Glycosylated hemoglobin and diabetic retinopathy.

Estimation of glycosylated hemoglobin (HbA1c) levels by modified Fluckinger and Winterhalter's method was done in 25 normal persons, 25 diabetic patients without retinopathy, and 25 diabetic patients with retinopathy. The HbA1c values were significantly higher in diabetic patients with or without retinopathy than in the control group (P less than .001). In diabetic patients with retinopathy, the mean value of HbA1c was higher in proliferative retinopathy than in background retinopathy, but statistical analysis showed this was not significant (P greater than .6).

Role of Tromaril as anti-prostaglandin drug on blood aqueous barrier.

In this experimental work, it was observed that after anterior chamber paracentesis there was more effusion of protein into the anterior chamber. Pretreatment with aspirin/indomethacin and a new drug, enfenamic acid (Tromaril), decreased this trauma-induced increase in aqueous protein content. It was further observed that this inhibition was more marked with Tromaril pretreatment in comparison with other drugs. Prostaglandin-mediated disruption of blood aqueous barrier can also be decreased with Tromaril pretreatment. Tromaril, a prostaglandin synthetase inhibitor, may help us to control the reactions to surgical ocular trauma and other ocular inflammatory diseases.

Lactate dehydrogenase activity in ocular tumors.

Estimates of lactate dehydrogenase (LDH) activity in aqueous and serum was carried out on 15 cases of benign ocular tumor, 15 cases of malignant tumor, and 15 normal cases. LDH levels were high in serum and aqueous of cases with malignant tumor as compared to benign tumor and control cases. Total LDH activity in serum and aqueous was grossly elevated in malignant tumor cases (specially retinoblastoma).

Clinical and experimental keratitis caused by the Colletotrichum state of Glomerella cingulata and Acrophialophora fusispora.

Two cases of mycotic keratitis caused by the Colletotrichum state of Glomerella cingulata and Acrophialophora fusispora are reported for the first time. Both the isolates produced experimental corneal lesions in rabbit eyes but A. fusispora was more pathogenic. The experimental infection was more severe, with both the fungi, in rabbits pretreated with cortisone as compared with untreated animals. In vitro A. fusispora was most sensitive to miconazole and tolciclate followed by clotrimazole, amphotericin B and lactones while clotrimazole exerted maximum inhibitory effect on Colletotrichum followed by miconazole, lactones, amphotericin B and arnebins. Arnebins and tolciclate were inactive respectively against A. fusispora and Colletotrichum. Of the 3 drugs tested in vivo, against A. fusispora keratitis in rabbit, amphotericin B showed better results than tolciclate and miconazole.

Clinical and experimental keratitis due to Curvularia lunata (Wakker) Boedijn var. aeria (Batista, Lima and Vasconcelos) Ellis.

A case of mycotic keratitis due to Curvularia lunata var. aeria is reported for the first time from India. Direct microscopic examination of the corneal scrapings in KOH (50 g1(-1)), stained with lactophenol cotton blue, revealed brown septate hyphae and the cultures were found positive for fungus. It was identified as C. lunata (Wakker) Boedijn var. aeria (Batista, Lima and Vasconcelos) Ellis (IMI 253204) and its growth was better at 28 degrees C than at 37 degrees C. In vitro, the fungus was most sensitive to amphotericin B followed by miconazole nitrate, clotrimazole and triphenyltin isoselenocyanate 4-picoline which had the same activity. Lactones were least active and tolciclate and arnebins and inactive. The fungus produced experimental keratomycosis in the rabbit cornea and the infection severity as well as histopathological changes were more pronounced in rabbits that received penicillin, streptomycin and cortisone.

Muscle involvement in association with filarial chyluria.

Twenty unselected consecutive patients of filarial chyluria were evaluated clinically, electromyographically, and histopathologically for neuromuscular dysfunction. None of the patients showed clinical evidence of muscle wasting or weakness suggesting myopathy, although generalized muscle weakness was complained by all of them particularly while climbing the stairs or on getting up or during lifting heavy weights. Electromyographic abnormalities were found in nine patients and slight histopathological abnormalities in two. The average duration of motor unit potentials and the mean amplitude were reduced, compatible with myopathy. There were no fibrillation potentials. Histological abnormalities consisted of a marked variation in muscle fibre size, sarcolemmal nuclear proliferation, and mild interstitial fatty infiltration. None of the patients showed evidence of clinical neuropathy, abnormalities in conduction velocity along the nerves or signs of segmental demyelination in the biopsy of the nerve. Our data suggest that muscle weakness in these patients is myopathic in nature without peripheral nerve involvement, and is possible due to hypoproteinaemia and hypolipidaemia, as these patients lose excessive amounts of protein and fat in their urine.

Intradural enterogenous cyst.

A 23 year old woman with an intradural extramedullary enterogenous cyst in the cervical region, unassociated with vertebral cleft, presented with two episodes of neurological disturbance at an interval of five years. Subtotal excision of the anteriorly placed cyst at C6 vertebral level was followed by rapid and complete recovery.

Serum lactate dehydrogenase in detecting liver damage associated with pre-eclampsia.

Assessment of liver damage by estimation of serum enzymes including lactate dehydrogenase (LDH) and its hepatic fraction, LDH5, was made in 60 normal pregnant women, 20 patients with pre-eclampsia and 10 with eclampsia. Definite evidence of damage was found in patients with pre-eclampsia and eclampsia and was related to the severity of the disease.

Predictive value of serum lactate dehydrogenase in head injury.

Serum total lactate dehydrogenase (LDH) was estimated serially for up to 15 days in 110 patients with head injuries who had no extraneural injury. Increase in LDH activity generally indicated the degree of parenchymal brain damage. The prognostic value of LDH was established by correlating its activity with clinical criteria such as duration of unconsciousness and post-traumatic amnesia, type of brain damage, and quality of survival. A significant rise in LDH activity suggested severe brain damage and poor prognosis.
