Abnormal echogenicity of the testicle caused by epididymal cysts.

Ultrasonography of the scrotum has proven to be highly accurate for the diagnosis of testicular and other scrotal abnormalities, particularly for defining focal lesions. False-positive examinations are unusual. Three cases are presented where large epididymal cysts compressed the testes and the remainder of the epididymis. This led to an abnormal echogenic area in testes of two patients and a hypoechoic defect of the testis suggesting malignancy in the thirds. The patients were sonographically and anatomically normal without testicular abnormality during and after operation, following removal of the epididymal fluid collections.

Urinary retention in sickle cell syndromes.

Four patients with sickle cell disease noted the development of urinary retention during an acute painful crisis. The acute urinary retention resolved with improvement in the painful sickle cell crisis. No evidence of anatomic urologic defect, systemic neurologic abnormality, or infection was documented in these patients. Patients with sickle cell disease in painful crisis should be observed for signs of acute urinary retention.

Urethral injury: experimental study.

The present experiment was designed to study several challenging therapeutic questions that remain disputable or unanswered: (1) Whether urethral healing post-trauma is influenced by the use of an indwelling urethral stent as an adjunct to suprapubic diversion; (2) the incidence of urethral stricture formation after immediate or delayed urethral repair; (3) the incidence of urethral stricutre formation after partial or complete urethral disruption; and (4) further assessment of radiographic findings in urethral injury. Herein is described the final conclusions we found to be relevant to this experiment.

Sarcoidosis and malakoplakia.

A case of malakoplakia in a patient with generalized sarcoidosis is reported. A discussion of the histologic and clinical characteristics is presented. This is the sixth case in the literature of concemitant malakoplakia and sarcoidosis. The suggestion is made that the two disorders may be related.

Priapism during filtration leukapheresis.

Priapism has been observed during two out of 3,680 filtration leukapheresis procedures in male donors and has been reported during hemodialysis. Both procedures are associated with enhanced granulocyte adhesion and aggregation presumably due to C5a. During both procedures, heparin is administered and this drug has been shown to cause heparin-dependent anti-platelet antibodies. It is suggested that complement mediated venous leukostasis or immune-induced platelet aggregates might impair the normal blood flow from the penis and result in a state of priapism.
