[Nursing and AIDS: knowledge and paradigm].

This study is a part of a research that comments the relation between Nursing Knowledge and AIDS. The actual period of time is treated like a period of transition between the modern science and one new paradigm. This is called post-modern science. It brings the dynamics of AIDS in Brazil after the registration of 79,908 cases from 1980 to 1996. The main objective is to describe nursing knowledge that has based nursing care to seropositive and aidetic patients in an University-Hospital located in Rio de Janeiro, Brazil.

Isolation, cDNA cloning, and characterization of an 18-kDa hemagglutinin and amebocyte aggregation factor from Limulus polyphemus.

An 18-kDa hemagglutinin which possesses the property of inducing both aggregation of amebocytes and agglutination of erythrocytes has been isolated from Limulus polyphemus amebocytes and purified by ion exchange chromatography. This nonglycosylated, single chain polypeptide with an M(r) of 18,506 and isoelectric point of 8.3 is stored exclusively in the large secretory granules of amebocytes. Based on the partial N-terminal amino acid sequence of 63 residues, DNA probes have been synthesized for screening a pBR322 cDNA library constructed from Limulus amebocytes. The cDNA coding for this protein reveals the presence of a 19-residue signal peptide preceding the 153-residue open reading frame. Northern blot analysis indicates the presence of a single mRNA species. The primary structure derived from the corresponding cDNA sequence reveals an internal homology consisting of two consensus sequences, Val-Asn-Asp/Ser-Trp-Asp and Glu-Asp-Arg-Arg-Trp. The formation of 5 disulfide bonds between 10 half-cysteines divides the molecule into three looped domains each containing the Glu-Asp-Arg-Arg-Trp repeating unit. One of the novel features of this protein is that it shares 37% identity with a 22-kDa mammalian extracellular matrix protein isolated from fetal bovine skin (Neame, P.J., Choi, H.U., and Rosenberg, L.C. (1989) J. Biol. Chem. 264, 5474-5479). The two proteins exhibit a similar pattern of looped domains, each domain containing a homologous consensus sequence (i.e. Glu-Asp-Arg-Arg-Trp). The overall structure of both proteins seems to be highly related, with the exception of an N-terminal tyrosine-rich region present only in the mammalian extracellular matrix protein. The functional properties of the two proteins are similar in that the Limulus 18-kDa protein agglutinates horse erythrocytes and aggregates Limulus amebocytes, and the mammalian 22-kDa protein is an effective adhesion promoter for dermal fibroblasts. On the basis of these unique properties, the newly characterized hemagglutinin has been termed Limulus 18K agglutination-aggregation factor (18K-LAF).

Basosquamous cell carcinoma of the skin with metastases.

Two cases of basosquamous cell carcinoma of the skin with lymph node, lung and bone metastases are reported. Metastases occurred 4 and 7 years after identification of the primary tumour. Both the primary and metastatic lesions had areas of typical basal cell carcinoma and squamous cell carcinoma and also intermediate carcinomatous tissue. In the bone metastasis of one case there were rudimentary hair follicles and areas of matrical differentiation. These cases further support the existence of basosquamous cell carcinoma and emphasize its metastatic potential.

Antipyretic activity of an aqueous extract of Zizyphus joazeiro Mart. (Rhamnaceae).

Bark infusions of Zizyphus joazeiro Mart. (Rhamnaceae) have been employed in Northeastern Brazil as a remedy for fever. This study investigated the antipyretic activity of an aqueous extract of the plant in rabbits rendered febrile by intravenous injection of E. coli endotoxin. Fever responses were significantly decreased (P less than 0.05) by the oral administration of a bark infusion of Z. joazeiro Mart. These results lend support to the popular use of infusions of this plant in folk medicine as a remedy for fever and suggest that the characterization of the principle(s) responsible for such activity deserves further investigation.

Continuous expansion for the treatment of skin deformities.

Continuous expansion using an infusion pump was studied in seven patients. Tissue expanders were attached to outpatients under general anesthesia. After a period of 7 days, the skin expansion was initiated with the patient hospitalized. Saline solution (0.9%, 1-3 ml/h) was infused via a gauge needle connected to the expander valve and to the infusion pump. The expansion speed was determined by clinical parameters, such as pain and the expanded tissue viability, until the desired volume was achieved. The total time required for this procedure was 13-18 days. Based on continuous skin expansion, we attempted to test an alternative shorter procedure. This method was well accepted by doctors and patients, and no complications occurred. Although fibrosis was observed, it was not as pronounced as in the outpatients undergoing the conventional expansion treatment. A principal advantage of the continuous expansion method is the increased safety resulting from more effective clinical control and adequate systemic antibiotic therapy. Coupled with on important reduction the expansion time and allowing a faster return of the patients to their activities.
