[HCV reinfection after liver transplantation for HCV cirrhosis].

Cirrhosis due to hepatitis C virus (HCV) infection is now the most frequent indication for orthotopic liver transplantation (OLT). Recurrence of hepatitis C infection is the major cause of late mortality in patients undergoing OLT for hepatitis C cirrhosis. Recurrent HCV infection develops in 100% of patients HCV + in pre-transplantation time. Histological recurrence occurs in 75-80% of patients after OLT:1/3 of them progress to allograft cirrhosis within 5-7 years. Cholestatic hepatitis C develops in a sub-group of patients who progresses rapidly to graft failure. As a result of this accelerated course of HCV infection, long-term graft and patient survival are significantly reduced in patients undergoing OLT for HCV-related cirrhosis compared with other groups. Moreover, several recurrence's risk factors have been described as predictors of disease severity including those related to the virus, the host, the donor. There are numerous therapeutic strategies to prevent and to treat HCV disease recurrence after OLT. The most common strategy to treat HCV infection post-OLT is based on interferons and ribavirin. Even if clinical trials have shown that the combination of ribavirin with Peg-interferons is more effective than its association with standard interferons, the use of Peg-interferons in transplanted patients is limited by the side-effects of the drug. About treatment of hepatitis C virus infection in the allograft dark and not still cleared points are a lot: the timing and the target of therapy, the dose and duration of pharmacological treatment.

Prevalence of myocarditis at autopsy in Turin, Italy.

OBJECTIVE: To evaluate the prevalence of myocarditis in a general hospital in Turin, Italy. DESIGN: We retrospectively reviewed 17162 postmortem records from autopsies routinely performed at San Giovanni Battista General Hospital, Turin, between 1965 and 1994. RESULTS: Applying the so-called Dallas criteria, myocarditis was histologically found in 91 cases (0.53%, 95% CI 0.4 to 0.7). The prevalence increased, reaching a peak between 1985 and 1994 (1.2%, 95% CI 0.9 to 1.6). The disease was found more frequently in patients from 20 to 39 years of age, with no difference between males and females. The present data were compared to those of a previous study, performed in 1985 and 1993 to 1994, in which we had prospectively taken into account 605 autopsies (not comprised in the present retrospective study) with standardized myocardial sampling for histological examination: a 5.1% prevalence was found (nearly five times as high as that retrospectively detected in the same period). CONCLUSIONS: If a standardized method of myocardial samples for microscopic examination is not followed, it is possible that myocarditis is overlooked in an unsuspected number of cases.

[ETG follow-up in transurethral incision of the prostate (TUIP)].

In absence of marked benign prostatic hyperplasia, alternative surgical procedure, the transurethral incision of the prostate (TUIP), is available. The Authors report their experience and suggest a comparison between the morphological data obtained with ultrasound and the functional data obtained with the uroflowmetry.

[Intraprostatic spiral: our experience].

A spiral urethral prosthesis was inserted with use of local anesthesia in 5 patients with benign prostatic hyperplasia and 2 patients with cancer of the prostate, using ultrasonic guidance. All the patients refused the surgical procedure of had high operative risk. The Authors showed the usefulness of the Ultrasound for insertion of intraprostatic spiral, in selected patients.

[Treatment of renal cysts].

The Authors report their experience in treatment of renal cysts by PCN ultrasound puncture: An aspiration technique is described. A sclerosing substance (sterile Alcohol 95%) has been injected into the cysts to prevent relapse by a pigtail catheter. Immediate and late results are discussed. The data obtained confirm this practice as the best choice in the treatment of this pathology.

[The role of echography in the evaluation of stenosis of the male urethra].

Twenty male patients with known urethral stricture disease were evaluated using Ultrasonic urethrography, retrograde and anterograde urethrography and Cystoscopy for comparative analysis of the three techniques. The Ultrasonic urethrography provides the most accurate information regarding anatomic side, stricture length, number, the degree of scarring spongiofibrosis. Therefore the Authors suggest a larger use of this technique.

[The role of echography in cancer of the penis].

The Authors, observing two cases of penile cancer, lay stress on the role of Ultrasonography in this disease. Its easy and quick execution, its low cost, the possibility to repeat it, its diagnostic sensibility in evaluating the extent of the penile cancer, permit us to consider it a screening technique of primary importance.

[Renal adenocarcinoma associated with a polycystic kidney (presentation of a case and review of the literature)].

The Authors report a case of renal neoplasm association to polycystic kidney. The report was quite occasional, as the patient was not aware to be bearer of a renal dysembryopathy, although such a familiarity was existing formerly. The diagnosis was placed subsequently to ascertainments aiming at establishing the origin of a transient arterial hypertension, occurring recently, in absence of any other symptomatology.

[Diagnostic imaging of kidney carcinomas. Our experience and review of the literature].

In the image diagnosis of renal carcinoma echotomography has played an increasingly important role over the past ten years due to its simplicity and low costs. This method is responsible, above all, for the increased number of diagnosed cases of kidney carcinoma at a pre-clinical stage, the so called "incidentaloma", for which an aggressive surgical approach may be employed with good probabilities of the patient's complete recovery Having illustrated their personal experience of the diagnosis of renal carcinoma, the Authors review currently used imaging methods.

[A case of bladder herniation].

The Authors examine one patient with hernia of the bladder. The incidence, etiology and clinical picture are discussed.
