[Orchidectomy in testicular cancer. Variant techniques and tactics].

Exploratory orchidotomy, generally leading to orchidectomy, is the first procedure performed whenever testicular cancer is suspected. The techniques and indications for wide orchidectomy have advanced since the time of Chevassu: radical orchidectomy with lymph node dissection via an inguinoiliolumbar incision is no longer performed, value of wide orchidectomy associated with radiotherapy or chemotherapy, place of enucleations in the case of solitary testis or synchronous bilateral lesions, and deferred orchidectomy after chemotherapy. The various techniques are reviewed and their indications are discussed.

[Diagnostic difficulties and value of partial surgery in multiple tumors of the kidney. Apropos of 4 cases].

Multiple renal tumors are rare. Ultrasonic and CT scan examinations will suspect the diagnosis in the majority of cases. With the histological diagnosis and its prognosis, the treatment may be watchful waiting, partial nephrectomy or radical nephrectomy. Partial nephrectomy is indicated in case of multiple tumor in a patient with solitary kidney or for bilateral tumors. Urinary fistulas or renal insufficiency are potential complications, but they are exceptional. Four cases are reported (oncocytoma, angiomyolipoma, papillary tumor and adenocarcinoma) to illustrate the difficulty of diagnosis with imaging and the surgical options.

[Triage of the injured in military surgery and in exceptional situations].

Sorting casualties is universally accepted as the best means of coordinating a large number of patients with war injuries. Sorting means classifying casualties by characteristic medical and health features according to three types of criteria: diagnosis: gravity of the injury; prognosis: predictable survival; and logistic: evaluation of demand for care and possibilities for evaluation. The sorting process may be systematic or selective. The rules applied in different war situations or in civilian catastrophies vary according to the emergency care system which is called in (French Army, NATO, International Red Cross, GNO). The current role of the intensive care physician as a sorter is underlined. More than a doctrine, surgical sorting is a concept of action to be understood.

[Bilateral tumors of the testis: the role of the diagnosis of carcinoma in situ in early detection].

In a series of 540 patients with testicular germ cell tumors, the diagnosis of bilateral tumor was observed nine times. Two patients presented a simultaneous bilateral tumor and for seven others a second tumor occurred after an interval varying from 1 to 16 years. The often late discovery of the contralateral cancer sometimes requires a prolonged follow up for patients with unilateral testicular malignancy. Incidence, risk factors and therapy are discussed, in particular the anomalies in testicular migration, hypofertility, testicular atrophy and carcinoma in situ. Prevention of bilateral consecutive tumors depends on the research of carcinoma in situ thanks to testicular biopsy. A slight radiotherapy allows eradication of this tumor, whose evolution towards invading cancer is particularly high. A rigorous watch of these patients can be considered. With the absence of early detection, the treatment of the simultaneous tumor needs taking into consideration the treatment of the initial tumor. In case of a diagnosis at stage 1, observation after orchidectomy is possible without further treatment.

[Urogenital injuries caused by projectiles. Data on recent conflicts and the North-American civilian experience].

The study of medical series from Vietnam and Lebanon, the analysis of US Army medical organization during recent UNO-IRAK conflict, the major technical contribution of North-American civilian publications should lead to draw correct organizational and technical management principles concerning genito urinary wounds, a for a long time misunderstood or neglected chapter of abdominal wounds.

[Primary melanoma of the esophagus].

The authors report 2 cases of primary melanoma of the esophagus, and describes the clinical, endoscopical, radiological and histological features from these exceptional tumors. Prognosis is very poor, despite various therapeutical possibilities.

[Subxiphoid pericardiostomy and pericardioscopy. Value and indications apropos of a series of 25 cases].

An experience of 25 subxiphoid pericardial windows with pericardioscopy in eleven cases is reviewed and presented. The operative indications in pericardial effusions result from hemodynamic compromise or from the need for etiologic criteria. Validated in its technical simplicity, its safety, its etiologic findings, its efficacy on immediate and delayed no recurrence of pericardial effusion, the low rate of late constrictive changes, this operative procedure is worth listing in the therapeutic know-how of a general surgeon. Indications in patients with suspected traumatic pericardial tamponade by chest wound are related.

[Rupture of the female urethra in fractures of the pelvic bones. Apropos of 2 new cases].

Female urethra ruptures associated with pelvic ring fractures are exceptionally observed. The authors compared these 2 cases (Besançon and Toulon) to already published cases. This diagnosis must be contemplated and established by the surgeon or in his/her presence using urethral catheterization. The surgical approach (most of time abdominoperineal) allows an accurate injury repair and prevents from fearsome complications due to late diagnosis and treatment. An active attitude following this urological act is highly required, in case of osteoarticular injuries, whose instability and rules are currently known.

[Pseudo-neoplastic aspects of thoracic actinomycosis].

Pulmonary actinomycosis, a rare bacterial disease, is characterized by focal suppuration and an expansile fibrogranulomatous process that has a pseudotumoral evolution. On radioclinical examinations, it often mimicks an advanced or even inextractable primary bronchial tumor, thus leading to extensive surgery or even to give up the treatment. The effectiveness of antibiotic therapy leads the authors to discuss the role and limitations of surgery.

[Testicular sarcoidosis].

The authors report a case of testicular sarcoidosis. This lesion may mimic cancer, but the association of germ cell tumour and systemic sarcoidosis appears to be too frequent to be simply a coincidence. It is important to diagnose combinations of these 2 diseases in the scrotum or in the mediastinum, but their pathogenesis remains hypothetical.

[Urological surgical effect of the repair of inguinal hernia using bilateral subperitoneal prosthetic mesh].

In view of the increasing frequency of using subperitoneal bilateral prosthesis for inguinal hernia repairs, urologists should be aware of the genitourinary implications of this procedure: risk of infection due to potentially septic procedures on bladder or prostate, postoperative genitourinary complications, interval before subsequent trans-prosthetic surgical incision, operative tactical outcomes of subperitoneal pelvic fibrosis.

Fibrosis↗

[Giant esophageal fibrovascular polyp. Apropos of a case].

Fibrovascular polyps, together with leiomyomas, constitute the great majority of rare benign tumors of the oesophagus. Their size and their frequently high level of attachment sometimes give them a spectacular clinical presentation. The hazards involved in the interpretation of radiological and endoscopic data account for frequent mistakes in their diagnosis. The determination of the pedicle site defines the choice of the surgical approach. A diagnostic error and the choice of an incorrect approach are the two main causes of therapeutic difficulties.
