[Clinico-anatomic study for constructing pedicled myocutaneous trapezius flaps].

The authors investigated musculocutaneous flaps, paying particular attention to vascular anatomy, the development and length of their main vessels, their potential arc of transposition. Beyond that, their applications for soft tissue coverage in the head and neck region and procedures for coverage of the donor site were studied. When compared with the literature, the results show that pedicled musculocutaneous and osteomusculocutaneous trapezius flaps are especially suited for reconstructive surgery of the head and neck.

Therapeutic outcome of patients suffering from malignant melanomas of the conjunctiva.

Eighty-one cases of conjunctival melanoma treated between 1960 and 1988 were studied to determine factors that might affect outcome in patients with such lesions. The therapeutic procedures performed were local excision (16), local excision followed by brachytherapy with Sr-90/Y-90 (32), local excision followed by cryotherapy with liquid nitrogen (16), brachytherapy with Sr-90/Y-90 (12), local excision followed by external beam irradiation (3), and local excision followed by brachytherapy and cryotherapy (2). The median follow-up period was 5.5 years (longest 26, shortest 1 year). Sixty two patients (76.5%) showed a complete regression of the melanoma, 19 (23.5%) developed recurrences, and 15 (18.5%) died from metastases. The melanomas had developed with almost equal frequency from a pre-existing naevus (25.9%), from primary acquired melanosis (25.9%), and 'de novo' (30.9%). Small tumours had a higher chance of regressing (80.6%) than larger ones (68.6%). The cumulative survival rate was 76% after five years and 60% after 10 years from any causes of death and 87.6% after five years and 76.3% after 10 years from deaths caused by metastases. Most deaths from metastases occurred within 5 years. At 88.5%, the cumulative survival rate of patients with small tumours (less than one quadrant of the bulbar conjunctiva and less than 2 mm thickness) was significantly higher than that of patients with larger tumours (more than one quadrant of the bulbar conjunctiva and/or more than 2 mm thickness) with 65% after eight years. Local excision followed by beta ray irradiation (Sr-90/Y-90) or cryotherapy can be recommended as the treatment of choice. Nevertheless the behaviour of conjunctival melanomas remains unpredictable in individual cases.

[Anatomic studies of the availability of the transverse cervical artery as the stem vessel of pedicled myocutaneous flaps in plastic surgery].

The course and the circulation area of the A. transversa colli were characterized preparatorically as well as with the help of injections of stains and angiographically with contrast media in corpses. Predictions concerning the formation of the angiosomes and thus the size and the shape of musculocutaneous trapezius flaps were derived from the investigations. The presentation of the specific topographic relations of the A. and V. transversa colli revealed indications concerning the formation of the vascular pedicle and thus the possibilities to transpose the musculocutaneous flaps in order to cover skin defects of the head, neck and thorax after extensive surgical intervention.

[Echocardiography findings before and following beta-irradiation (106Ru/106Rh) of choroid melanoma].

Echographic follow-up examinations were performed in 72 patients in whom a choroidal melanoma was treated with (106Ru/106Rh) applicators. The observation period was between 6 and 15 months. Echographic measurement of tumor prominence and assessment of the degree of reflection proved to be important indicators for judging the success of beta radiation therapy. If the prominence diminishes and the degree of reflection increases after treatment, regression of the tumor may safely be assumed to have occurred. Apart from an increase in the prominence of the tumor, a constantly low or even falling level of reflection are signs of a less favorable prognosis.

[Oculopression tonometry (OPT): a new tonographic procedure in the diagnosis of glaucoma].

OPT is a new tonographic procedure permitting easy and reproducible examination of the regulative IOP behavior and the detection of impaired hydrodynamics as an early sign of glaucoma. OPT avoids errors associated with conventional tonographic procedures, takes measurements in IOP ranges where better separation between healthy and glaucoma eyes may be expected, performs simultaneous examination of both eyes, and permits IOP measurement by applanation tonometry in all phases of the examination process. OPT is easier to perform, is better tolerated by the patient and only takes half as long as conventional tonography. The effectiveness of the new procedure is demonstrated by the results of examinations of 253 eyes.
