to try to separate the three types of PO, psychotic (PPO), borderline (BPO) and neurotic (NPO) by means of the DMT and also to compare the results with a similar study in adult psychiatric patients. The results showed that it is possible to separate significantly the three groups of PO. The BPO group seemed to be heterogeneous. The results were fairly similar to those obtained with adult psychiatric patients. The overall results supported the concurrent validity of Kernberg's theory of PO and for the DMT as well. The DMT seems to be a useful diagnostic method in respect of adolescent psychiatric patients.

Daily-diary evaluated side-effects of conformal versus conventional prostatic cancer radiotherapy technique.

Conventional 4-field box radiotherapy technique induces high morbidity for patients with localized prostatic cancer. Using a patient daily diary, the present study compared side-effects after conventional radiotherapy with conformal radiotherapy for prostate cancer. Fifty-eight patients treated with the conventional technique (with or without sucralfate) were compared with 72 patients treated with conformal technique. The patient groups were compared with an age-matched control population. Patients treated with conformal technique were also evaluated regarding acute and late urinary problems. Results showed that patients treated with conformal technique reported significantly fewer side-effects as compared with conventional technique. Patients treated with sucralfate also showed slightly decreased intestinal morbidity in comparison to non-sucralfate group. Acute and late morbidity evaluated by the patients was decreased after conformal radiotherapy as compared with the conventional technique. Sucralfate may be of value if conformal radiotherapy is used for dose escalation in prostatic cancer patients.

Self-assessed sexual function after pelvic irradiation for prostate carcinoma. Comparison with an age-matched control group.

BACKGROUND: Treatment of localized prostate carcinoma is often accompanied by disturbances in sexual function. The patient's own opinion and experience with these problems can be of great importance for his quality of life. In men older than 50 years, disturbances in sexual function are common. Treatment such as radiotherapy (RT), which can induce sexual dysfunction, should be evaluated in relation to the problems in an age-matched population without prostate carcinoma. METHODS: Sexual function was evaluated with a self-assessment questionnaire using linear-analogue scales. The questionnaire was sent to 199 patients with prostate carcinoma, median age 71 years (range, 51-86 years), who had received pelvic RT with curative intent and to 200 age-matched men in northern Sweden. Mean follow-up time after RT was 48 months (range, 24-56 months). RESULTS: The response rate was high: 141 (71%) and 181 (91%) in the control and patient groups, respectively. Field reduction and treatment pause during RT was not associated with decreased problems in the patient groups. A failure to achieve erection was indicated in 12% of the control subjects, 56% of the patients who had received (RT only) and 87% of the RT + castration (RT + A) patients. In general, patients < 70 years treated with RT+A indicated more sexual problems than the RT only patients < 70 years. There was a strong negative correlation between age and sexual problems in the RT + A < 70 years group. However, in patients < 70 years, sexual activity after RT only, was not significantly different from the age-matched control population. CONCLUSIONS: Patients with prostate carcinoma treated with RT only indicated higher levels of sexual dysfunction than age-matched controls. This was most obvious in patients younger than 70 years, although their sexual activity was comparable to age-matched controls. The addition of castration to RT tended to increase sexual problems, especially in patients < 70 years. In men between 70 and 74 years, the maintenance of sexual function seems to be very susceptible to disturbances. For patients older than 74 years, decreased sexual function was not perceived as such a significant problem, despite abolished desire and erection.

Relations between age, metabolic control, disease adjustment and psychological aspects in insulin-dependent diabetes mellitus.

The relations between age, metabolic control, disease adjustment, and psychological factors in boys and girls with recently diagnosed insulin-dependent diabetes mellitus (IDDM) were studied. Older girls had significant higher postremission glycosylated haemoglobin A (Hb AIc) levels (p = 0.008). Girls with more hospitalizations had a lower developmental level (p = 0.05), and had significantly more problems in the behavioural rating (p = 0.05). Boys with more hospitalizations had a more external locus of control (p = 0.01), more difficulties with disease adjustment, more emotional problems, and were also clinically assessed as having more behavioural problems. Boys showing more difficulties in psychological adjustment to the disease also had higher postremission Hb AIc levels (p = 0.02). Although Swedish children with IDDM of short disease duration do not differ from healthy children in important psychological aspects, older girls and a small group of problematic younger boys are at risk of developing metabolic imbalance after a short disease duration.

Self-assessment questionnaire for evaluating urinary and intestinal late side effects after pelvic radiotherapy in patients with prostate cancer compared with an age-matched control population.

BACKGROUND: Pelvic irradiation to patients with prostate cancer is accompanied by urinary and intestinal reactions. In men older than 60 years, treatment-induced problems should be evaluated in relation to problems in an age-matched nonirradiated population. METHODS: In the present study, problems in the urinary tract and intestine were evaluated with a self-assessment questionnaire using the linear-analogue scale. The questionnaire was mailed out to 200 patients and to an age-matched population 24-56 months after irradiation. RESULTS: Twenty-five percent of the control group and 50% of the patient group reported some kind of problem in the urinary tract. The most common urinary problems in the control group and in the patient group, respectively, were urgency (19 and 42%), starting problems (22 and 33%), and leakage (11 and 32%). In the control and patient groups, 14 and 59%, respectively, reported some kind of gastrointestinal problems. The most common intestinal problems in the control and patient groups were respectively, mucus (4 and 38%), cramp (5 and 14%), leakage (2 and 27%), and blood (2 and 36%). Ninety percent of the patients' problems were minor. CONCLUSION: Pelvic irradiation induced a relatively large number of minor problems, evaluated with a self-assessment questionnaire and compared with an age-matched population of men, of approximately similar magnitude as with a physician's systematic evaluation. The most important urinary factors were urgency and leakage. The most important intestinal factors were blood, mucus, and leakage. The results support the ongoing efforts to use 3-D computed tomography-based conformal therapy to decrease irradiation dose to the rectum and bladder.

Psychosocial aspects of type 1 diabetes mellitus in children 0-14 years of age.

A total clinic sample of 67 diabetic and their parents were followed prospectively together with a referent group in this longitudinal study of psychosocial factors in childhood diabetes. Measurements of psychosocial stress showed statistically more negative life events and coping problems in the diabetic group during the first two years of life. The crisis/coping pattern at onset of the disease varied but most parents/and children scored low distress already after 4 weeks. High distress at onset correlated to high distress at a twelve month follow-up interview. Among the parents planful problem solving and distancing were the most prevalent coping mechanisms. The diabetic children had a rather good adaptation to the disease after 12 months of disease duration. Psychiatric problems were not more common in the diabetic children compared to a normal group. Metabolic imbalance correlated to external locus of control, low development quotient and many life events.

Personality organization defined by DMT and the Structural Interview.

The psychoanalytic concept of Personality Organization (PO) may be operationalized by means of a Structural Interview as well as by means of psychological tests. The present study utilized the Structural Interview and a projective test called the Defence Mechanism Test (DMT) to operationalize the PO concept on a sample of 50 psychiatric inpatients. The reliability of the PO judgements were found to be acceptable for both the Structural Interview and the DMT. The validity, which was estimated as the correlation between the two methods, was also found to be substantial. The conclusion is that the concept of PO may be reliably operationalized for psychiatric patients and seems to have concurrent validity. Both the DMT and the Structural Interview may be used for differential diagnosis of PO.

A comparative FLASH and EPI study of repetitive and sustained visual activation.

Functional responses to either brief repetitive or sustained activation of the human visual cortex (movie presentation) were monitored using both fast low angle shot and echo planar imaging sequences. To allow for proper comparisons, native image contrasts were equally sensitized to changes in cerebral blood oxygenation with other experimental conditions matched as much as possible. Putative influences of receiver bandwidth and absolute voxel size were specifically addressed. In all cases resulting correlation maps and regional signal intensity time courses showed excellent spatial and temporal congruence, respectively. In particular, for a 6 min protocol of sustained activation, both FLASH and EPI yielded an initial signal increase (oxygenation overshoot), a subsequent signal decrease during ongoing stimulation, and a marked signal drop (oxygenation undershoot) after the end of stimulation. These findings exclude technical differences between FLASH and EPI as the source of previous contradictory observations more likely to be explained by differences in stimulus design.

The binaural interaction component in human ABR is stable within the 0- to 1-ms range of interaural time differences.

The effect of unilaterally delayed acoustic stimuli on binaural interaction was studied in 12 subjects with normal hearing. Auditory brainstem response (ABR) was obtained in the midline between the forehead and the neck, and click stimuli were unilaterally delayed at 0.2-ms intervals in the 0- to 1-ms range. Binaural interaction was evaluated by measuring the beta wave in the binaural difference waveform, i.e. the remainder after subtracting the binaurally evoked registration from the sum of the two monaural registrations. Computation of the binaural difference waveform showed the beta wave amplitude to be relatively stable throughout the 0- to 1-ms range of interaural time differences. This finding suggests stable binaural interaction within the range of interaural time differences where binaural click stimuli induce a binaurally fused intracranial image.
