Macular and lichenoid amyloidosis: a possible secretory product of stimulated basal keratinocytes. An ultrastructural study.

This study examined the histogenic pathways of amyloid formation in skin-limited amyloidosis. Skin biopsies taken from 10 cases of macular or lichenoid amyloidosis were studied with the light and electron microscope. Keratinocytes with filamentous degeneration were frequently encountered. They were normally eliminated either transepidermally through the surface or by macrophages at the dermoepidermal junction. Only occasionally was amyloid deposition observed to be associated with apoptotic keratinocytes undergoing filamentous change. On the other hand, small aggregates of amyloid were frequently observed at the dermoepidermal junction closely applied to healthy non-degenerate basal keratinocytes. The early appearance of amyloid at the dermoepidermal junction, its pattern of distribution and its intimate association with overlying active basal keratinocytes suggest that these fine, straight, interlacing fibrils may be a secretory product of stimulated basal keratinocytes.

Rational approach to the management of eczemas.

Eczema is an inflammatory skin disease commonly encountered in clinical practice where the underlying cause often appears uncertain. Diagnosis is essentially clinical, depending on clinical signs which varies with the stage of evolution of the inflammatory process. Although the mainstay of treatment is still topical corticosteroid preparations, much more can be done if an accurate diagnosis of the type of eczema is made and patients advised about the prognosis, and natural history of this troublesome and sometimes disabling condition.

Eyelid grafting as a preprosthetic measure for restoring facial defects.

Prosthetic restoration of a facial defect involving the loss of an eye and associated orbital tissues may be restricted by incorrect orientation of the prosthesis if the reconstructed tissue bed is uniform and without distinctive orbital landmarks. A surgically grafted eyelid is useful in this situation. The lower eyelid graft serves to provide an anatomical guide with which to position the ocular component of the orbital prosthesis in relation to its established visual axis. This assists the subsequent orientation of the adjoining tissue portion of the orbital prosthesis, contributing to a well-positioned and cosmetically pleasing result.

Secondary localized amyloidosis in trichoepithelioma. A light microscopic and ultrastructural study.

Two trichoepitheliomas with secondary localized amyloidosis were documented in a 74-year-old man. The ultrastructural findings support an origin of amyloid from tumor cells. It is suggested that the occurrence of amyloid is a reflection of the ability of tumor cells to produce an excessive number of tonofilaments from which amyloid is believed to be derived. The intimate spatial association between amyloid and fibroblasts may suggest the existence of some yet undefined functional relationship.

The role of fine needle aspiration for breast cancer in the climacteric--a preliminary report.

Fine needle biopsy has been used with great accuracy in the diagnosis of breast tumours. There have been few reports on its efficacy as an adjunct to other breast screening techniques like x-ray mammography. Abnormal mammograms in the absence of a palpable breast lump are sometimes detected in the climacteric and these pose a problem of tissue sampling for pathologic diagnosis. We have tried to evaluate the technique of radiologically guided fine needle aspiration in such patients. Our report is of an initial series of 13 cases of non-palpable, mammographically abnormal breast lesions. Eight cases were fibroadenoma, 4 cases mammary dysplasia and 1 a benign breast lesion. Representative cytologic samples were obtained in 11 of our 13 cases. Cytologic diagnosis should be correlated with mammographic findings.

Measuring the cleft deformity.

Comparative studies in cleft lip and palate patients are difficult because of the difficulty in documenting them. Both qualitative assessment and quantitative measurements are needed so that a more meaningful comparison of long-term results could then be made. The different protocols used previously by various authors were reviewed and a protocol was drawn up for the Department of Plastic Surgery, Singapore General Hospital. This was used for a trial period and then revised. The difficulty in measurement lay mainly in determining the accurate anthropometric points on the face from which measurements were made.

Cephalometry

Initial experience with the argon laser in cutaneous vascular lesions.

An 18-month clinical study on the effects of argon laser in cutaneous vascular disorders in 41 patients carried out at the National University Hospital is discussed. The results confirmed the excellent response that can be achieved in lesions such as capillary hemangiomas, granuloma pyogenicum, cherry angiomas, telangiectasias and spider angiomas, but not in venous flare of the lower limbs. We are cautiously optimistic about its role in portwine stains; results are still being assessed. Complication rates so far have been relatively low with scarring observed in one patient.

Acute disseminated cutaneous candidiasis.

Acute disseminated candidiasis is a serious and difficult problem often seen in immunocompromised states. Appearance of a characteristic skin eruption is helpful in the diagnostic. We report below a case report of an eight year old girl with aplastic anemia who had received multiple courses of antibiotics. A profuse monomorphic papular nodular eruption subsequently appeared on the face, palms and soles. Candida tropicalis was identified from the skin biopsy taken from one such lesion.

Acute Disease

Tissue expanders in reconstructive surgery.

The shortage of skin and soft tissue has been the limiting factor in many reconstructive problems. Tissue expanders (a temporary inflatable device) were recently introduced, to provide the solution. In the first 25 patients of our series of patients age 7 years to 46 years, a total of 30 expanders were used in different parts of the body. The expanders used were made by Cox-Uphoff (CUI) and varied in size from 30 ml to 1000 ml, with an attached remote valve. These expanders were placed subcutaneously, subfascially or submuscularly. Expansion was carried out as an outpatient procedure over a period of 6 weeks. 21 of the 25 patients underwent successful treatment. Failures were due to technical problems, extrusion of expanders, haematoma and infection. Tissue expanders are a new dimension in reconstructive surgery. They cause stretching of skin and soft tissues over a period of time and permit resurfacing of skin defects in difficult situations. Complications may be avoided with experience and further refinements in the construction of the expanders.

Primary scalp cancer in Singapore.

Cancers of the scalp are uncommon. Owing to their unique site they pose problems in surgical treatment. A retrospective study of all scalp cancers notified to the Singapore Cancer Registry over 17 years has shown that squamous cell carcinoma is the most common cancer followed by basal cell carcinoma. Other cancers are sweat gland carcinoma, angiosarcoma, dermatofibrosarcoma protruberans, liposarcoma, fibrosarcoma and malignant lymphoma. Scalp cancers are more common in Chinese and very rare in Indians. There is no difference in the incidence between the sexes. The surgical treatment and reconstructive techniques employed in 52 patients are described. When localised, scalp cancers are managed like other skin cancers but once they invade bone, they become aggressive and are difficult to eradicate. Wide radical excision provides cure and the skull bone if involved should be removed.

Ampicillin-induced interstitial nephritis with generalised exfoliative dermatitis.

Drug induced renal failure is a serious, potentially, fatal illness that is preventable. Hypersensitivity reactions involving the kidneys can occur with any of the penicillin groups. However, there are few reports in the literature of ampicillin as a cause of acute interstitial nephritis. In the following case report, a patient developed acute interstitial nephritis with renal failure and exfoliative dermatitis following ampicillin therapy. The diagnosis was based on the rapid onset of renal failure, presence of eosinophilia, skin rash, and characteristic renal biopsy finding, following the administration of ampicillin. Renal function returned to normal eventually, but she continued to have relapses of exfoliative dermatitis up to a year later.

Acute Kidney Injury

Opsite, a synthetic burns dressing.

Opsite, a sterile, occlusive, adherent, polyurethane film has been used in a prospective study involving 150 cases of partial thickness burns. All cases were admitted to the Department of Plastic Surgery, Singapore General Hospital. The patients' ages ranged from infants to the elderly. All sites of the body were tried on Opsite except the head and face and total burns areas applied with opsite varied from less than 5% to 25%. 80% of cases did well, 20% had to have the dressing discontinued for various reasons, the most common being infection. The advantages were in the ease of wound observation, patient comfort, early mobilisation and relative economy. Opsite is a suitable burns dressing for the partial thickness burns especially of the trunk and limbs and is very useful in children. The complications are relatively few and minor.

Congenital cleft lip and palate in Singapore.

Cleft lip and palate is the most common congenital abnormality seen in the Department of Plastic Surgery, Singapore General Hospital. A total of 461 operative cases seen over a period of 5 years (1977 to 1981) is analysed. There is a relatively high incidence in the Singapore population of 2.0 per 1000 live births. In both cleft lip and cleft palate deformities, there is no significant difference in the sex distribution. The cleft patterns show that 78% of the cases are unilateral clefts, 53% are left sided, 25% are right sided and 22% of the cases are bilateral clefts.
