Detection of type III collagen in skin fibroblasts from patients with Ehlers-Danlos syndrome type IV by immunofluorescence.

Immunofluorescence showed that cultured skin fibroblasts from 15 out of 17 patients with Ehlers-Danlos syndrome type IV retained abnormal amounts of type III collagen within the cytoplasm. This was not shown by fibroblasts from normal subjects or from patients with other inherited connective tissue diseases. The diagnosis of Ehlers-Danlos syndrome type IV may be facilitated by this finding.

MR imaging of orbital tumors with CT and ultrasound correlations.

Fourteen patients with various orbital space-occupying lesions were studied by magnetic resonance (MR) imaging with a 0.15 T imager using spin-echo and inversion-recovery pulse sequences. Computed tomographic and ultrasonographic (US) examinations were done in 13 of these patients. All 14 lesions could be demonstrated by MR. Magnetic resonance was found to be better than US but not as good as CT in providing morphological details. Regarding tissue characterization, US was the most useful among the three modalities. With the T2-weighted spin-echo pulse sequence, a possible trend in tissue characterization could be shown. From our preliminary experience, we conclude that MR imaging using a 0.15 T resistive magnet offers no distinct advantage over the combination of CT and US in the diagnosis of orbital tumors.

Nasolacrimal bone changes in diseases of the lacrimal drainage system.

The bone pieces removed from 71 patients who had undergone dacryocystorhinostomies (DCR) or canaliculodacryocystorhinostomies (CDCR) were examined pathologically. Nineteen percent of cases showed evidence of active bone remodeling, 23% had immature, woven bone present, and the remaining cases showed normal mature cortical and cancellous bone. There were no inflammatory changes found. The clinical histories of the patients were reviewed and related to the bone findings. The clinical applications of the bone changes are discussed.

The relationship between serum and saliva trimethobenzamide concentrations in man.

Six healthy volunteers provided simultaneous serum and mixed saliva specimens at various time intervals after receiving three different concentrations of trimethobenzamide HCI (total dose 800 mg) via rectal syringe on three separate occasions. Specimens were analyzed for trimethobenzamide content and the results subjected to pharmacokinetic analysis and other statistical procedures to identify characteristics of the relationship between distribution processes in these specimen types. On the basis of linear regression analysis there was a significant correlation (P less than 0.01) between saliva and serum drug content for both mean values (r = 0.94) at each sampling time and all individuals' paired data points (r = 0.72). Because of the large degree of scatter in individual pairs, the data were subjected to analysis of variance in an attempt to determine a potential source of this divergence. The relationship between serum and saliva drug concentrations was found to depend upon the time after drug administration. We conclude that a nonuniform process determines the relative degree of partitioning trimethobenzamide between serum and saliva.

Artefacts of localization of atherosclerosis in pinned aortas.

Detailed studies of aortas from 8 rabbits showed that serious artefacts occur if the aortas are pinned at their in vivo dimensions, rather than fixed at physiological pressure. In the pinned aortas, the proximal parts of the branches were pulled up onto the aortic wall. This was more pronounced for the large branches of the abdominal aorta than for the smaller intercostal branches. This artefact caused atherosclerotic lesions, which had developed at the origin of the branch, to appear as if they were entirely on the aortic wall. We found that a marked change in the elastin pattern was present at the origin of the branch; this can be used to mark the true origin of the branch. With the pressure technique we found that lesions had different shapes and locations at different branch points.

Intravenous glucose tolerance, insulin, glucose, and free fatty acid levels after myocardial infarction.

Glucose tolerance tests performed on 12 patients within 15 hours of myocardial infarction and repeated two to four weeks later showed failure of insulin secretion, hyperglycaemia, glucose intolerance, and high free fatty acid levels. More pronounced changes were found in patients with cardiogenic shock. These findings suggest that the therapeutic use of potassium, glucose, and insulin should be re-evaluated.
