Shrinkage of the below-knee stump in leprosy.

The long-term progress of below-knee stumps was studied in 65 leprotic amputees. Thirty-seven patients underwent amputation with a long posterior flap (LP) and 28 patients with an equal anterior and posterior flap (EF) technique. Mean follow-up was for 6 and 7 years respectively. Shrinkage of the soft tissue and retraction of the posterior calf muscles caused the posterior skin flap of the stump to rotate posteriorly in all cases, with a mean 26 degrees of rotation in LP stumps and 42 degrees of rotation in EF stumps. The LP stumps remained thicker, with soft tissue padding at the bony ends, and had fewer stump complications than the EF stumps.

Pressures in the carpal tunnel. A comparison between patients with carpal tunnel syndrome and normal subjects.

We studied pressure in the carpal tunnel in patients with carpal tunnel syndrome and in normal control subjects, using a slit catheter and recording in the neutral position, 90 degrees dorsiflexion, and 90 degrees palmarflexion of the wrist. For each position the mean pressure in the patients was very significantly higher than in the controls, the highest pressure being in 90 degrees dorsiflexion, and the lowest in the neutral position. Using an upper limit of normal pressure of 5.5 mmHg in the neutral position gave a diagnostic sensitivity of 78.7%, a specificity of 78.1%, an accuracy of 78.5%, and a positive predictive value of 87.3%.

Red rubber bulb, cheap and effective vacuum drainage.

Red rubber bulbs have been used for vacuum drainage in head, neck, breast and several other operations by the authors since 1975 quite effectively without any major problems. The vacuum pressure of the red rubber bulbs was found to be higher than the expensive commercially available vacuum wound drainage device. The question of remaining old blood and infective microorganisms inside the reservoir for the reused ones were tested by the manual cleaning process and the standard sterile technique using steam under increased pressure (autoclave). The result is quite satisfactory. We encourage the use of this cheap and effective (made in Thailand) vacuum wound drainage in Thai hospitals and Thai medical schools.
