[A look back at 2 mumps outbreaks].

Two studies are presented. The first one concerns a mumps outbreak in a kindergarten in Geneva in June 1991. Of 26 children, nine (34.6%) got mumps. Of nine children vaccinated with the Rubini vaccine strain, seven had the disease as opposed to only one of 14 children vaccinated with the Urabe strain. The vaccine efficacy of the Rubini strain was estimated at 22% with a 95% confidence interval of -10% to 45%. The second study concerns a cluster of 112 mumps patients seen by a pediatrician in the Bernese Jura region between September 1992 and May 1993. A case-control study was carried out resulting in a vaccine efficacy estimate of 50% with a 95% confidence interval of -19% to 81%. Of the cases, 51 (45.5%) had been vaccinated against mumps, 50 of them (98%) with the Rubini vaccine strain. Of the controls, 30 (61.2%) had been vaccinated, 86.7% of them with Rubini. Methodological problems of case selection and their possible effects on the estimated vaccine efficacy are discussed. The results of these two studies have been confirmed by more recent investigations. In retrospect, we therefore conclude that small studies can serve as early indicators for epidemiological evidence and that they can be finally integrated into a more complete picture.

[Responding to health needs of apprentices and secondary-school students: current programs in 2 Swiss cantons].

To meet the health needs of adolescents 16 to 19, two Swiss cantons have largely modified their school health systems: this paper describes two programs located in the French speaking part of Switzerland that include both screening and educational activities. These activities are run on an collective and individual basis by specially trained nurses and physicians. Three conditions appear essential for the effectiveness of such programs: priorities should be derived from the needs as expressed by the adolescents themselves. The system should be able to adapt constantly to new problems as soon as they are detected. All activities should be carefully evaluated.

Blood pressure values in 8-9- and 14-15-year-old schoolchildren of Italian and Swiss origin: the Geneva survey.

In a cross-sectional epidemiological survey of the precursors of cardiovascular risk factors, 8-9- and 14-15-year-old boys and girls of Italian and Swiss origin were studied. Each of the 8 groups comprised about 100 participants chosen at random. In total 387 Italian and 410 Swiss participants of comparable socio-economic origin, and residing in Geneva, were studied under standardized conditions. The anthropometric measurements, the determinations of systolic and diastolic blood pressure and the lipoprotein lipids of both ethnic groups are presented and discussed in detail. The blood pressure values show only slight differences between the two ethnic groups, especially in the higher percentiles; significant differences exist, however, between the various anthropometric characteristics, some lipoprotein lipid measurements and sexual maturity. The blood pressure values are clearly correlated with body mass index, sexual maturity, anthropomorphic measurements, and heart rate. The results of the Geneva survey compare well with the reported results of other international studies.

[Preventive activities of the Geneva Youth Health Service].

Based on a centralized structure and a mission oriented toward prevention, the Geneva Youth Health Service carries out extended surveillance activities in the areas of somatic health and environmental hygiene. Its services are directed at the two- to twenty-year-olds, and in particular the children and adolescents in preschool institutions, in public schools and in the technical instructional institutions. Its activities complement those of other services of the Geneva Youth Office, in particular in the medical-educational and social-educational areas.

[Health education: team work at school].

In this field, team work is invaluable, as the relevant frame for both content and method to be permanently readjusted to various levels imposed by the specific objectives. The better the members of this team are integrated into the educational system, as straff members of a service, the deeper the impact of the programme.

[Infections transmitted in swimming pools].

Public swimmingpools can be the source of infections due to micro-organism such as mycobacterium balnei, adeno and enteroviruses, the virus of plantar warts and molluscum contagiosum, the TRIC-Agent of swimmingpool-conjonctivitis and pathogenic fungi. The transmission of trichomonas vaginalis is considered unlikely-Water of pools, supposed to present satisfactory qualities by standard controls, was found to contain pathogenic staphylococci and pseudomonas aeruginosa. Effective preventive measures include the continuous recording of the redox-potential of the water, limiting the number of visitors to pool design specifications, better desinfection of sanitary installations, regular maintenance of technical equipment including frequent backwashing of filters and exclusion of visitors with communicable disease.
