Duplex examination.

BACKGROUND: Accurate venous diagnosis remains a challenge for the phlebologist and vascular laboratory. In recent years duplex ultrasound has become the preferred method of diagnosis for deep venous thrombosis and deep and superficial venous insufficiency. OBJECTIVE: To describe the physical principles, typical protocol, and indications for venous duplex examination. CONCLUSION: Duplex examination offers a precise, noninvasive technique to make anatomic and functional venous diagnoses.

Correlation of serum ferritin levels and postsclerotherapy pigmentation. A prospective study.

BACKGROUND: Postsclerotherapy pigmentation is a common sequela of sclerotherapy to varicose and telangiectatic leg veins. This pigmentation is thought to be due to hemosiderin deposition in the dermis. A preliminary study indicated that serum ferritin was a good indicator of susceptibility to postsclerotherapy pigmentation. OBJECTIVE: To clarify the relationship between serum ferritin and postsclerotherapy pigmentation using a prospective study. METHODS: Serum ferritin levels were taken from 233 consecutive patients prior to commencement of sclerotherapy. Patients were assessed clinically for the presence of pigmentation at 3, 6, and 12 months. RESULTS: There was a linear relationship between pretreatment serum ferritin level and the occurrence of pigmentation at each posttreatment assessment date. CONCLUSION: The results confirm a relationship between serum ferritin levels and postsclerotherapy pigmentation. This indicates that a significant predisposing factor to the development of persistent postsclerotherapy pigmentation is a high level of total body iron stores.

Copper vapor laser and microsclerotherapy of facial telangiectases. A patient questionnaire.

BACKGROUND: Facial telangiectasia due to photodamage is a common disfiguring condition in modern society. Over the past 10 years yellow light laser photocoagulation has become the treatment of choice. OBJECTIVE: To compare the effectiveness of copper vapor laser alone with combined microsclerotherapy and copper vapor laser in the treatment of facial telangiectases. METHODS: Two hundred thirty-nine consecutive patients were asked to complete a questionnaire evaluating the two different treatments; 180 (75%) patients responded. RESULTS: Both treatments were found to be effective but patients who received the combined treatment gave significantly higher ratings in effectiveness and overall satisfaction. There was also non-significant reduction in the incidence of adverse effects reported by patients treated by the combined method. CONCLUSION: Supplementary microsclerotherapy enhances the effectiveness of copper vapor laser photocoagulation of facial telangiectases.

Recurrent varicose veins. Part 2: Injection of incompetent perforating veins using ultrasound guidance.

Treatment options following duplex evaluation of recurrent varicose veins are discussed and a method of injecting incompetent perforating veins using ultrasound guidance is described. The results of duplex evaluation 6 months postinjection using this technique are presented. These early results indicate that sclerosant injection employing ultrasound guidance is an effective and safe method of treating incompetent perforating veins.

Recurrent varicose veins. Part 1: Evaluation utilizing duplex venous imaging.

There is the need to develop a universally accepted standard investigation for recurrent varicose veins. Duplex venous imaging offers a precise, non-invasive technique to make anatomic and hemodynamic diagnoses. A routine protocol of duplex imaging of recurrent varicose veins is described based on the known recurrent sources of reflux from deep to superficial veins. Results from this protocol indicate that incompetent perforating veins are the most common site of reflux from deep to superficial veins in patients with recurrent postsurgical varicose veins. Other important sites of reflux detected by this method are recurrent communications with the common femoral vein, the saphenopopliteal junction, and incompetent pelvic veins. Duplex imaging is recommended as a safe, non-invasive method of evaluating recurrent varicose veins.
