Biochemical characterization, localization and immunostimulating properties of a soluble glycoprotein, Ag1, isolated from in vitro cultures of Plasmodium falciparum.

The soluble amphiphilic glycoprotein, Ag1 (gp60), purified from supernatants of in vitro cultures of Plasmodium falciparum has a molecular mass of 60 kDa and did not exhibit size variation in the different P. falciparum isolates tested by immunoblotting. Ag1 was shown to interact with the lectin Erythrina christagalli agglutinin, which is specific for carbohydrates bearing beta-D-galactose(1-4)-D-N-acetylglucosamine. Indirect immunofluorescence studies showed that Ag1 is located on the surface of trophozoites and schizonts but not on the surface of merozoites. Ag1 is recognized by human immune sera from six different malaria-endemic regions. Ag1 induces in vitro proliferation of lymphocytes from malaria-immune individuals in an antigen-specific manner.

[Elements of the early diagnosis and staging of prostatic cancer. Functional results of radical surgery. Apropos of 50 complete prostatectomies].

The authors report a series of 50 radical prostatectomies and analyse and discuss the elements of: early diagnosis of adenocarcinoma: 63% of patients did not have a strictly normal digital rectal examination, 70% had abnormal ultrasonography, 73% has a PSA assay greater than 10. Suspicious signs were absent in 17% of cases, but present in 83% of cases (one sign), 63% of cases (two signs) and 40% of cases (three signs). Ultrasound guided biopsy of suspicious zones (on rectal examination or on ultrasonography) and in adjacent zones by dividing the prostate into quadrants has an increasing diagnostic yield (77% of the last thirty cases, 100% of the last fifteen cases). Staging frequently underestimates the exact volume of the tumour and the state of the prostatic capsule. MRI using a high power magnetic field apparatus seems to improve the accuracy of preoperative staging. Operative results were excellent in terms of mortality (nil), morbidity (6% of cases) and functional results for continence (recovered within one month) and sexual activity (satisfactory in 73% of cases) when Walsh's technique was able to be applied.

[In vitro induction of Plasmodium falciparum schizogony by human high density lipoproteins (HDL)].

The effects of serum and human lipoproteins (HDL, VLDL, LDL) were investigated on the intraerythrocytic cycle of P. falciparum using in vitro synchronized cultures. The reinvasion process of erythrocytes by merozoites and the development until the young trophozoite stage are independent of serum components. In the absence of serum, schizogony did not occur. However, addition of serum before the 24th hour of culture in basal medium restores a normal schizogony. Serum replacement by the different lipoprotein fractions showed that only the HDL fraction was able to assure a complete schizogony as well as a normal erythrocyte reinvasion. No division was observed with the apolipoproteins.

Purification and identification of a neutral endopeptidase in Plasmodium falciparum schizonts and merozoites.

An endopeptidase specific to the Plasmodium falciparum erythrocytic schizont stage and to free merozoites was detected using the fluorogenic GlcA-Val-Leu-Gly-Lys(or Arg)-AEC substrate. The enzyme was purified by high performance liquid chromatography (HPLC); its optimal activity was around pH 7.5 and its isoelectric point was 4.4. The molecular weight of the enzyme was about 68,000, as demonstrated by sodium dodecyl sulfate polyacrylamide gel electrophoresis (SDS-PAGE) under reducing conditions. The endopeptidase was strongly inhibited by thiol proteinase inhibitors, leupeptin, and antipain. The possible involvement of this neutral endopeptidase in the reinvasion process is discussed.

Neutral proteases involved in the reinvasion of erythrocytes by Plasmodium merozoites.

Neutral proteases of Plasmodium sp erythrocytic stages were studied by means of a sensitive fluorogenic method and gelatin-SDS-PAGE. The substrates gluconoyl-Val-Leu-Gly-Lys(or Arg)-3-amido-9-ethylcarbazole were selectively hydrolyzed by an endopeptidase from rodent Plasmodium berghei (Pb) and Plasmodium chabaudi (Pc) and from human Plasmodium falciparum (Pf) parasites. These endopeptidases were purified from 100,000-g soluble schizont extract by high pressure liquid chromatography; they have a similar Mr of 68,000 in SDS-PAGE, and an optimal activity at pH 7.4. The Pb 68 and Pf 68 endopeptidases were localized in schizonts and also in merozoites as shown by indirect immunofluorescence on Pb merozoites and by the identification of the Pf 68 endopeptidase activity in free viable merozoites. The Pb 68 and Pf 68 endopeptidases belong to the class of cysteine proteases. Analysis by gelatin-SDS-PAGE of a Pb 68 endopeptidase-enriched fraction showed a reproducible 95,000 proteolytic band. The initial extracts showed a similar 95,000 proteolytic band, and also 2 other 90,000 and 85,000 major bands. During reinvasion experiments, it was possible to recover a 95,000 and a 40,000 protease band from supernates of cultures grown in a semidefined medium without serum. Hydrophilic peptide derivatives related to the substrate of Pf 68 endopeptidase are shown to be potential inhibitors of the Pf reinvasion process in vitro.

[Statistical EEG detection of subjects with intracranial tumors. Study of a sample of 40 quantified recordings].

Spectral analysis of 16 channel EEG recordings was applied to 40 normal subjects (N) and 40 patients with supratentorial intracranial tumor (T). A set of 22 spectral parameters was computed from each lead. This study tries to evaluate if univariate and multivariate scoring techniques of quantified EEG data can achieve good separation of the two groups. A reference profile of data is derived from the standardized data of the group N. The 40 patients as well as the 40 normal subjects are compared to the reference data profile. Scorings from 264 univariate features do not separate the two groups. Scorings from a smaller number of multivariate features proved to be more powerful and distinguished 90% of patients. Badly classified subjects are discussed.

[Prenatal echographic diagnosis of iniencephaly. Apropos of a case].

Iniencephaly is a rare lethal malformation. It is associated with a defect in the occiput, alterations in the spinal column that are variable and retroflexion of the fetus. Antenatal diagnosis by ultrasound is possible because of the typical appearance: the fetal head is hyperextended, with distortion and shortening of the cervical spine. The occiput can be fused with lower vertebrae. Many associated malformations may be found: polyhydramnios, neurologic, facial and visceral abnormalities.

[The narrow lumbar canal. Study of a surgical series of 139 cases].

Over a period of 9 years, 139 patients with a mean age of 61 years were operated upon for disorders due to narrow lumbar vertebral canal. Clinical expression varied: lumbago, movement sciatica, intermittent claudication... Importance must be attached to discordance of examination findings in apparently benign root pain (67% of cases). The two key examinations of radiologic investigation are radiculography and a CT scan. Treatment is surgical, generally by a two-stage wide laminectomy combined with abrasion of an articular facet (64%), excision of osteophytic pads (35%) and/or treatment for a soft herniated disc (26%). Recovery was obtained in 35% of cases and pronounced improvement in 49% (84% of very good and good results) as evaluated by patients and surgeons.

[Microcomputer processing of medical records].

The necessity of computers becoming undeniable, authors have brought up the project of a program for management and treatment of medical files on a hospital word scale. Using a monoposte microcomputer, characterized by its simplicity, autonomy and low cost, this system is to be considered among the centralized multi-ramified systems, whose results as far as using the medical parameters are unsatisfactory.

[Fractures of the posterior arch of C2. Apropos of 34 cases].

The authors preferred the term of "fracture of the posterior arch of the axis", rather than the restrictive terms of bipedicular or bi-isthmic fractures commonly referred to in the literature. It was possible, from 34 cases to joint out 4 different figures of fractures of the posterior arch of axis: isthmus, body and articular process, pedicle and lamina. From this classification it is possible to get indications for surgical treatment, as some of those fractures need internal fixation and others not. On completing Xrays study with research of instability between C2-C3 it's possible to fix a standard management for those fractures. --at first reduction for many patients, then --immobilisation by a minerva cart in mostly cases --or surgical approach and internal fixation in case of diastasis or of bad reduction --anterior arthrodesis if needed (anterior instability) --or both posterior fixation and anterior arthrodesis.

[Proposal for a numerical prognostic evaluation in intracerebral hematoma].

From 300 cases of spontaneous intracerebral hematomas, 5 parameters are extracted they are: Age, sex, level of consciousness, localisation of hematomas and risk factors. A value, varying from 1 to 5 is attributed to each parameter, the sum of values give a score for each patient. This score allows the clinician to predict, within the early 48 hours, the vital prognosis. Survival chances are: 80% if score between 5 and 8; 60% if score between 9 and 13; 40% if score between 14 and 17; 20% if score equal or superior to 18.

[Semiology of spinal meningiomas and neurinomas (author's transl)].

The last 60 patient who underwent surgery for neurinoma or meningioma of the spine provide the basis for this report. One-fourth of all cases of spinal cord compression are due to meningioma or neurinoma. Attention is drawn to the characteristic clinical features of these tumors, particularly their time-course, since diagnosis if often delayed. The sooner diagnosis is established, the better postoperative recovery will be, especially in elderly patients (60% of cases). Results of operative treatment are presented.

[Cystic dilatations of the subarachnoid space at the lumbo-sacral level. Clinical and therapeutic aspects].

The cystic dilation of the sub-arachnoidal space individualised by Tarlov in 1938, may be integrated in the diagnosis of non discal surgical sciatica. The classification proposed by Lazorthes sumes up the eventualities encountered at the lombo-sacral level. The clinical study of five personal observations and the analysis of literature enable the establishment of a clinical picture evoking the anomaly. It would appear that the affection is characterised by lumbago or lumbo-sciatica, the attacks separated by free intervals. In an acute period the algia lessens in the Trendelenbourg position. A significant sign would be the pain on moving the sacrum. Among the complementary examination the radiographic study of the sacrum with tomographies is indisplusable, the diagnosis is based on the radiography of spinal nerve roots with late time in a vertical position. Medico-surgical treatment is in order, it gives a favorable result when surgical criteria have respected: invalidating evolutive algia with or without a clear sign of deficiency. The surgical result is essentially conditioned by a strict indication.

[Radio-anatomical study of the main trunk of the middle cerebral artery (author's transl)].

This work corroborates anatomical data well known since G. Lazorthes about the main trunk of the middle cerebral artery. The data are interesting in various fields: Sylvian aneurysms, big sphenoïd ridge meningiomas, extra-intra cranial micro neurosurgical anastomoses. This work is based upon 280 angiographic pictures, 20 dissections of brain arteries and 12 plastic injections of the brain arterial vascular tree. The most important data to point out are variations of length and of division (no division, simple bifurcation, trifurcation or multiple divisions), variability in the central arteries and some rare anomalies like accessory middle cerebral arteries and duplication.
