Efficacy of two flowable formulations of Bacillus sphaericus against larvae of mosquitoes.

Laboratory evaluation revealed that the Spherimos and Vectolex formulations of Bacillus sphaericus produced 97 and 100% larval mortality respectively in Culex quinquefasciatus at a dose as low as 0.008 ml/sq m as against 93 and 97% mortality respectively at 1 ml/sq m in Anopheles stephensi. However, in An. culicifacies similar level of mortality was not observed even at 10 ml/sq m of these formulations. Field evaluation revealed 100% reduction of Cx. quinquefasciatus larvae for 2-3 weeks in pools and wells with single application of Spherimos @ 2 ml/sq m as against 95.4% reduction @ 10 ml/sq m in irrigation channel for one week. Vectolex @ 10 ml/sq m provided 99-100% reduction of Cx. quinquefasciatus larvae up to 9 weeks in wells and 1 week in channels.

Humoral immune responses in osteomyelitis.

Fifty-three patients with osteomyelitis were grouped according to the duration of the disease at presentation, the bone and the part involved, the aetiology, activity, severity and stage of disease. Sera from these patients was estimated for IgA, IgM and IgG levels and in 23 matched controls. The results were compared with the normal range, and correlative studies were carried out between serum immunoglobulin levels and the different clinical parameters. Serum levels of the 3 immunoglobulins were found, in general, to fail to rise above the normal in the presence of infection. Furthermore, the levels did not show significant variation with the clinical parameters. These findings suggest that there is an intrinsically subdued immune response to osteomyelitis in most of the patients, partly confirming the suspicion of an immunodeficient state.

Early evidence of beneficial effects of chordal preservation in mitral valve replacement on left ventricular dimensions.

Earlier authors have shown the improved left ventricular (LV) function after chordae-preserving mitral valve replacement (MVR) in follow-up studies. Seventy-nine consecutive patients undergoing MVR were studied preoperatively and pre-discharge with M-mode and two-dimensional Color Doppler echocardiography for early LV morphometric changes. Conventional MVR was performed in 42 patients (Group 1) and the posterior leaflet-chordae-papillary muscle complex was preserved in 37 patients (Group 2). Both cohorts were similar in age, sex, preoperative NYHA class and valve pathology. Four St. Jude bioprostheses, 9 Medtronic and 66 Sorin prostheses were implanted. The intraoperative and perioperative management protocol was uniform for both groups. The median left atrial dimension in both groups decreased to better physiological levels (53 to 46 mm in group 1, 52.5 to 46 mm in group 2). The median LV enddiastolic dimension increased from 47 to 48 mm in group 1 while it decreased from 56.5 to 49 min in group 2. The median LV endsystolic dimension remained unchanged in group 1 but decreased in group 2 (37.6 to 36 mm). Echocardiographic documentation of such early changes in the LV dimensions after modified MVR indicates that the beneficial effects of chordal preservation become evident early in the postoperative period and may explain the improved perioperative LV function after modified MVR compared to conventional MVR.

A comparison of the clinical efficacy of ranitidine and sucralfate in reflux esophagitis.

A total of 40 patients with endoscopically proved reflux esophagitis were treated in a randomised study. Treatment was given for six weeks and consisted of daily doses of either 1 gm sucralfate four times a day or one, 150 mg ranitidine tablet twice daily. Twenty patients received each drug. Clinical evaluation was done weekly and endoscopic evaluation after six weeks. Forty percent patients on ranitidine and 50% patients on sucralfate became asymptomatic at six weeks. Fifty percent patients on ranitidine and 60% patients on sucralfate were endoscopically healed. The results did not show any difference in the effect of ranitidine and sucralfate treatment (p > 0.05). Overall tolerance and compliance in the two forms of treatment was good. Though ranitidine has a more convenient dose schedule and is more cost effective, sucralfate needs further clinical trials.

Acute toxicity of certain organochlorine, organophosphorus, synthetic pyrethroid and microbial insecticides to the mosquito fish Gambusia affinis (Baird and Girard).

Acute toxicity of certain organochlorine, organophosphorus, synthetic pyrethroid and microbial insecticides to the mosquito fish Gambusia affinis were determined to collect baseline data for selecting the resistant strains of the fish. The synthetic pyrethroid, Lambdacyhalothrin was most toxic to the fish (LC50 = 0.0022 ppm), followed by deltamethrin, cypermethrin and fenvalerate. Organochlorine insecticides, DDT and gamma-HCH, were less toxic than the pyrethroids, and these were followed by organophosphorus insecticides, malathion, fenthion, monocrotophos and temephos. The last two insecticides were least toxic among the different chemical insecticides (LC50 greater than 80 ppm ai). The microbial insecticide ABG-6262 (Vectolex 2.5 AS), a Bacillus sphaericus preparation, was totally harmless to the fish at 2500 microliters/l up to one week.

Evaluation of juvenile hormone analogue JHM/S-31183 against immature stages of mosquitoes in natural habitats.

A study was carried out to evaluate the potentiality of Juvenile hormone compound JHM/S-31183 against immatures of mosquitoes in natural habitats. Of two formulations tested 1% emulsifiable formulation was marginally superior than the granule formulation. Adult emergence of An. stephensi was completely inhibited upto 12 weeks when 0.5% granule formulation was applied at 0.04 ppm in wells as against 50% inhibition upto 8 weeks in pools. However, in Culex quinquefasciatus the per cent inhibition of adult emergence varied from 52 to 90 per cent. 100% inhibition in pools upto one week at 0.04 ppm in An. stephensi was also obtained with 1% emulsifiable formulation but the effect was diluted in successive weeks. The impact of this formulation was not much pronounced against Culex quinquefasciatus.

Esophagitis: clinical and endoscopic assessment of sucralfate therapy.

Twenty patients with endoscopically proved reflux esophagitis were studied. Each patient was given 1 gm sucralfate 4 times a day for 6 weeks. Clinical and endoscopic assessment of the effects of sucralfate on reflux esophagitis was done. 50% patients became asymptomatic and 60% healed endoscopically. The drug was well tolerated. We conclude that sucralfate is an effective and well tolerated drug for treatment of reflux esophagitis.

Evaluation of certain mosquito repellents marketed in India.

A study was carried out in certain villages of Distt. Ghaziabad (U.P.) to evaluate the efficacy of commercially available mosquito repellent preparations and devices. Results revealed that none of the repellents tested provided absolute protection against mosquito bites. The protection varied from 38 to 98% with different species of mosquitoes. In general, better protection was provided against Anopheles bite than against Culex. Of ten repellents evaluated. Mylol oil and Tortoise coils were marginally superior to Odomos cream and Rooster coils. Among electrical devices, Good Bye and Casper were marginally superior to Good Knight and Knight Queen.

Field evaluation of deltamethrin against resistant Anopheles culicifacies in Distt. Ghaziabad (U.P.) India.

A field study to evaluate the impact of deltamethrin spraying on DDT and HCH resistant A. culicifacies population was carried out in Razapur primary health centre (PHC), Distt. Ghaziabad (U.P.) India. The PHC comprising of about 0.14 million population was divided into 3 sections of equal size and sprayed with (i) 3 rounds at 12.5 mg/sq m at 6 weeks interval, and (ii) 2 rounds each at 20 mg/sq m and 25 mg/sq m at 8 weeks interval. One section in Dadri PHC located at a distance of about 22 kms was held as control. In this area 3 rounds of HCH were sprayed by the NMEP as was done to control malaria in this district. Deltamethrin spraying was carried out for 3 years. Results revealed that spraying at 12.5, 20 and 25 mg/sq m resulted in drastic reduction of DDT and HCH resistant A. culicifacies population and in the interruption of malaria transmission. In control area high vector densities and malaria transmission was encountered throughout the period of study. Spraying of deltamethrin resulted in build-up of Culex quinquefasciatus population due to resistance.

Insecticide impregnated ropes as mosquito repellent.

Ropes were impregnated with different dosages of deltamethrin and burnt throughout the night in human dwellings and cattlesheds. Smoke from smouldering ropes treated with various dose levels gradually saturated rooms and prevented the entry of mosquitoes. This method provided very good protection from mosquito bites including the principal vector of malaria, A. culicifacies. Results of ropes impregnated with 80 ppm deltamethrin were more consistent than at the lower dosage. The technique is indigenous, cost-effective, simple and appropriate for rural areas and does not require any special skills in its application.

Evaluation of Bacillus sphaericus to control breeding of malaria vectors.

Bacillus sphaericus formulations (Solvay liquid 2362 and Abbott granules 2297) were tested in the laboratory and field for the control of mosquito breeding. Results of laboratory evaluation revealed that both formulations had good larvicidal activity against Culex quinquefasciatus, at 0.04 ml and 1 gm/sq m dosages respectively. Higher dosages at 5 ml liquid or 5 gm/sq m granules were required to control A. stephensi and A. subpictus. A. culicifacies required still higher dosages of Solvay (25 ml/sq m) to achieve same levels of control whereas Abbott granules were not effective even at 20 gm/sq m.
