Low-cost teleradiology for Australia.

Teleradiology refers to the transmission of clinical images from their source to remote sites. In this paper a low-cost means of establishing teleradiology using conventional personal computers is described. This is a simple and effective means of transmitting valuable clinical data from hospital to hospital, and hospital to individual, markedly enhancing the quality of clinical communication. The system described herein has proved invaluable in a busy neurosurgical department, in both emergency and more routine settings. Teleradiology has the potential to transform the early management of critically ill and injured patients, especially those from remote parts of Australia.

Traumatic internal carotid artery aneurysm and massive epistaxis.

A case of traumatic internal carotid artery aneurysm (ICAA) is described. Although rare, the occurrence of rupture of traumatic ICAA with epistaxis has a high mortality. Successful management requires an awareness of the entity, prompt recognition, and aggressive first aid procedures. Definitive treatment involves trapping of the internal carotid artery either by a direct approach, or by balloon embolization.

Spinal subdural abscess.

Spinal subdural abscess is a rare condition that is clinically similar to spinal epidural abscess. A case of spinal subdural abscess is reported, and the characteristic clinical findings and specific myelographic features in this patient are noted. Urgent myelography and surgery are necessary to prevent the rapid deterioration that often occurs in this condition.

Delayed traumatic intracerebral haematoma: case report.

A case of delayed intracerebral haematoma following closed head injury in a young male is reported. The patient was treated surgically with a satisfactory result. The problem of the unsuspected delayed traumatic intracerebral haematoma is highlighted.

Intracranial subdural hematoma after lumbar myelography.

Two cases of intracranial subdural hematoma after lumbar myelography are reported. This complication should be considered in patients who complain of prolonged headache or who develop neurological signs after lumbar puncture.

Aged

Observations on brain abscess. Review of 28 cases.

Twenty-eight cases of intracranial abscess managed at the Alfred Hospital, Melbourne, over ten years are reviewed. In half the cases, a brain abscess was a complication of paracranial infection. In one-third of the cases, infection arose from distant spread, usually from the heart. The temporal lobe was the most common site. While neither the site nor the source of infection was a consistent prognostic factor, the patient's level of consciousness at the time of operation was considered significant. Mortality was lowest in those with the least disturbance of consciousness. The best results were obtained with excision, though this was usually only appropriate in chronic cases. The total mortality rate was 10 deaths in 28 patients (36%), and the operative mortality rate was eight deaths in 26 patients (31%). The lethal nature of brain abscess, despite recent advances in diagnosis and treatment, is emphasised.

Aspiration of breast cysts.

One hundred and sixty nine women with 220 breast cysts were treated by needle aspiration between 1950 and 1980. Mean follow up was seven years. One hundred and twenty two (72%) patients were aged from 40 to 49 years. The majority (61%) of cysts occurred in the left breast. In 147 patients 174 (79%) of 220 cysts were aspirated once and did not recur at that site. One hundred and eleven (66%) patients presented with a single cyst and had no recurrence in either breast after aspiration. Two patients developed carcinoma of the opposite breast at 8 and 12 years after cyst aspiration. Needle aspiration of the breast is regarded as a safe, simple and economic diagnostic and therapeutic modality in women presenting with single breast lumps.

Dementia and hydrocephalus in Paget's disease: a case report.

Dementia and hydrocephalus in an elderly female were related to basilar impression caused by Paget's disease of the skull. Ventricular and lumbar thecal pressures were normal, but isotope cisternography suggested obstructive hydrocephalus. The patient improved after ventricular shunting. The importance of prompt investigation of dementia occurring in patients with Paget's disease is emphasised.

Symptom duration, tumor staging and survival in patients with carcinoma of the breast.

Between 1950 and 1980, 435 patients were operated upon for carcinoma of the breast. Symptom duration data were available for 402 patients. Fifty-three of the patients had preoperative symptoms for less than one one week; 113, for one week to one month; 144, for one to six months and 92, for six months or more. Tumor staging was available on 353 of the 402 patients. With increasing peroperative symptom duration, there was an increase in the incidence of Stage IV tumors, p=0.003, and a reduction in Stage I tumors, p=0.006. Cancer specific survival time was better for the total patient series and for those treated by potentially curative operation when symptoms had been present for one week or less as compared with those who had symptom duration of six months or more, p=0.007. Survival prospects were also better in those patients who had symptoms from one week to one month as compared with those who had six months or more symptom duration, p=0.005. These results indicate that patients in whom diagnosis is made and operation performed early in the symptomatic phase of carcinoma of the breast have less advanced tumors and better survival prospects.
