Postpartum depression without delivering a child?

OBJECTIVE: Depression in people related to delivering women is documented in their mates, but only anecdotal in other family members. We describe a case of depression in a woman who had previously experienced postpartum depression after the birth of her nephew. METHOD: A clinical description of the case. RESULTS: A 53-year-old woman, hysterectomized at age 47 years, was admitted for attempted suicide. She developed major depressive episode 1 month after her daughter had delivered a son. She had a past history of two postpartum depressive episodes clinically identical to the current episode. The episode resolved after 5 weeks. At 1-year follow-up, the patient is still asymptomatic. CONCLUSION: Psychological and cultural factors were at play in this case more than hormonal and biopsychosocial ones.

Duration and stability of the rapid-cycling course: a long-term personal follow-up of 109 patients.

BACKGROUND: Recognition by the DSM-IV of rapid cyclicity as a course specifier has raised the question of the stability and long-term outcome of rapid-cycling (RC) patients. Data on this topic is sparse and often inconsistent. To our knowledge, these are the first personally followed patients over the long term, dealing directly with the issue of the duration of the RC course. METHODS: We examined the evolution of the course of 109 RC patients (68 women and 41 men) followed for a minimum of 2 years and up to 36 years, beginning with the index episode when the RC course was diagnosed by the authors (A.K., G.P.M., P.G., L.P., D.R.). Patients were included in the study if they met criteria for RC as defined by>or=4 affective episodes per year (Dunner and Fieve, 1974). The follow-up period varied from 2-5 years for 25 patients, 6-10 years for 24 patients, 11-15 years for 24 patients, 16-20 years for 19 patients, 21-25 years for 13 patients, 30-36 years for four patients. RESULTS: In 13 patients (12%), RC emerged spontaneously and in 96 patients (88%), it was associated with antidepressant and other treatments. In 19 women (28% of all women) RC course started in perimenopausal age (45-54 years). The mean duration of RC during the follow-up period was 7.86 years (range 1-32) and its total duration (including RC course prior to the follow-up period) was 11 years (range 1-40). The total duration of the affective disorder, from the first episode to the end of the follow-up, was 21.78 years (range 1-70). At the end of the follow-up, 36 patients (33%) had complete remission for at least the past year, 44 (40%) stayed rapid cycling with severe episodes (six of this group committed suicide), while 15 (14%) were rapid cycling but with attenuated episodes. The other 14 patients (13%) became long cyclers, eight with severe episodes and six with milder ones. The main distinguishing features between those who remitted from and those who persisted in the RC course were: (1). the initial cycle pattern: patients with Depression-Hypomania(mania)-Free interval cycles (53 patients) had a worse outcome: 26.4% remitted and 52.8% persisted in the RC course through to the end of the follow up period. The Mania/Hypomania-Depression-Free interval cycles (22 patients) had a significantly better outcome, with 50% remitted and 27.2% persisting RC; and (2). the occurrence of the switch process from depression to hypomania/mania and the occurrence of agitated depressions made the prognosis worse. Continuous treatment was more effective against mania/hypomania than against depression, yet in all persisting RC cases the mania/hypomania remitted only partially. LIMITATIONS: These data derive from clinics known for their expertise in mood disorders, and they may have attracted and retained patients with a more severe course. Treatment was uncontrolled and consisted more of lithium than divalproex, lamotrigene and olanzapine, recently shown to be beneficial in subgroups of patients with rapid-cycling. CONCLUSIONS: Our findings suggest that rapid cyclicity, spontaneous or induced, once established, becomes for many years a stable rhythm in a substantial proportion of patients, linked to endogenous and environmental factors. The suggestion is made to consider as rapid-cyclers, at least for research purposes, those patients who have had a rapid cycling course for at least 2 years, borrowing the duration criterion currently employed for other chronic disorders such as Dysthymia and Cyclothymia. That our patients had poorer prognosis than some other cohorts in the literature is probably due to the shorter duration of "rapid-cycling" at entry in the latter cohorts. A true understanding of the nature of rapid-cycling will require a rigorous definition of not only duration, but also pole-switching and course patterns at entry into study.

[Psychiatric approach to the clinical aspects of drug dependence: psychiatric comorbidity in drug addicts].

The authors analyze the most important theoretical models in the field of drug addiction: the moral, the medical, the psychodynamic and the psychiatric model, with particular reference to the concepts of comorbidity and dual diagnosis. It is pointed out as these approaches invariably lead to a dichotomy between the "defect/process" model and the "conflict/development" model. The proposed hypothesis is an attempt to reconcile the opposite points of view in an unitary vision: on the basis of the fundamental psychopathological elements (drive dyscontrol and dependence), the addicts are so distributed into three areas, different for nosography and treatment.

Epidemiology of adult medulloblastoma.

Medulloblastoma is one of the most common primary tumors of the central nervous system seen in children; in contrast, it is somewhat uncommon in adult age. Due to the infrequent occurrence, data on incidence rates are sparse. The present study was aimed at ascertaining the epidemiological characteristics of medulloblastoma in adult age in Piedmont during the period 1976-1995. Piedmont is a region in northwest Italy, which had a total population of 4.30 millions/year for the period mentioned. From the files of clinical records of patients hospitalized in neurologic and neurosurgical departments, 45 cases (32 males, 13 females) of histologically verified medulloblastoma were recorded. The incidence rate (annual per million) in the whole period studied was 0.5 (95% confidence interval, 0.36-0.67). The incidence rate was high in the age group 15 to 19 years (2.33/million/year) and decreased up to age 40, consistent with the embryonal origin of the tumor. No time-trend of incidence rate was found. Male excess was evident in all age groups and in each time period. Median survival time was 17.6 years; the 5-year survival rate was 69.9%. Survival rate in the present group of adult medulloblastoma is slightly better than that reported in clinical series. A comparison was made with incidence data concerning pediatric medulloblastoma reported in the Registry of Childhood Cancer of Piedmont: from 1980 to 1989, adult medulloblastomas represented 34% of medulloblastomas. The figure is higher than that generally assumed, and indicates that the occurrence of this embryonal tumor in adult age is relevant. Our epidemiological data are consistent with an embryonal origin of medulloblastoma.

[Ambulatory, interdisciplinary team work in the tension field between theory and practice--Vorarlberg social medicine organization].

Presented is the Social-Medical Organization active in the field of ambulatory neurological care of adults in Vorarlberg, Austria, offering interdisciplinary cooperation possibilities in terms of team consultation, case presentation and discussion, as well as supervision. Ambulatory interdisciplinary teamwork is situated in a theory-practice field of tension, with occupational training failing to teach interdisciplinary cooperation and interdisciplinarity role models not encountered either during practicals. The ensuing, deliberately identified problems and issues have been addressed in a planned process. Familiarization with the various occupational fields involved, with each field having its specific job profile, as well as the notions on cooperation among the various fields are presented. The role the various occupational fields have in neurological aftercare as well as existing job profile clichés are reflected upon. Communication is in addition hampered by the diversity of training contents in the various occupational fields. Focussing on case presentation and discussion, teams are considered a place for obtaining advice, for joint development of targets and strategies, and interdisciplinary interfacing is no longer perceived as threatening but as enriching and productive. The role of stronger family doctor inclusion in formulation therapy goals remains an issue as yet unsolved.

[Considerations on the concept of suggestion in relation to pharmacology].

After some brief considerations on the history of the concept of suggestion, the authors evaluate the importance of this concept in the doctor-patient relationship in which the pharmacological variable is present. They report their personal experience through a case report and hope for an integrated approach in which the drug could be seen as a biological and relational agent.

[Cognitive investigation on the quality of life of patients with oro-facial neoplasms in advanced stage].

As part of a Ministry of health founded research project into the quality of life among advanced cancer patients and in collaboration with the National Tumour Institutes of Milan, Naples and Genoa, an investigation was conducted on a national and international scale into the quality of the assistance given to cancer patients. Specific subjects of investigation included: the existence of pain treatment centres, home help and the type of psychological support offered such patients. In addition a questionnaire was presented to Italian oncologists, hospital staff and general practitioners covering areas like deontological problems of information, emotional, responses and quality of treatment. The answers revealed differences in the profiles of these three categories that make a significant contribution to the current discussion of the problem of the quality experienced by advanced cancer patients.

[Target symptoms in the utilization of antidepressants].

The authors intend to bring their contribution to the controversial question of target symptoms in the use of antidepressants. An extensive literature is analyzed, and, for comparative purposes, only works in double blind are chosen that use a specific rating scale (H.D.R.S.) in such a way as to obtain results referable to homogeneous symptoms on the phenomenal level and therefore qualitatively comparable among themselves. Each item of the H.D.R.S. is compared to the target symptom only if the relative score shows a statistically significant decrease, not only in comparison to the length of treatment, but above all in comparison with other antidepressants. On the grounds of the data obtained from this methodology, it is postulated that the relation between the action of a drug and the positive response to treatment is not to be placed in comparison with phenomenal traits, but in comparison with psychopathological processes that would make up the effective targets of the action of a drug.

[Changes in the bone marrow in cancer patients. 61 bone marrow biopsies].

The authors studied 61 bone marrow biopsies carried out in cancerous patients, presumably suffering from a bone metastasis and before any treatment. They feel that quantitative and qualitative changes in the bone marrow may be considered to be an indirect diagnostic indication of metastatic spread.

[Technic for the preparation of bone marrow biopsies. Evaluation of bone marrow cell count].

After having briefly reviewed the different subjective and objective procedures for interpreting Bone Marrow Biopsies from the standpoint of evaluating Bone Marrow Cellularity, the authors propose a method for overall morphological examination, controlled by a rapid evaluation by means of an integrator ocular. This simple, reliable and easily reproducible "semi-subjective" technique seems useful in routine pathological diagnosis, particularly to compare successive Bone Marrow Biopsies from the same patient when treated by antimitotics, or to study the changes of the hematopoietic tissue in the area of medullary metastasis.

[Nosographic problems and clinico-phenomenal characteristics of psychoses in epilepsy].

A critical analysis is made of the published reports on the epidemiologico-clinical and etiopathogenetic aspects of psychoses in epilepsy. Having underlined the principal characteristics of this pathology together with the main methodological and descriptive problems which emerge from the studies examined, the paper concludes by stressing the need for an approach based on a multifactorial understanding of the causes of these psychoses. In operative terms it is also necessary to evaluate the role of the predominant risk factors in each individual case.
