Superinfection by Aspergillus fumigatus of a pulmonary lesion caused by Mycobacterium xenopi.

A 36-year-old man had a pulmonary infection with a cavity in the right upper lobe caused by Mycobacterium xenopi. A superinfection by Aspergillus fumigatus occurred 1 year later. Both organisms were found in the resected lung specimen. The patient remained asymptomatic after surgery. The superinfection of healed tuberculous lesions by 'atypical' mycobacteria or Aspergillus species is well known but the co-existence of the 2 organisms is very rare.

[Double-blind study of the treatment of disc lumbosciatica by chemonucleolysis].

A randomized, double-blind study was made of 39 patients with discolumbar hernias that were unresponsive to medical treatment. They were treated either by nucleolysis with chymopapain (4 000 U of discase), or by discography followed by injection of distilled water as a placebo. After one month, treatment was found to be successful in 55% of cases undergoing nucleolysis and in 26% of cases receiving the placebo. The results after 3 months were 65% and 42%. respectively. After one month, the intensity of lumbar pain decreased by 44% on average in the group treated by chymopapain, and by 0% in the placebo group. Radicular pain decreased by 53% in the first group and by 26% in the placebo group, and improvement in Lasègue's sign was 51% and 20%, respectively. Analgesic treatment was reduced in patients treated by chymopapain: 65% of patients in the first group report some or much improvement in their sciatica, whereas this figure was 26% in the placebo group (p less than 0.02). Contralateral sciatica was observed in 3 patients of the placebo group. The average give in the disc one month after nucleolysis was 34% in the chymopapain group and 27% in the placebo group, without correlation with the clinical result. On average, patients were monitored for one year after nucleolysis, and during this year 6 patients from the chymopapain group were operated upon, compared with 10 patients from the placebo group. Although these results are at the limit of statistical significance because of the number of patients studied, they confirm the innocuousness and effectiveness of chymopapain in the treatment of discolumbar hernias.

[Normal radiological aspects of a cemented total hip replacement].

The authors draw attention to the normal radiological appearances and the minor complication which may affect a cemented total hip replacement. Knowledge of these appearances enables the radiologist to define his attitude to the patient as well as his remarks on the case.

[Intra-osseus hemorrhagic lesions in congenital afibrinogenemia. A new case (author's transl)].

Congenital afibrinemia is a rare disease, transmitted by an autosomal recessive mode, and due to deficient fibrinogen. Osseous involvement is exceptional, since only 2 cases have been reported. We describe a third case. The roentgenographic features are well limited radiolucent lesions, arising in the medullary space, and slightly expanding. These lesions are related to fibrous cysts, resulting from intraosseous hemorrhages. Spontaneous resolution may occur: the absence of fibrin deposits and blood clot probably favours resorption.

[The posterior arch of the Paget's ivory vertebra. Biopsy of six cases (author's transl)].

The authors report 6 cases of isolated "ivory" vertebra which were confirmed as being due to Paget's disease after trocar biopsy. They analyse the radiological characteristics and find evidence of constant involvement of the posterior arch which shows the classical signs of Paget's disease: bone condensation, increase in size and cortico-medullary de-differentiation.

[Radioguided puncture-infiltration in the treatment of refractory calcifying periarthritis of shoulders].

Trituration-aspiration-infiltration of calcifications of the shoulder region is a method applied for treatment of intractable calcifying periarthritis. A television screen is used to guide the needle until it is in contact with the calcifications provoking the symptoms. Trituration and aspiration of the calcified lesions is followed by saturation with prolonged released corticoids of 41 shoulders treated, recovery or improvement was noted in 70% after one month, results being rated as excellent in 19 patients followed up for 3 months.

[Bone maturation and osteoarticular lesions in top level sportsmen. Apropos of 105 cases].

At the time of the European Gymnastics Championships (Lyon, 1980), X-rays of the left wrist were taken in 105 high level junior athletes. Most importantly, we found a delay in bone age in 73% of the boys and in 78% of the girls, which is probably related to an unbalanced diet and to repeated micro-trauma to the growing epiphyses. In particular, we observed traumatic lesions at the distal extremity of the radius with considerable frequency (83% of gymnasts). They are sometimes accompanied by cubital lesions and they are due to an increased fragility in the epiphyso-metaphyseal region due to the delay in bone maturation.

[Treatment of lumbar disk hernias using chemonucleolysis].

The authors present 46 cases studies of herniated lumbar discs treated by chemonucleolysis in the previous 2 to 9 months. One patient suffered from recurrent acute lumbago; the other 45 patients suffered from lumbar root pain. The diagnosis of herniated disc was confirmed by radiography of the lumbar roots. Chemonucleolysis was performed under neuroleptanalgesia in the majority of cases, after failure of medical treatment and as an alternative to surgery. The patient treated for recurrent lumbago obtained a good result. After one month, 20 patients no longer suffered from sciatica and two had only a mild and intermittent sciatic pain. The results are lasting. Overall, chemonucleolysis is successful in 69% of cases. The success rate decreases with the age of the patient and the age of the symptoms. The results appear to be unchanged by the number of discs treated, their level or their discographic features. The results appear to be unchanged by the number of discs treated, their level or their discographic features. The results seem to be better when the disc being treated has not collapsed and when the hernia is not too large. Seven failed cases went to operation when, in each case, a non ruptured herniated disc was found. In 4 cases there was minimal tissue and in 2 cases the hernia was soft. The tolerance was excellent. Chemonucleolysis is effective in the treatment of herniated lumbar discs, but should be reserved for selected patients. Used with care, the treatment is safe. In cases where the treatment fails, the surgical procedure is not interefered with.
