[Endoscopy in the diagnosis and treatment of abdominal adhesions].

Therapeutic and diagnostic laparoscopy was performed in 48 patients with chronic abdominal pain due to an adhesive process in the abdominal cavity. Laparoscopic treatment was applied only in patients who experienced pain when the adhesions were pulled with instruments. The adhesions were divided, the vessels were coagulated, and the organs were separated from the adhesions with instruments introduced into the abdominal cavity through additional punctures made in the anterior abdominal wall. The immediate results of the treatment were good in 44 out of 48 patients (91.6%). The late-term results were studied in 46 patients in follow-up periods of 12 months to 6 years. Recurrences were found in only 4 cases. Complications requiring special treatment were not encountered. Therapeutic and operative endoscopy is an effective and safe method for treating patients with adhesive disease manifested by abdominal pain.

[Assessment of endoscopic, cytological and histological diagnostic methods in stomach polyps].

The results of endoscopic and morphological diagnostic methods for gastric polyps in 91 patients are analysed in detail. It is concluded that endoscopic examination makes it possible to reveal gastric polyps of any localization. However, to solve the problem of therapeutic policy it is necessary to examine the biopsy material from polyps, prior to their removal, both cytologically and histologically. This is due to the fact that small size polyps after endoscopic polypectomy are no longer suitable for morphological assay, whereas cytograms obtained from the transverse sections and surface of large polyps are equivalent diagnostically to cytograms of the biopsy specimens.

[Laparoscopic diagnosis of intra-abdominal metastases of extra-abdominal tumors].

In 67 of 227 patients endoscopy revealed intraabdominal metastases or extensive cancer dissemination. Endoscopic findings made it possible to reject surgery in 35 patients and to recommend but not extensive palliative operations in 38 patients. The remaining 154 patients were subjected to radical operations. Laparoscopy was shown to be a valuable diagnostic method permitting to support to pinpoint further and, in a number of cases to detect abdominal metastases, thus obviating the need for extensive and unnecessary operations in many cases. The data obtained in laparoscopy are comprehensive and precise, their diagnostic value being not inferior to those obtained during the operation.
