[The role of infection and inflammation in the pathogenesis of atherosclerosis].

Pathogenesis of the atherosclerotic process is deemed as multifactorial. To the most important risk factors, besides certain family predisposition, there belongs hypercholesterolemia, arterial hypertension, obesity, diabetes mellitus, smoking and others. In the last years there are more and more data about the role of inflammation and infection in the whole development of atherosclerosis. The witness for this hypothesis is the findings of high parameters of inflammation in involved vessels as well as in the blood of atherosclerosis suffering persons. Opinions about the inflammation theory appear from the 90th. Local sterile inflammation in the subendotelium of the middle and big arteries has been proved to consist of specific immune reaction (activation of the T-lymphocytes) as well as nonspecific characteristic by elevated monocytes in the artery wall during the whole process of atherogenesis. Inflammation in the plaque can trigger and hold several factors engaged in the atherosclerotic process, such as oxidized LDL cholesterol, elevated production of various superoxides, activated macrophages, activated T-lymphocytes, cytokines (IL-1, IL-6, interferon gamma) and lipoprotein Lp (a). In this inflammation process levels of CRP (acute phase protein), fibrinogen and erythrocyte sedimentation are elevated as a reaction of the organism to nonspecific chronic infections. Because of this it is thought that elevated fibrinogen and erythrocyte sedimentation are markers of the cardiovascular risk. Some papers deal with antiinflammatory effects of statins, because these lower CRP levels so they also lower atherosclerotic risk through not only lowering of cholesterol levels. Also asprine, as an antiinflammation agent, changing the CRP levels, would be of benefit for patients with vascular disease because its antiaggregation and antiinflammatory effects. ACE inhibitors are also antiinflamatory through blocking of tissue production of angiotensin II (artery wall and atherosclerotic plaque). Enzymatic inhibitors changing angiotensin can also have a partial antiinflammatory effect. The infection theory is supported also by tracing of some microorganisms in the atherosclerotic plaque or in the blood, as e.g. Helicobacter pylori or Chlamydia pneumoniae; to the autoimmune origin is indicated the presence of the specific immunity reaction against heat shock proteins (HSP) or oxidized LDL. This infection theory offers new therapy possibilities. Therefore eradication for example by antibiotics can lead to stabilization of the atherosclerotic plaque with positive consequences, as it was discovered by many studies.

[Choroid cysts--laser therapy].

The author presents the survey of cysts of choroid classification. He covers a case of serous cyst of iris of a young man. The cyst caused the vision disorder because of pressure on the front capsule of lens. The successful therapeutical method was a laser photocoagulation. There was not find any recurrent disease during the six years of monitoring.

[Iridoschisis--case report].

The authors present an account of a 78-year-old female patient with bilateral iridoschisis. The disease was present in the lower portions of the iris. The pupil was centred, mobile. Part of the ruptured fibres of the iris floated freely in the anterior chamber. The cornea was not decompensated. The patient had open angle glaucoma and cataract. The method of choice in the treatment of the condition is laser iridotomy or microsurgical iridectomy and extraction of the cataract with implantation of PC IOL. Conservative treatment attempts to prevent possible damage of the optic nerve.

[Prevention and treatment of mushroom poisoning].

In the submitted review the author gives an account on basic aspects of primary, secondary and tertiary prevention of mushroom poisoning. Mushroom poisoning is not caused only by toxic mushrooms (true intoxications) but under certain conditions also by edible mushrooms (false intoxications and pseudointoxications). The basis of primary prevention is not to pick and eat mushrooms which could damage health, and knowledge of basic facts, and types of mushroom poisoning. The main objectives of secondary prevention of mushroom poisoning are to prevent absorption of toxins from the digestive tract into the human organism and their effective elimination from the organism.

[Differential diagnosis of mushroom poisoning].

Mushroom poisoning is a very urgent problem of internal medicine. Mushroom picking is one of the most popular hobbies in the Czech and Slovak Republic. This is one of the main reasons of the rising incidence of mushroom poisoning, incl. fatal cases. Mushroom poisoning is characterized by a very varied clinical picture which is reflected also in recent classification systems. The main objective of the present work is to draw attention to cardinal symptoms and signs of different types of mushroom poisoning which respect new findings in the diagnosis and differential diagnosis of mushroom poisoning.

[Prevention and treatment of unstable atherosclerotic lesions in atherosclerosis].

Primary, secondary and tertiary prevention and treatment of atherosclerosis and unstable atherosclerotic lesions as well as of their sequelae must be comprehensively angioprotective (comprehensively antiatherogenic), above all antilipidogenic (antihyperlipidaemic), antithrombogenic (antithrombotic) and endotheloprotective (cytoprotective). It is important to use an individual approach, differentiated with regard to actual results of basic and functional physical, instrumental and laboratory angiological examination which must be made at frequent, regular intervals. Prevention and treatment must not be alternatives substituting each other.

[Angiology as a most important specialty and an integral part of internal medicine].

The author wishes to remind the broad medical public, in particular specialists in internal medicine, that vascular diseases are the most widespread, most serious and most urgent medical, social and economic problems of all countries in the present world. They cause a high morbidity, associated with work incapacity, disability and about half of all deaths in the population, i.e. twice as many as the second most frequent cause of death--oncological diseases (angioepidemy). Vascular diseases and their risk factors are internal diseases which usually effect several organs and tissues of the human organism. The author refutes disintegration of internal medicine and explains the integral relationship of angiology and internal medicine.

[The Pelli-Robson contrast sensitivity test].

The author gives the first information on the practical experiences with the new examining method finding out the sensitiveness to contrast according to Pelli-Robson at ophthalmology department in Trnava. This examining method allows to gain more complex and quality information about sight quality. It makes possible to reify the lower sight quality in cases of normal values of central visual acuity examined by a classic method, e.g. by means of Snellen optotypes. The examination is more and more used at some chosen professions, those requiring work under difficult light conditions, such as drivers, pilots, engine-drivers etc. The examination can quicker help to discover endangered eye diseases.

[4 years' experience with use of the "Ambulancia, Oddelenie, Sklad" software made by Softprogress Piestany installed on the local computer network in the Ophthalmology Department of the Trnava Hospital].

The authors give information about their positive and negative experience at using computers connected to local network in ophthalmology department. They put the stress on a need to respect the requirements on hygiene of work at the beginning of the process of planning of suitable hardware choice software and workplace equipment. The need of sufficient number of workplace-terminals and software choice are considered to be necessary and the latter one allows a complex data processing and forming of the common database of patients for outpatient department, department and storage files. They refer to risks of local network switching over with other workplaces and possibility of undesirable information escapes and virus penetration into the network.

Follow-up of outpatients with essential hypertension. A comparison of three methods of blood pressure measurement.

Compared with conventional, routine blood pressure measurement in the outpatient department of a hospital, 24-hour blood pressure monitoring provides more comprehensive information about blood pressure values in day-to-day life of the hypertensive patient. The hospital setting with all its stimuli affects the hypertensive patients so that their values will be statistically significantly higher compared with normal ones. Recent years have seen the introduction into clinical practice of self-monitoring by the patients themselves. Comparison of these three methods for blood pressure measurements revealed that self-monitoring may provide fairly accurate values comparable with those obtained by the "standard method", i.e., 24-hour monitoring. In view of the low variability of the mean value of blood pressure over a period of several days, self-monitoring can be employed in groups of selected outpatients on long-term follow-up.

[The significance of longitudinal Holter electrocardiogram monitoring in the differential diagnosis of syncope in elderly patients].

Longitudinal monitoring of the electrocardiogram by Holter's method is indicated in particular for the diagnosis of intermittent dysrhythmia which may be related to the symptom of syncope in elderly patients. Transient dysrhythmia with subsequent cerebral ischemia may be the cause of altered consciousness, vertige, presyncopes, or true syncopes. Transient dysrhythmias may be therefore considered after elimination of other causes of syncopes as their aetiology, in particular in elderly patients and old age groups where organic heart disease is already present. In those instances Holter's ECG is a useful non-invasive examination method which makes it possible to start appropriate treatment, and later by using serial monitoring, it makes it possible to follow up the effectiveness of antiarrhytmic treatment, or to follow up the function of an implanted cardiostimulator (pacemaker).
