Case management by nurses at a county facility.
A case management system using registered nurses as case managers was implemented at one clinic operated by San Francisco General Hospital in an effort to improve the continuity and quality of care provided. The program was targeted to serve medically unstable patients with complex care needs who often experienced avoidable health problems requiring expensive medical care. Clinic nurses, who are trained in case management through ongoing education seminars, are responsible for identifying patients in need of case management, developing and implementing care plans, and monitoring and documenting patient progress.
