[Color-coded Doppler sonography in pregnancy].

Recently the advantages of angiodynography have been reported for different purposes. The goal of this study was to find out the validity of colour flow mapping for obstetric patients. Additionally, we compared the obtained results with those obtained by conventional pulsed Doppler measurement. 30 women with an uneventful pregnancy were investigated. Angiodynography enabled us to depict a selected vessel quicker and to visualise the course of the investigated vessel. Furthermore, the new method of colour flow mapping and subsequent pulsed Doppler measurement results in visual vessel recognition of fetal and maternal vasculature and therefore subsequent Doppler measurement is also possible in small vessels. This method could be valuable for the diagnosis of high-risk pregnancies.

[X-ray diagnosis of renal osteopathy in dialysis patients with reference to clinical aspects].

Pathophysiological histological and radiological findings in renal osteodystrophy are described. Special emphasis is laid on secondary hyperparathyroidism. Preliminary results of the authors' investigations show a good correlation between radiological findings in the phalanges of the hand and the concentration of parathyroid hormone (PTH) in 14 patients. The concentration of the hormone in the blood was measured by a new "two-site" immunoradiometric assay, which is specific for the intact, biologically active hormone. Patients with high concentrations of PTH in the blood tended to have more severe radiological changes. In 4 patients for whom radiographs of the hands revealed no pathologic findings, normal PTH concentrations in the blood were measured by this method, whereas the conventional assay gave elevated hormone concentrations for the same patients. This is due to the lack of specificity of the conventional method for the intact, biologically active hormone. Nevertheless, further investigations are needed to confirm these findings.

Chronic Kidney Disease-Mineral and Bone Disorder↗

[Incidence and clinical importance of chronic reactive periosteal new bone formation in the cervical region in patients with differing neurologic symptomatology].

81 patients classed into three groups with clinical evidence of neurological symptoms and posttraumatic pain of the cervical spine and the incidence of degenerative disorders were studied noninvasively via CT scanning. Imaging of osseous structures in the axial plane by CT was excellent in all cases, even in the lower part of the cervical spine where soft-tissue discrimination is often impossible because of shoulder artifacts. In about half of the patients with nerve-root symptomatology as well as with signs of involvement of long tracts, narrowing of the foramen intervertebral, respectively of the spinal tract, was seen, attributable to degenerative osseous apposition with excellent clinical segmental and (according to radicular symptoms) side correlation. In contrast to these results the group of patients with posttraumatic clinical symptoms showed almost 50 per cent less preexisting degenerative disorders of the cervical spine. In conclusion, we assume that high-grade osseous appositions of the dorsal part of the vertebral body play an important role in the development of radicular symptomatology and cervical myelopathy, respectively. Hypertrophic changes of the processus articularis with narrowing of the spinal canal occurred in 14 per cent and were therefore of minor clinical significance.

[Angiodynography: technical principles and clinical uses].

Angiodynography is a newly developed system that enables Doppler sonography to be used to obtain a colour-coded real-time image of the blood vessels in the familiar ultrasound slice image. A new computerised technique allows both the familiar pulse echo signal and the Doppler shift of the signal resulting from movement in the body to be displayed for each point on the ultrasound slice plane. Stationary tissue structures are shown as the familiar grey scale image, while movement such as of the blood is shown in colour. 453 patients were examined. Normal and pathological flow measurements were obtained in the carotid artery, the jugular vein, in renal transplants, in the thyroid, the testis and in the urethra. Pathological flow velocities were visualised in stenosis, tumours and diffuse alterations of parenchyma.

[Synthetic ultrasound blocks and contact gels in sonography: hygienically satisfactory?].

Different sponge-like plastic blocks and several samples of two coupling gels used during small-parts real-time sonography were investigated microbiologically. Whereas the coupling gels showed no contamination a great deal of different bacteria could be found on the plastic blocks. These findings lead to a restricted use of those devices especially when investigating immunocompromised patients. Sufficient procedures of decontamination do not exist. Because of the relative high purchase cost the one-way use of plastic blocks is uneconomical.

[Sonography of benign skin tumors].

Evaluation of 62 pigmented skin tumors of unknown pathology showed that benign and malignant tumors can easily be separated. Benign skin tumors have many internal echoes whereas malignant melanomas show an almost echo-free structure. Superficial spreading melanomas (SSM) and very flat benign tumors were both recorded as thin echogenic layers. It is not possible to differentiate them by US. Real time US is a simple non stressful, reproducible procedure that should be established as a routine diagnostic tool in dermatology.

[Color-coded Doppler sonography of the carotid arteries].

Doppler colour flow imaging provides colour-coded visualisation of blood flow additional to the B-mode image. In this study, the carotid arteries of 92 patients were examined comparing duplex sonography with Doppler colour flow imaging. Efficiency of these methods was evaluated by angiography in 16 patients. Examination time was shorter by using Doppler colour flow imaging; evaluation of haemodynamics and anatomical changes was simplified.

Arterial Occlusive Diseases↗

[Color-coded Doppler sonography of kidney transplants].

A prospective study was carried out in 81 patients to compare the ability of colour coded Doppler sonography (angiodynography) and Tc bolus examination for demonstrating renal blood supply. In patients with normal transplant function both methods performed equally well. In anuric patients angiodynography was superior to the Tc bolus examination: In three cases angiodynography was able to provide important additional information such as absence of blood flow in peripheral intrarenal vessels or missing blood supply of a transplant pole. Angiodynography seems to be able to replace the Tc bolus examination; this statement excludes very obese patients until low-frequency probes for angiodynography will be available.

[Color-coded Doppler sonography in thyroid diseases: initial experiences].

The thyroid gland was investigated in 43 persons (12 healthy volunteers, 31 patients with diseases of the thyroid gland) with a new colour Doppler system (angiodynography). In euthyroid persons (normal thyroid gland, diffuse or multinodular goitre) only a few vessels were detected. In cases with Graves' disease however, hypervascularization was evident. Autonomous adenomas had a hypervascular periphery, whereas our two carcinomas showed a high vascularisation in the nodule. Angiodynography might become an important diagnostic tool for the evaluation of thyroid glands. The number of radionuclide studies and punctures might be limited.

[Color-coded Doppler sonography of the pelvis-leg arteries: an appraisal of the value of that method in comparison with angiography].

The aim of this study was to determine the accuracy of colour-coded Doppler sonography (CDS) and its advantages as compared with angiography and pulsed Doppler sonography. For this purpose, the arteries in the pelvis, thigh and knee of 42 patients were examined by CDS, the gold standard being angiography. In ten patients, pulsed Doppler sonography was performed as well and the length of the examination compared with that of CDS. It was shown that CDS is valuable in the investigation of peripheral vessels and that it can replace angiography after femoro-popliteal bypass operations or PTA. Compared with pulsed Doppler sonography, CDS has the advantage of being much quicker.

[Spontaneous carotid dissection as the cause of cerebral infarct in a youth].

The case of a 19-year-old patient is presented who was admitted with aphasia and hemiparesis due to basal ganglia infarction as a result of spontaneous dissection of the internal carotid artery. The difficulties in diagnosing this disease with CT and MRI in the acute stage are demonstrated. Angiography is still imperative in order to ascertain that a carotid dissection has occurred.

[Computerized tomography analysis of cement-free total hip endoprostheses].

16 patients with total hip endoprosthesis were investigated by high resolution computed tomography (CT) to analyse possible advantages of CT over conventional radiography. The quality of the examinations was good only in pure titanium systems: examinations of chromium-molybdenum alloys were severely deteriorated by strike artifacts. Positioning of implants is well defined in radiographs; CT provides better visualisation of the relation between implant and cortical bone. Evaluation of the contact of the shaft to the femoral corticalis is difficult in radiographs but facilitated in axial scans. Determination of relevance of CT analysis of total hip endoprosthesis remains subject to a future long-term follow-up study.

[Duplex sonography in the assessment of normal flow relations in the transplanted kidney].

26 patients with normal renal transplants were examined by means of duplex sonography to define normal blood flow parameters. The renal blood flow pattern showed in each segment of the renal arteries a rapid upslope in systole followed by graduate downslope in the early diastole with persistent flow throughout the diastole. There was no significant difference between the main renal artery and the three different orders of intrarenal arteries. Mistakes can be made due to flow disturbance from turbulences at the anastomosis or by obtaining signals from other vessels located near the renal parenchyma. Several (at least two) arterial signals should be obtained by aligning the transducer along the axis of the transplant renal artery and renal parenchyma. Only the strongest signals should be recorded.

[Radiologic detection of pulmonary artery hypertension].

In 43 patients with obstructive and restrictive lung disease a catheterisation of the right heart with measurement of pulmonary artery pressure was performed. In a retrospective study several radiological parameters of pulmonary hypertension were evaluated on the chest radiographs of these patients. Considering those parameters on the p.a. and lateral chest radiograph, the diagnosis of pulmonary hypertension in patients with elevated pulmonary artery pressure at rest can be made with great accuracy. When pulmonary artery pressure is elevated only during exercise, the accuracy of radiological diagnosis is much lower.
