Efficacy of tetracycline and metronidazole alone or with ranitidine on the healing of duodenal ulcer and eradication of Helicobacter pylori. A randomized controlled multicenter study. Tetra-Metro-Ran Study Group.

In almost all eradication regimens, which contain antibiotics and bismuth derivatives, the administration of acid suppressing drugs for 4-6 weeks is recommended for healing of duodenal ulcer. The aim of this multicenter double blind study is to elucidate the effect of two classic antibiotics tetracycline (CAS 60-54-8) and metronidazole (CAS 443-48-1) alone or combined with ranitidine (CAS 66357-35-5) on the healing of duodenal ulcer and eradication of Helicobacter Pylori. Patients with duodenal ulcer were randomized to two treatment groups: group A received either ranitidine 4 x 150 mg or tetracycline 4 x 500 mg or metronidazole 3 x 250 mg for 2 weeks. Group B received 4 x placebo + tetracycline and metronidazole as in group A for 2 weeks. A final endoscopy was performed after 8 weeks. Four biopsy specimens were obtained from the antrum (two) and corpus (two) for both urease test and hematoxylin stain for detection of H. pylori. Out of 201 patients entering the study 156 completed the study (78 in A and 78 in B). The healing rate of duodenal ulcer was 98.7% in group A and 97.5 in group B. The eradication rate was only 33.3% in group B but 64% in group A (p < 0.001), when additionally ranitidine was given. The present study shows that treatment with the two antibiotics tetracycline and metronidazole alone results in a very low H. pylori eradication, but almost complete healing of duodenal ulcer after 8 weeks. Prolonged administration of antisecretory drugs in eradication regimens containing two antibiotics is not necessary for duodenal ulcer healing. However, the addition of H2-receptor antagonists or proton pump inhibitors to antibiotics increases the eradication rate.

[Therapeutic follow-up using automatic perimetry in chronic cerebroretinal ischemia in elderly patients. Prospective double-blind study with graduated dose ginkgo biloba treatment (EGb 761)].

The chronic cerebral retinal insufficiency syndrome in elderly patients is an organ specific expression of a generalized vascular cerebral deficiency. The progress of the disease is characterized by complex symptoms, variation in course, spontaneous remissions and, until recently inadequate diagnostic measurement methods. The new method of automated perimetry with the octopus 2000 P offers a patient-friendly procedure for indirect non-invasive diagnosis of circulatory state in limited cerebral retinal perfusion. In the present study measurements were made with this method on 24 patients (4 men and 20 women with an age of 74.9 +/- 6.9 years). The effect of the extract of Ginkgo biloba (EGb 761) on the reversibility of visual field disturbances was tested using a randomized and double blind study-design in two phases and with two dose levels. The main parameter investigated in this study was the change in the luminous density difference threshold after therapy with EGb 761. In group B (EGb 761 dose 160 mg/day) a significant increase in retinal sensitivity was seen within 4 weeks (p less than 0.05). In the lower dose (80 mg EGb 761/day) group (A), this change in retinal sensitivity was first seen after increasing the dose to 160 mg/day (p less than 0.01). The relative sensitivity of damaged retinal areas was more strongly influenced than "healthy" areas. The assessment by both doctors and patients of the general condition of the patients showed a significant improvement after the course of therapy. The results presented here show that damage to the visual field by chronic lack of bloodflow are significantly reversible.(ABSTRACT TRUNCATED AT 250 WORDS)

[Luminal deviation of the normal pancreatic duct a post mortem x-ray study (author's transl)].

The study was performed in 64 post mortem ductograms of pancreata, missing any inflammatory and/or neoplastic alterations in extensive histological investigations by whole organ sections. The pancreatic duct was measured in the radiograms. From these measured values and by mean of a computer program of a cubic spline function the straightened pancreatic ducts were plotted. The number, the length and the amplitude of ductal deviations were measured in these plots. Luminal dilatations were found in all organs, ranging from 0.5 mm to 6 mm and more. Their frequency was significantly lowered in young people (11-30 years).

[Comparative radiological and morphological study of the human pancreas. III. Morphometric investigation of the major pancreatic duct (author's transl)].

A ductographical and histological study was carried out in 70 human pancreata postmortem. The diameter, the thickness and the length of the major pancreatic duct were measured. The obtained data were evaluated statistically. The autopsy revealed in 7 cases an acute or a mild chronic pancreatitis, in 20 cases a disease of the bile duct system, in 4 both an acute pancreatitis and a bile duct disease, while the other patients showed no inflammatory or neoplastic diseases of the pancreas or the bile ducts. The relative wall thickness of the duct increases with growing distance from the papilla, while the real thickness of the wall decreases. The inner and outer diameter of the duct steadily rises with aging. The relative thickness decreases with aging, but in a considerable less degree. The length of the duct growths with aging. All these relations were expressed in mathematical equations. In acute pancreatitis, in mild chronic pancreatitis and in disease of the bile duct system no statistically significant differences of the outer and inner diameter, of the length and the relatative thickness of the wall were found in comparison to normal pancreata.

[Oral iron treatment: intestinal absorption and the influence of a meal (author's transl)].

Intestinal iron absorption after a single dose of a special galenic preparation (50 and 100 mg iron as ferrous glycocoll sulphate in micropellets) was investigated in 105 persons with normal iron stores (42), latent (40) or manifest iron deficiency (23). Absorption rate rate on fasting as well as after breakfast was measured in 21 persons. Intestinal absorption increased in persons with iron deficiency, but maximal absorption after a single dose of 100 mg did not exceed 25%. This absorption rate was already reached in persons with latent iron deficiency. There was no significant reduction of absorption when the preparation was given at the end of breakfast.

[Comparative radiological and morphological study of the human pancreas. 2. Morphometrical investigation of the human pancreatic duct system (author's transl)].

The diameter and the thickness of all the side ducts were measured in 27 normal or nearly normal human pancreata. The obtained data (about 5,000 pairs of Figures) were evaluated statistically. The relative thickness of the walls increases with growing distance from the papilla as well as with the reduction of the diameter of the side ductes. Beyond the age of fifty the diameter of the ducts is 10--13% higher than in younger age. In a logarithmical system there is a strong correlation between the diameter and the thickness of the side ducts, slightly diminished with aging. The regression coefficient is slightly diminished with aging. But the variance of the wall thickness in respect to the diameter of the ducts shows such a rise with aging, that the number of ducts with relatively thick and small walls increase with aging. By reason of the logarithmic system the positive variance has a higher weight. Therefore periductal fibrosis increases with aging.

[Intestinal absorption from therapeutic iron doses (author's transl)].

On a total of 105 persons with normal iron stores, iron depletion, and iron deficiency the intestinal absorption from therapeutic iron doses (100 mg Fe and 50 mg Fe as ferrous glycocoll sulphate) of a special galenic form was measured. The measurements were performed by means of a whole-body counter and preparations labelled with radio-iron (59Fe). Mean values of absorption rates from 100 mg Fe in healthy males were 5.0% and in healthy females 5.6% whereas in latent iron deficiency and in iron deficiency anemia mean values of 10% and 13% were obtained, respectively. The maximum absorption rate of 20 to 25% is reached already in the late stage of latent iron deficiency. Advancing severeness of iron deficiency is not followed by an increase of iron absorption. Investigations on 21 persons showed no significant difference between absorption rates of the galenic preparations used when administered orally before or after breakfast, respectively.

[A method to calculate the plausibility of paternity using blood group results of any relatives (author's transl)].

A calculation program is described by means of which the frequency of any type of relationship in cases of disputable descent (questionable partenity, maternity, parenthood, brother- and sisterhood) using blood group results, can be calculated. Within the scope of forensical practice, the program is above all important in cases in which the alleged father and/or mother of the child is deceased, where however, blood group results of other relatives are at disposal, and also in cases of suspected incest - when the alleged father of the child is at the same time father or brother of the child's mother, and, finally, in cases where two men who are related to one another are considered as possible fathers. The program can also be applied to search cases.
