Pathological features of renal cell carcinoma incidentally discovered at autopsy.

Renal cell carcinomas incidentally discovered at autopsy (IRCs) were retrospectively analyzed to determine whether they had any pathological features differing from clinical renal cell carcinomas (CRCs): From 7,970 autopsy cases, there were 51 cases (0.65%) of renal cell carcinoma (RC) including 42 men and 9 women (ratio 4.6:1). The mean age of RC cases was 66.3 +/- 12.9 (range of 20 to 91 years). Of the 51 RC cases, approximately half of them (25/51) were IRC cases. No difference was shown in age and sex between IRC and CRC cases. Mean tumor size of IRC was 2.6 +/- 2.5 cm in diameter. This was significantly smaller (P less than 0.01) than that of CRC (7.3 +/- 2.8 cm). Histologically, both grade 1 and clear cell type were significantly more predominant among IRC cases than CRC cases (P less than 0.01 and P less than 0.05, respectively). Only one out of 25 cases of IRC had distant metastasis, as compared to 23 out of 26 CRC cases (88.5%). These pathological features indicate that the incidentally discovered renal cell carcinoma may have less malignant potential than clinical tumors. Unexpectedly, the majority of the IRC cases (88.0%) were associated with advanced neoplasia of other sites, as compared to only 19.0% of CRC cases (P less than 0.01).

Differential expression of class I major histocompatibility complex determinants by lymphoblastic leukemia-lymphoma cell lines.

Two T-cell lines (XR11-4T and XR11-5T), established from radiation-induced, murine lymphoblastic lymphomas, were examined for the expression of class I major histocompatibility complex antigen and tumor induction. These cell lines expressed class I private determinants, H-2.9 and H-2.26, but not the monomorphic determinant defined by monoclonal antibody M1/42. Both cell lines produced tumors in syngeneic and allogeneic hosts. The monomorphic determinant could be demonstrated on both cell lines following growth in allogeneic (BALB/c mice) but not in syngeneic (RFM mice) hosts. The re-expressed determinant present on cells following growth in allogeneic mice was not of host origin. Thus, tumorigenic x irradiation may differentially affect the expression of class I major histocompatibility complex determinants.

Bilateral diffuse nephroblastomatosis, pancortical type. A case report with immunohistochemical investigations.

A variation of Perlman's syndrome of the pancortical type is reported in a male neonate whose parents were cousins. The patient was the product of a 35-week pregnancy, the Apgar score was 3, and the patient died of respiratory failure one hour and 12 minutes after delivery. Autopsy revealed bilateral diffuse nephroblastomatosis, pancortical type, associated with malformations (usually facial), congenital anomalies of the heart, hepatosplenomegaly, pancreatic islet cell hyperplasia, bilateral cryptorchidism, and hyperflexibility of the left knee joint. Renal immunohistochemical investigations revealed positive bindings with peanut and soybean agglutinins and epithelial membrane antigen along the luminal surface of the epithelium in the moderately differentiated tubules, but not in blastoma or primitive epithelium.

Proliferative function of cadaveric bone marrow cells.

The proliferative function of human and murine cadaveric bone marrow was studied using methylcellulose clonal cell culture assays and the murine spleen colony technique. The study revealed persistence of hemopoietic functions for as long as 19 postmortem hours in cadaveric marrows of some patients. Studies of murine cadaveric marrows corresponded with those of human marrows. These results strongly suggest that human pluripotent hemopoietic stem cells survive in cadaveric marrows.

Isoantigens A, B, and H in morphologically normal mucosa and in carcinoma of the larynx.

A modified Coombs' mixed cell agglutination reaction was performed on biopsy specimens of 24 patients with squamous cell carcinoma of the larynx. Isoantigens A, B, and H(O) were absent in 15 cases, partially lost in eight, and completely retained in one case. In four cases the initial biopsy specimens were histologically benign yet negative for isoantigens. In subsequent biopsies these showed the presence of carcinoma. The morphologically normal-appearing squamous mucosa adjacent to a carcinoma was found to be devoid of isoantigens in ten cases. The investigation suggested that isoantigenic alteration as detected by mixed cell agglutination reaction was a sensitive sign, since it occurred in squamous mucosa prior to recognizable morphologic change for malignancy. The reaction may represent a functional dedifferentiation in cancerous tissue, analogous to but sometimes preceding morphologic dedifferentiation. The results were comparable to those in previous studies of neoplasms of various other anatomic sites.

The effect of dextran-40 on platelet adenosine triphosphate release in vitro.

Studies of dextran's effect on platelet function have revealed inconclusive results; some reports have suggested it acts as an aggregating agent, others as an inhibitor to aggregation. This study demonstrates the inhibitory effect of dextran (40,000 MW) on in vitro platelet adenosine triphosphate (ATP) release when stimulated by conventional aggregatory reagents. These data support inhibition of platelet surface phenomena, possibly involving receptor sites, rather than intracellular phenomena as the mechanism of dextran's anti-aggregatory effect. Significant inhibition occurs in vitro at dextran levels approximately twice that normally attained in vivo.

Elastase-1-secreting acinar cell carcinoma of the pancreas. A cytologic, electron microscopic and histochemical study.

A case of acinar cell carcinoma of the pancreas was diagnosed by fine needle aspiration cytology during surgery. The cytologic characteristics of this neoplasm are described. Electron microscopy disclosed numerous zymogen granules in the tumor while histochemistry demonstrated the presence of elastase-1. Serum elastase-1 levels were markedly elevated. The cytologic differential diagnosis of pancreatic tumors is discussed.
