Nursing summary and experienced workload.

Nursing summary and experienced workload form a new approach to patient documentation. The data used for this documentation are provided by nurses without being subjected to a formal nomenclature. It is important to highlight patient complexity, nursing difficulty and to evaluate workload. The emphasis on these concepts provides data which allow qualitative and quantitative assessment of the patient's entire stay. After a discussion of the philosophical and economic aspects that are the basis of the methodology, this paper focuses on the design and the development of a nursing database.

[Performance appraisal of internal health services].

In the present context of scarce resources, satisfying at best essential needs in the area of health care leads gradually towards a new conception of the management of health services and towards the development of information tools designed to evaluate performance. This paper presents a general framework for that purpose; it can be adapted to more specific and contextual problems. The authors review the difficulties involved in the measure of performance, the basic data required by the methods as well as some indicators that can be derived from their application, all exemplified in a case study.

[Nature, frequency and variability of health counseling by 7 pediatric physicians ].

A prospective study of health counseling was conducted in 7 private pediatric ambulatory settings. The frequency, duration, differences and contents of these advices are given. These latter were classified as systematic (given by the pediatrician), given (occasional), requested, preventive, or therapeutic, for statistical analysis. 1,703 counsels were registered during 1,503 consultations. 828 of them health counsels (HC) and will be reported here. Educational HC were the most frequent, followed by those concerning therapy, hygiene, immunization and nutrition. The mean counsel duration was 2 minutes. Their frequency and contents varied greatly among the 7 pediatricians. The content of the immunization counsels only was homogeneous. 22% of the HC were systematic, when 47% were requested. A better teaching of health counseling would further improve the credibility and efficacy of health counseling in pediatric practice.

[Program for a medical information system at the Public Welfare Hospitals in Paris].

The authors present the adaptation of the so-called "Program for a Medicalized Information System" using the clinical complexity of the patient, his nursing weights and the expenses directly imputed. A critical analysis of the "Diagnosis Related Groups" method is shown and the new form of "Normalized Complexity Index" proposed. Some issues explain the advantages of this new method.

Streptococcal pharyngitis-tonsillitis in Swiss children. Diagnosis and management.

In a prospective study, during a period of 1 year, 1,116 children over 1 year of age, with acute pharyngitis-tonsillitis, were examined by a group of Swiss pediatricians. Throat swabs were cultured for the presence of group A beta-hemolytic streptococcus, and treatment was withheld pending results in most cases. The throat swab cultures were positive in 29.6% of the cases. There were no incidences of acute glomerulonephritis, acute rheumatic fever or severe purulent local complications. The findings confirm the recommendations in the American literature concerning clinic, diagnosis and management, and they can be applied in Switzerland.

What seven pediatricians in middle Europe observed and did in their office practice from August 1979 to March 1980.

The authors present the systematization of medical documentation in pediatric practice checked with 5,721 recordings of consultations. General statistics and comparative statistics of 7 pediatric offices are shown. The system serves today as basic documentation for scientific retrieval and is planned to be integrated in an on-line medical information system providing features as patients' follow-up, indications of laboratory values, treatment, and facturation.

Short treatment of urinary tract infections in children.

111 episodes of acute urinary tract infection in children without urinary tract malformations were treated by trimethoprim-sulfamethoxazole, either during 3 days or during 7-10 days. Cure rate attained almost 100%. Recurrence rates of the first infection were similar after short treatment (29.6%) and after conventional treatment (30%). It is concluded that uncomplicated urinary tract infection in ambulatory practice may be treated safely and successfully by only 3 days of an appropriate antimicrobial.

Urinary tract infections in pediatric practice. A comparative study of three diagnostic tools: dip-slides, bacterioscopy and leucocyturia.

1,261 fresh urine specimens of patients of a pediatric office were examined by optical microscope and cultured on dip-slides (Urotube). 132 pathological bacteriurias, representing 95 certain episodes of UTI, served as reference material. The bacteriuria, leucocyturia and bacterioscopy were analysed by means of four validity tests: sensitivity test, predictive value of negative test, specificity test, and predictive value of positive test. The comparison shows that bacterioscopy is as valuable as dip-slides, provided that (a) only the clearly or highly positive bacterioscopies, or (b) the associated bacterioscopy and leucocyturia--but only double negative or double positive--be considered.

Accidents in children: the potential contribution of the pediatrician to the field research approach.

Preventing injuries means using epidemiological knowledge of accidents. In order to increase this knowledge on accidents in childhood, field research is badly needed, in addition to other approaches. First, a comparison of the main characteristics of three of these approaches is presented. Second, the authors try to demonstrate why, in their opinion, the contribution of the practising pediatrician in this kind of applied research is to be considered as: (a) interesting for him; (b) fruitful in terms of data gathering and analysis, and (c) useful for all children and their family in terms of future, more specific preventive action. Some preliminary results of the data systematically collected by the members of GREPA illustrate this position taken by the authors.

Testing for bacteriuria by home culturing in preschool girls.

Home urine culture in dipslide was performed in 297 preschool girls. Results correlated well with those of cultures at the medical office, specially if midstream urine was used. This method proved to be an inexpensive, reliable (100% sensitivity) technique for detecting significant bacteriuria in preschool girls. False-positive tests occurred in 22 cases.
