Overexpression of p53 protein in basal cell carcinomas of human skin.

Basal cell carcinoma (BCC) of the skin is the most common human cancer, but its molecular-genetic pathogenesis is unclear. In many other types of cancer, mutations of the tumor-suppressor gene p53 occur frequently and may lead to overexpression of a long-lived mutant form of p53 protein. In this study, overexpression of p53 protein was detected immunohistochemically in 30 (83%) of 36 specimens of BCC of the head and neck. The same regions of tumor typically were reactive both with a monoclonal antibody (PAb240) specific for the mutant protein and with one (PAb1801) directed against an epitope common to both wild-type and mutant p53 protein. Keratinocytes of chronically sun-exposed epidermis adjacent to BCCs also focally overexpressed p53 protein in the majority of cases, whereas those of sun-protected buttock skin did not. Mutation of p53 may form an important part of the pathogenetic sequence in a majority of cases of BCC.

Kaposi's sarcoma occurring in a dermatome previously involved by herpes zoster.

A 31-year-old black man with a history of intravenous drug abuse developed a mass in his neck, a biopsy of which revealed Kaposi's sarcoma. The patient underwent radiation therapy, and the mass diminished in size. Approximately 2 months later the patient developed a herpes zoster infection in the left T3 distribution. The vesicular eruption resolved, but postherpetic neuralgia remained. Two months after the herpes zoster infection, the patient developed many small nodules in the area of the prior vesicular eruption. Biopsy revealed these nodules to be Kaposi's sarcoma. At this time no other cutaneous lesions were present. We believe that these nodules represent the occurrence of the Koebner phenomenon in a patient with Kaposi's sarcoma and the acquired immunodeficiency syndrome.

Importance of complete cutaneous examination for the detection of malignant melanoma.

With the rate of melanoma increasing 1,000% in the past 50 years, the early detection of the disease is becoming more important. Data from 2,239 persons seen at the Manhattan Melanoma/Skin Cancer Detection Screening Program were analyzed to determine if a complete cutaneous examination would significantly increase the chance of detecting melanoma. Thirteen of the fourteen melanomas detected were on anatomic sites normally covered by clothing. Patients having complete skin examinations were 6.4 times more likely to have a melanoma detected than those having partial examinations (p = 0.025). These findings reinforce the importance of complete skin examination for the early detection of malignant melanoma.

Cutaneous histologic findings in chemical workers with and without chloracne with past exposure to 2,3,7,8-tetrachlorodibenzo-p-dioxin.

Histopathologic examination of the skin in seventy-seven chemical workers with past exposure to 2,3,7,8-tetrachlorodibenzo-p-dioxin (TCDD) was performed in 1981 to determine whether changes specific for chloracne were present. Prior to biopsy, subjects had been classified as having clinical evidence of residual chloracne (48%), having a past history of chloracne but no current dermatologic evidence of it (24%), or never having had chloracne (27%). Follicular infundibular dilatation and/or comedones and cysts were found in 76%, 39%, and 32% of the current, past, and never chloracne groups, respectively; the differences were significant between current and never groups (p less than 0.001) and between current and past groups (p less than 0.01). A combined mean score of follicular infundibular dilatation, comedones, and cysts was significantly different only between the current and never groups (p less than 0.005). Solar elastosis, histologically present in all subjects, was related to age but not chloracne. No relationship was found between smoking habit and any of the histologic findings. Changes specific for chloracne were not found. Four-millimeter skin biopsy is unlikely to be either diagnostic or useful in persons with past exposure to TCDD that is ill defined or insufficient to produce chloracne.

Metastatic Crohn's disease.

Cutaneous granuloma formation distant from the gastrointestinal tract in patients with Crohn's disease of the bowel has been called metastatic Crohn's disease. We report two patients with this entity, including the first to present with an erysipelas-like eruption of the face. A review of the world's literature reveals that all patients with metastatic Crohn's disease have had gastrointestinal disease involving the colon or rectum. Clinical features of the nine previously reported cases are reviewed.

Sebaceous gland neoplasia.

Neoplasms with sebaceous differentiation cover a broad spectrum. They range from benign sebaceous hyperplasia and adenomas to locally infiltrative sebaceous epitheliomas which have biologic behaviors similar to basal cell carcinomas, to sebaceous carcinomas which can have very malignant biologic behaviors. In this paper, we have purposely excluded the nevus sebaceus of Jadassohn, a nevoid tumor composed of several cutaneous elements. As sebaceous glands are only one part of this tumor, it is better classified as an organoid nevus.

Nevus lipomatosus cutaneus superficialis. A light and electron microscopic study.

Nevus lipomatosus cutaneus superficialis is a rare idiopathic abnormality characterized by isolated dermal collections of adipose tissue. The lesions usually are either congenital or present by the third decade of life and occur predominantly in the pelvic-girdle area. We saw a case that was unusual because the nodules present arose in the patient's fifth decade of life and were confined to the abdominal wall. Histopathologic examination of the nodules showed mature adipose tissue, perivascular mononuclear cell infiltrates, and numerous mast cells. By electron microscopy, we observed small mature lipocytes within the perivascular mononuclear infiltrates. However, neither lipoblasts nor the transition of mesenchymal cells to lipocytes could be demonstrated.

Verrucous lupus erythematosus: ultrastructural studies on a distinct variant of chronic discoid lupus erythematosus.

Ten patients with classic discoid lupus erythematosus of the face associated with verrucous, papulonodular lesions on the arms and hands were studied by electron microscopy. The ultrastructural findings on the verrucous lesions included apoptotic keratinocytes, intraepidermal lymphocytes, and gapping, detachment, and reduplication of the basal lamina. Also, tubuloreticular inclusion bodies were present in the endothelial cells. These observations, together with the clinical, histopathologic, and immunofluorescence findings, suggest that the verrucous lesions represent a rare, but distinct, variant of chronic discoid lupus erythematosus.

Skin metastases from small-cell carcinoma of the lung.

This paper reports the first case of skin metastases from small-cell carcinoma of the lung with electron microscopic confirmation. The 2 to 3-cm cutaneous lesions present on the chest and limbs were hard, nontender, smooth-surfaced, freely moveable nodules with normal appearing overlying skin. Characteristic dense-core granules 1562 +/- 123 A (SEM) in diameter were detected by electron microscopy. The detection by electron microscopy of dense-core granules may assist in confirming the diagnosis of small-cell carcinoma in cutaneous lesions with equivocal histologic findings.

S-phase fractions of colorectal carcinomas related to pathologic and clinical features.

The S-phase fractions (SPFs) of epithelial cells in 100 resected colorectal carcinomas were measured by in vitro exposure to tritiated thymidine and autoradiography. The frequency distribution of SPFs was gaussian with a median of 17.8 per hundred in 90 unirradiated carcinomas, whereas in ten carcinomas given radiation therapy preoperatively, it was positively skewed with a median of 6.9. Analysis of the unirradiated carcinomas showed no relationship between SPF and various clinical and morphologic features that included age, race, sex, site, size, Dukes' stage, histologic grade of the tumor, number of metastasis-bearing regional lymph nodes, presence of adenomas of the large bowel, survival or relapse-free survival of the patient, or SPF or adjacent normal colorectal crypts. The results show no evidence that colorectal carcinomas can be divided into kinetic subsets. The spatial orientation of labeled cells in autoradiographs indicated presence of a nonproliferative fraction of cells in many tumors that may modulate response to radiation therapy and chemotherapy.

New fine structural features of cardiac rhabdomyoma: report of a case.

Electron microscopy of a cardiac rhabdomyoma revealed two varieties of cells: (1) typical "spider cells' characterized by few myofibrils and large aggregates of glycogen particles free in the cytoplasm; and (2) cells with more myofibrils, less glycogen, and glycogen within membrane-limited vacuoles. These differences in myofibril development and glycogen content represent two succeeding stages in the early maturation of normal cardiocytes. The vacuoles containing glycogen were interpreted as autophagic and, consequently, active in the glycogen decrease typical of normal maturing cardiocytes. Rhabdomyoma cells did not show the structures, e.g., T system, characteristic of the late normal heart development. This study supports the theory that arrested cardiocyte maturation is a factor in the pathogenesis of cardiac rhabdomyoma.
