Centchroman--a non-steroidal anti-cancer agent for advanced breast cancer: phase-II study.

Treatment with Centchroman (3,4-trans-2,2-dimethyl-3-phenyl-4-p-(beta-pyrrolidinoethoxy) phenyl-7-methoxy chroman) has been evaluated in 4 male and 75 female patients with advanced breast cancer. The overall response rate, including both male and female cases, was 40.5%. Among the female patients, the overall response rate was 38.7%. The median duration of response was 6 months. One of the 4 male patients showed a complete response and 2 showed partial responses. The responses were more marked for bone, pulmonary, soft tissue, skin and lymph-node metastases than for liver metastases.

Leprosy in low endemic areas of India: an appraisal and suggested measures for control.

Prevalence of leprosy in the low endemic areas of India is described based on the observations of patients attending an Urban Leprosy Centre in the Union Territory of Delhi from the neighbouring states. The rising incidence in these so-called low to moderate endemic places is closely linked to factors related to urbanisation, movement of people in search of employment, etc., which necessitate fresh surveys in these areas. A significant number of leprosy patients attending the Centre were irregular (37.7%) in therapy and many absconded after the initial visit (35.3%), the reasons for which are discussed. These figures are compared to that from similar low endemic areas and known high endemic parts of the country. Suitable modifications to the control programme in these areas are suggested under the purview of the National Leprosy Eradication Programme.

Rifampicin induced uterine bleeding.

A case of uterine bleeding after intake of rifampicin is reported in a 35-year-old female. Provocation test was also positive. The underlying mechanism whether it was a hypersensitivity phenomenon like fixed drug eruption or due to induction of uterine acyl-hydrolase enzyme, is not clear. Uterine bleeding has not been observed as a side-effect of rifampicin in the past.

Panniculitis following steroid therapy.

A 10-year-old child experienced painful nodular lesions following rapid withdrawal of large doses of orally-administered prednisolone prescribed for treatment of an episode of hepatic encephalopathy. Histopathologic examination of a nodule revealed extensive fat necrosis, needle-shaped clefts within fat cells, and marked nonspecific granulomatous infiltrate that spared the interlobular septa. The lesions subsided completely in about four weeks without any treatment, leaving only residual hyperpigmentation.

Unilateral lichen planus.

A case of lichen planus with unilateral distribution of lesions is reported in a 26-year-old woman. The patient had no associated neurologic signs or symptoms. The role of neurologic influences in lichen planus is stressed.

Glucose tolerance studies in lichen planus.

A study of glucose tolerance and insulin response to oral glucose was carried out in 56 consecutive histologically confirmed cases of lichen planus. 30.3% patients showed abnormal glucose tolerance. The pattern of insulin response was similar to that seen in type II diabetes. Insulin/glucose ratio showed the same relationship as seen in type II diabetes. These results reinforce the possibility of glucose intolerance in lichen planus patients. However, no correlation between glucose tolerance abnormalities and duration and distribution of lesions was observed.

Effect of long-term centchroman treatment on some estrogen-sensitive biochemical constituents in the genital organs of female rhesus monkeys (Macaca mulatta).

Effect of long-term Centchroman (3,4,-trans-2,2-dimethyl-3-phenyl-4-p-(beta-pyrrolidinoethoxy)-phe nyl)-7-methoxy chroman) treatment on some estrogen-sensitive biochemical parameters in the genital organs of female rhesus monkey was studied. Centchroman treatment did not demonstrate any consistent pattern of stimulation or inhibition in any of the estrogen-sensitive biochemical parameters. However, glycogen recorded a gradual decline in uterus, cervix and vagina. The results thus indicate that Centchroman evoked a mixed response probably due to manifestations of both estrogenic and antiestrogenic properties.

Effect of long-term centchroman treatment on plasma steroid and peptide hormones in female rhesus monkeys (Macaca mulatta).

Effect of weekly oral administration of Centchroman (3,4-trans-2, 2-dimethyl-3-phenyl-4-(p-(beta-pyrrolidinoethoxy)-phenyl)-7-met hoxychroman) at 1 and 2.5 mg/kg for 12 months on plasma estradiol, progesterone, luteinizing hormone and follicle stimulating hormone of female rhesus monkeys was studied. Centchroman administration at both doses did not disturb the menstrual pattern of rhesus monkeys except for a prolongation of the first treatment cycle. The subsequent cycles were of normal duration. The general pattern of plasma estradiol, progesterone, luteinizing hormone and follicle stimulating hormone was not affected by Centchroman treatment. The basal and peak levels were similar in pretreatment, control and treatment cycles. The results clearly indicate that Centchroman treatment up to 1 year does not affect the hypothalamo-pituitary-ovarian axis in female rhesus monkeys.
