Usefulness of a short femur in the in utero detection of skeletal dysplasias.

In 28 fetuses studied during a 4 1/2-year period, the initial femur was below 2 standard deviations (SDs) of the mean when compared with the biparietal diameter. These fetuses were considered at risk for skeletal dysplasias and were followed up. Studies were performed at a mean gestational age of 26.7 weeks (range, 15.3-41.0 weeks). Group 1 had a femur length 1-4 mm below the 2-SDs line (range, -2.0 to -4.0 SDs); no other abnormalities were detected. Interval examination of 12 femurs showed that 10 either remained shortened to the same degree or had a growth spurt. At birth, all subjects were healthy except one with mild growth retardation and one with a chromosomal abnormality. Of the two subjects that failed to continue normal growth, one was healthy and the other was a heterozygous achondroplastic dwarf. Group 2 had greater femoral shortness; all measurements were more than 5 mm below the 2-SD line (range, -4.3 to -31.0 SDs). All had fetal abnormalities and significant skeletal dysplasias. The authors conclude that the number of millimeters below the 2-SDs line is an accurate, easy criterion for evaluation of femoral length.

[Results of an interdisciplinary study of primary inoperable stage T3 and T4 squamous cell carcinoma of the mouth, oropharynx and hypopharynx].

From Dec. 1980 to May 1984 26 patients with inoperable carcinomas of oral cavity, oropharynx and hypopharynx, were treated by us by a primary systemic chemotherapy. At all there were 95 patients with carcinoma of oral cavity, oropharynx, and hypopharynx, among these in 62 cases primary surgery was possible. In seven cases no tumor specific therapy was possible. A complete remission (CR) was obtained in 17% of tumors, partial remission (PR) in 39%. The results after subsequent operation (5 patient underwent operation) and radiation (all patients underwent radiation) were 35% CR and 26% PR. The median survival time of the patients with the multimodality-therapy was 13 months. Now 7 patients are alive with NED between 12+ and 38+ months.

[Comparative studies of tocainide and propafenone in the treatment of ventricular arrhythmias].

In 15 patients with ventricular arrhythmias without previous treatment the effectiveness of 2-amino-2',6'-dimethylpropionanilide (tocainide, Xylotocan) and propafenone was compared in a cross-over-trial. 14 Patients had ventricular arrhythmias of the Lown classification IVa-V, 1 patient of Lown classification III. In 3 patients after the first course of tocainide resp. propafenone no change in therapy was made because of reducing the Lown classification by two or more grades. All patients had 24-h-ECG-Holter-Monitoring before, at the 4th day before change of therapy, and at the 9th day of therapy. Following relevant results were found: Both tocainide (p less than 0,005) and propafenone (p less than 0,05) reduce the total amount of ventriculare premature beats (VPB) without statistical significant difference between both substances. Both tocainide (p less than 0,005) and propafenone (p less than 0,02) reduce the number of isolated polytope VPB without statistical significant difference between both substances. Only tocainide (p less than 0,005) reduces couplets, but there was no statistical significant difference to propafenone. Neither tocainide nor propafenone alone was able to make total suppression of the ventricular arrhythmias in the patients included in this study. Both substances were able to lower VPB buth with a more favourable but statistically not significant therapeutic effect of tocainide.

[The atrioventricular valve opening time on the normal echocardiogram].

54 healthy volunteers were examined. In 48 the time from the beginning of the QRS complex to the opening of the atrioventricular (AV) valves (point D in the echocardiogram), the Q-Dt for the tricuspid valve (TV) and for the mitral valve (MV), was measured by echocardiography. In the other 6 subjects this was done by atrial stimulation using an esophageal probe. The Q-D period is dependent on heart rate and embraces the total electromechanical systole (Q-S2) of the left or right ventricle and the isovolumic relaxation time (IRT). The time for the opening of the TV is 408 ms (+/- 86 ms) [2 SD] and that for the MV 428 ms +/- 86 ms at a mean heart rate of 73 beats/min and 72 beats/min respectively. However, this 20-ms difference between the opening times of TV and MV was not seen under stimulation with the esophageal probe. In order to compare Q-Dts measured at different heart rates, all values were corrected to correspond to a heart rate of 72 beats/min by using the regression line of Q-Dt/heart rate. Prolongation of Q-Dt TV and Q-Dt MV in comparison to normal values is discussed in 9 patients. Of 6 patients with left heart failure, 2 had delayed opening of the MV; in 5 early diastolic filling was delayed or there was exclusively late diastolic filling of the right ventricle. It can be assumed that prolongation of Q-Dt TV or Q-Dt MV is due to a dysfunction of isovolumic relaxation. The time taken to reach the early diastolic AV pressure gradient seems to play an important role in determining the length of the IRT. A lengthening of the IRT and the electromechanical systole is discussed in 3 patients with right ventricular load and prolongation of the Q-Dt TV.

[Hemodynamic and exercise capacity in a 72 years old patient under a ventricular and bifocal pacemaker (author's transl)].

The results of haemodynamic, echocardiographic and bicycle exercise investigations in a 72-year-old man with a permanent complete atrioventricular block and heart failure following infarction are presented. Comparative measurements were obtained under the conditions of a ventricular and an av-sequential pacemaker (PM). The ventricular demand pacemaker (VVI) was implanted three years ago and because of further impairment of cardiac performance an av-sequential pacemaker (DDD) was used to restore atrio-ventricular synchronisation. The treatment with bifocal PM improved dramatically cardiac output and exercise capacity. The echocardiographic findings demonstrate the significance of Frank-Starling mechanism in this case due to better filling of the ventricles. As noninvasive methods, echocardiography and bicycle-exercise test allow long-term analysis of cardiac function.

[He washout from obstructed lung segments by collateral ventilation (author's transl)].

The time of He washout from totally obstructed segments in the intact dog lung is about twice that of non-obstructed segments. Within one breath the He is transferred from obstructed to non-obstructed to non-obstructed segments in relative great quantities, indicating that the transport of the gas occurs by collateral ventilation and not by diffusion. This is in good agreement with calculations about the He transfer through the alveolar wall by diffusion or by flow through the alveolar pores. It can be assumed that the transfer by collateral ventilation in obstructed segments is about 10(4) times greater than by diffusion through collateral channels.

[Diffusion disorders in patients with liver cirrhosis].

In 17 patients with cirrhosis of theliver and in 11 controls the pulmonary diffusing capacity for CO (DLCO) was determined at three different levels of alveolar oxygen tensions. The diffusing capacity of the alveolar membrane (DMCO) and the intra-pulmonary capillary volume (VC) were calculated following the formula given by Roughton and Forster. The following results were obtained: 1) Both DLCO and DMCO were lower (p less than 0,01) in the patient group than in the controls. 2) VC showed larger variations in thepatient group than in the controls (p less than 0,01). The mean values did not differ, however. 3) There was a significant linear correlation (p less than 0,001) between DM and 1/VC in the patient group (DM and VC in % of the predicted value). The results suggest, that a change in the configuration of the capillary bed may be responsible for the transfer defect found in cirrhosis.
