[Intra-uterine artificial insemination. Indications, techniques, results; 83 cases].

OBJECTIVES: Retrospective study of results after intra-uterine artificial insemination performed in the Poitiers University Hospital. MATERIAL: There were 95 infertile couples who benefited from intra-uterine artificial insemination. Indications involved pathology of the cervix in 53% of the cases and male infertility in 30%. Another cause of infertility was found in 5% and no cause could be identified in 12%. METHOD: The technique was as follows. Ovary stimulation using clomifene and hMG, sonographic monitoring with oestradiol assay, hCG for triggering ovulation, 2 inseminations at 24 and 48 h after swim-up sperm preparation and luteal phase supplementation. RESULTS: The overall success rate was 32.6% with a mean success rate per cycle of 11.1%. The best results were obtained in couples with unexplained infertility and cervix-related infertility (42 and 32% respectively). These results are comparable to those in the literature. CONCLUSION: The results of this series and the data reported in the literature demonstrate the interest of intra-uterine artificial insemination in cases of cervical sterility where it is indicated as first intention treatment. In case of impaired ejaculation, unexplained sterility and certain other causes of male infertility, intra-uterine artificial insemination can be useful but requires further study in order to determine the criteria for indications and the techniques to be used. Associating ovary stimulation increases the probability of success, especially in cervical sterility. Finally, more than 4 to 5 attempts do not increased success rate.

Anatomic and radiologic bases of combined transplantation of liver and small intestine in the pig.

In order to assess the tolerance induced by hepatic transplantation in a multivisceral graft, a model of a transplant combining the liver and small intestine was developed in the pig, using animals of Large White Isogroup O, typed in the system of major incompatibility as "Swine lymphocyte antigen" (SLA), whose weight varied from 30 to 40 kg. In order to limit the duration of warm ischemia, dissection of the graft was performed on the donor animal with respect for the following anatomic features: absence of intestinal attachment to the posterior parietal peritoneum, anti-clockwise torsion of 360 degrees of the intestine around the superior mesenteric a. with the small intestine convolutions on the right and the colonic helix on the left. The bifid pancreas follows this rotation and its resection must respect the vascularisation of the proximal small intestine; the hepatic a. arises from a common trunk with a gastrosplenic branch of the aorta between the crura of the diaphragm and travels sagittally from behind forward toward the liver pedicle, which it reaches ar the upper border of the duodenum; the hepatic a. and its branches are dorsal in relation to the portal vein; the hepatic arterial distribution follows a right-left and antero-posterior systematisation for the six hepatic lobes of this quadruped animal.

[Arterial risks in celiosurgery].

It is important to recognize exceptional vascular events in celiosurgery because of their gravity. Most occur during pneumoperitoneal manoeuvres using a Palmer needle and rarely at the insertion of the first trocar. We performed a tomodensitometric study in 200 subjects free of abdominal pathology in order to determine the distance from the umbilicus to the aortic bifurcation, their projection over the spine and the superficial relations of the large vessels to the cutaneous umbilical cover. Generally, the projections of the umbilicus, and also the aortic bifurcation, lie over L4. The distance between the skin and the large retroperitoneal vesses is generally equal to one-third of the antero-posterior abdominal diameter. In thin subjects with a flat abdomen however, this distance may be smaller, even less than 3 cm. These findings emphasize the importance of "safety" manoeuvres during celiosurgery, especially during the learning period.

[Support and suspension of the pelvic viscera in women. Functional and surgical anatomy].

This work is an attempt to settle the semantics of the pelvic connective tissues based on the international nomenclature (Nomina Anatomica) on the one hand on the other following a study of the macroscopic anatomy together with the microscopic appearances and a functional study together with a radiological study which includes in particular tomodensitometry. The Pelvic viscera are held in position by visceral fasciae which are continuous with the parietal pelvic fascia and the retro peritoneal connective tissue. This connective tissue, which is moderately loose, is arranged around the blood vessels as visceral ligaments. These are more obvious when traction is exerted during surgical operations and it resembles the way strands of a net come together when they are being stretched. It is essential to ligate and cut these ligaments if the viscera are to be freed. The surfaces which join the viscera together (the septum and the retro-pubic and retro-rectal spaces) are very important and from the biomechanical point of view resemble bonds that are stuck together. In fact, their resistance has the effect of interlocking neighbouring surfaces, and so effects a better distribution of pressures that are exerted and of their absorption. The changes that take place at rest and at movement of the pelvic organs during the menopause and during pregnancy and the puerperium have been studied. The role of episiotomy in particular as a step towards preventing urinary incontinence is emphasized. In fact, it lessens the pull on the external fibres of the sphincter muscle of the urethra which are bound up with the tissues of the vaginal wall. We have found in 140 women who have been operated on for stress incontinence that 62% had a tear or an episiotomy that was carried out late and that was insufficiently large; 13.9% had a tear: and 11.4%, on the other hand, had an episiotomy without a tear. On the other hand, in another group of similar women over the age of 45 who had had an episiotomy, 16.17% had stress incontinence.

[Development of the curvature of the cervical spine in utero. Attempted interpretation].

It is conventional saying that the fetal rachis shows only one ventral concavity. An echographic study allowed to observe in utero the rachis of more than a hundred live fetuses. The radius of this curvature gradually increases according to the development, revealed by the disappearance of the cervical curvature under the influence of the weight of the head and the uterine constraints. The sagittal curvatures of the vertebral column explains mans adaptability to the erected position. The authors think that the conventional conception, in which ontogenetic development is comparable with phylogenetic development, needs to be reconsidered: the cervical curvature is a prime curvature setting the evolutionist question about the origins of the rachidian curvatures.

[Ripening of the cervix and induction of labor at term with prostaglandin F2 alpha gel in increasing doses].

The induction of labour at term has a place of choice amongst the numerous obstetric applications of prostaglandins. The present clinical trial (50 cases) showed that F2 alpha prostaglandin, after incorporation in a high-viscosity gel and following insertion into the extra-amniotic space, is capable of accelerating cervical ripening and of inducing myometrial activity close to spontaneous physiological conditions. The use of increasing doses of prostaglandin avoids the development of acute foetal distress. Our experience confirmed the efficacy (8% true failures), the rapidity and the good maternal and foetal tolerance of the induction of labour at term by prostaglandins.

[Antispasmodic agents (author's transl)].

The authors summarize current knowledge of the anatomical and physiological basis of uterine contractions and their inhibition, and review the different antispasmodics used at the present time in France. For each product they describe the pharmacological effects, the mode of administration, maternal and fetal tolerance, indications, contra-indications and efficacy. The choice of product and their association are then discussed. The authors emphasize the value of treatment by these products, and stress the need for careful prescribing, because of the severity of the accidents provoked by overdosage or when their contra-indications are not respected.

Adrenergic beta-Agonists

[Electromyography of the leg. Study during various segmental movements].

Using anatomical landmarks as fixed points, the authors have squared the cutaneous surface ot the leg into areas which cover the following superficial muscles: m. gastrocnemius, caput mediales et caput laterales, m. peroneus longus, m. tibialis anterior, m. soleus. The electrical surface activity, collected in these areas for some segmented movements (dorsal and plantar flexions with and without load) was integrated and quantified. The individual and inter-individual variability in electrical activity was obtained from six subjects and discussed. The typical electromyography of the leg was established for the four segmental movements investigated. This analysis had also lead to determine the most suitable electrode site for each of the superficial muscles. This study confirms the functional role classically attributed to these various muscles.

[Electromyocartography of the thigh. Attempt at a functional analysis of the locking of the knee against resistance].

Electromyocartographic analysis of the extension of the leg in different positions has shown that: - on the whole energy developped by all superficial muscles of the thigh is more important in the position of dorsal decubitus than in a seated position; - functionnal activity of the rectus femoris muscle varies according to different subjects. Its activity is lower in the seated position, compared to dorsal decubitus, a classic notion; - in contradiction to classic notions, the most active muscle electrically is the vastus lateralis muscle. The vastus medialis muscle is however also very active; - from these results, it is possible to obtain a smaller variability owing to their relative quantification. One observes a hierarchy of electric activity levels of different surfaces, that constitutes the basis of functionnal and clinical study.

Electromyography

[Significance of echotomography B in the analysis of pronation].

By an ultrasonic technique (B. scan), the authors have analysed dynamically, the wrist during pronation. They could show that when the elbow is bended at 90 degrees, the distal tip of the radius effects a circular movement of great radius but low grade value, while the ulna draws in the opposite direction a nearly complete small circle.

[Electromyocartography of the posterior and inferior wall of the abdomen during extension of the torso].

Analysis of the functions of the muscles of the inferior and posterior regions of the trunk was carried out by electromyocartography for the basic movements of extension of the trunk in ventral decubitus. For a given subject, the results varied little with time. By contrast, the results were substantially the same from subject to subject in the case of only one movement : extension of the caudal portion of the trunk combined with extension of the hips. A functional balance-sheet can be based only on this movement. The sacro-spinalis (without attempting to determine the respective roles of the various bundles) seems to be the only one involved in certain movements of extension of the trunk: Its action is at a maximum, highly variable, when the extension involves both the caudal and the cranial portions of the trunk; It is considerable in the extension of the caudal portion alone, and Moderate in the extension of the cranial portion alone.
