[Infections by Arcanobacterium haemolyticum: an emerging pathogen].

Arcanobacterium haemolyticum is a grampositive rod wich belonged, until a short time ago, to Corynebacterium genus, and recently classified in a new genus, with only one specie. Human is the main reservoir. It has been isolated from the skin and pharinx of healthy individuals, but also it is cause of infection, specially pharingitis, in children, and chronic cutaneous ulcus, in diabetic patients. Less frequently, it is cause of osteomyelitis, meningitis, pneumonia, abscess, endocarditis and sepsis. Diagnosis is difficult because its double quality: comensal and pathogen. There are not established guidelines for the treatment of these infections, although most of isolated strains are susceptibles to penicillin, erythromicin, clindamycin and tetracycline. High doses of penicillin, with or without gentamicin, it is recommended for the treatment of deep infections.

[Study of the susceptibility of black yeasts of the Exophiala genus to antifungals using the Sensititre System].

Black yeasts belong to a group of dematiaceous fungi which are difficult to classify and identify. One of the genera that cause human infection, mainly phaeohyphomycosis, is Exophiala, which includes the species E. berger, E. castellanii, E. dermatitidis, E. jeanselmei, E. lecanii-corni, E. moniliae, E. pisciphila, E. salmonis and E. spinifera. We carried out a susceptibility study of five of these species (E. castellanii, E.dermatitidis, E, jeanselmei, E. lecanii-corni and E. moniliae), isolated from cutaneous exudates, to the usual antimycotic agents. Strains were cultured on Sabouraud agar and potato-dextrose agar. Species identification was made by conidiogenous microscopic observation and the following characteristics: maximum growth temperature, tolerance to cicloheximide and carbohydrate assimilation. Susceptibility to antimycotic agents (amphotericin B, fluconazole, itraconazole, ketoconazole and 5-fluorocytosine) was determined by the Sensititre YeastOne (Aamar Blue) commercial method. Amphotericin B activity was good for the five tested species, although the MIC for E. jeanselmei was high (1 mg/l. MICs for fluconazole were high for all the species, but only E. moniliae and E. lecanii-corni c be considered resistant. Activity of itraconazole, ketoconazole and 5-fluorocytosine was good.

Extracellular enzymatic activity in 11 Cryptococcus species.

The extracellular enzymatic activity of 36 strains of yeast belonging to 11 species of the genus Cryptococcus, has been investigated, using the API-ZYM (BioMérieux, France) commercial system, with the objective of determining the differences in the enzymatic profiles of the various species. The strains studied were: 9 of C. neoformans, 7 of C. albidus, 6 of C. laurentii, 5 of C. uniguttulatus, 3 of C. humicolus, and 1 each of C. ater, C. curvatus, C. dimennae, C. hungaricus, C. infirmo-miniatus and C. magnus. All the strains showed enzymatic activity with positivity to Phosphatase alkaline, Esterase lipase C8, Leucine arylamidase, Phosphatase acid and Naphthol-AS-BI-phosphohydrolase, and negativity to Lipase C14, Trypsin, Chemotrypsin, beta-galactosidase, beta-glucuronidase and alpha-manosidase. Variable enzymatic activity was shown to Esterase C4, Valine arylamidase, Cystine arylamidase, alpha-galactosidase, alpha-glucosidase, beta-glucosidase, N-acetyl-beta-glucosaminidase and alpha-fucosidase. This allowed 11 separate enzymatic patterns to be established. The species C. neoformans and C. laurentii each presented two distinct patterns; C. uniguttulatus, C. hungaricus and C. magnus shared the same pattern; C. albidus, C. ater, C. curvatus, C. dimennae, C. humicolus and C. infirmo-miniatus presented an individual enzymatic pattern. The results obtained suggest that the API-ZYM system could be useful for the identification of species of the genus Cryptococcus and for the differentiation of the enzymotypes for epidemiological purposes.

[The gas-liquid cromatography with mass spectrometry for the identification of yeasts].

Methods based on gas chromatography, have been used for identification of the yeasts. In order to know the value of the patterns obtained by this method, we have used this technique and mass spectrometry on 44 strains belonging to 16 genus and 21 species of collection yeasts, identifying the corresponding peaks to 22 fatty acids methyl esters by means of the reaction times of the corresponding standards and the confirmation of molecular weigh by mass spectrometry. The correlation coefficient was of 0.848965. The chromatographic technique seems of great utility for the determination of lipidotypes.

Microbiological characteristics and susceptibility patterns of strains of Rhodotorula isolated from clinical samples.

The members of the genus Rhodotorula show a marked ubiquity. In man, they have been isolated from faeces, nails, skin, sputum, digestive tract and adenoids, forming part of the normal human flora, although in recent years cases have been reported of both local and systemic infection by this yeast. There are virtually no studies in the literature on the sensitivity of this genus to the antifungal agents in common clinical use. Therefore, it is considered of interest to study the microbiological characteristics and the susceptibility patterns of Rhodotorula isolated from clinical samples. A total of 35 different strains of Rhodotorula were studied. In vitro susceptibility testing to 5-fluorocytosine, amphotericin B, ketoconazole, fluconazole and itraconazole was performed. All the strains were considered sensitive to 5-fluorocytosine, amphotericin B, ketoconazole and itraconazole and resistant to fluconazole. As a conclusion, we can state that all the antifungal agents tested, except fluconazole, are useful medicaments for the treatment of infections by the Rhodotorula genus.

Case of onychomycosis caused by Microsporum racemosum.

We report the case of a Spanish 60-year-old female who presented in 1997 with onychomycosis of the left thumbnail following an injury caused by a fresh fish bone. Microsporum racemosum was repeatedly cultured from nail scrapings, and its identity was confirmed by sequencing the isolate's ITS1/ITS2 and 5.8S rRNA regions. The patient was successfully treated with itraconazole, which was administered for 12 weeks. This represents the first case of onychomycosis due to M. racemosum and the first time that this species has been isolated from a human in Europe.

[Usefulness of the CHROM-agar Candida culture medium in the differentiation and presumed identification of yeast of clinical interest].

BACKGROUND: The efficiency of a novel differential culture medium, CHROM-agar Candida, is evaluated in order to differentiation and presumptive identification of clinically important yeasts. METHODS: We studied 600 clinical yeast strains, pertaining to 8 genera (Candida, Cryptococcus, Geotrichum, Rhodotorula, Trichosporon, Kluyveromyces, Pichia and Saccharomyces) and 27 species. Strains were previously identified by conventional methods and by commercial system ATB ID32C (Bio-Mérieux). RESULTS: Four species (C. albicans, C. krusei, C. tropicalis and t. beigelii) were presumptive recognized after 48-72 hours of incubation, regarding their colonial morphology and color. Colony color of other species showed low specificity. CONCLUSION: CHROM-agar Candida medium is recommended for recognition of mixed yeast cultures and differentiation of the common clinically yeast species.

[Urinary tract infection caused by Haemophilus spp. in pediatrics: a rarely studied disease].

BACKGROUND: Haemophilus influenzae and Haemophilus parainfluenzae have been, in very seldom times, described as a cause of urinary tract infection (UTI), mainly on adults. Their real incidence has been scarcely studied. In view of this situation, we have investigated the cases occurred during the year of 1994 in our hospital, in Cádiz (Spain). METHODS: A total of 16,446 urine cultures from in-patients and ambulatory patients were processed, including the chocolate agar medium in the cultures. RESULTS: Urinary infection by Haemophilus supposed an incidence of 0.27% with respect to all positive urine cultures, and a 0.88% of the paediatric ones. H. influenzae was isolated in 7 occasions, and H. parainfluenzae in 3 of them. The 80% of the cases of UTI corresponded to paediatric patients. CONCLUSIONS: It would be convenient the inclusion of chocolate agar medium in the urine cultures for determined patients, chiefly on children.
