[Subjective sequelae of splenectomy].

111 of 806 former patients splenectomized at the Würzburg university hospital during the years 1968-1983 were interviewed for their complaints since their operation. Investigations included the use of Beck's inventory for measuring depression and the Giessen questionnaire for the evaluation of general complaints used in psychosomatic medicine. There was a significant increase of symptoms after splenectomy as compared to the normal population. A special list of 18 questions to investigate typical postsplenectomy complaints was answered by 95 of these 111 persons and by a control group of "statistical twins" with similar upper abdominal surgery without splenectomy. The leading difference was the highly significant increase of the susceptibility to infections after splenic loss followed by accelerated exhaustion and increased physical and mental weakness. The distribution of other complaints like for instance the intolerance to alcohol hitherto judged as typical sequelae of splenectomy was not statistically different in both splenectomy was not statistically different in both groups. Since the symptoms listed above as significantly increased in the splenectomized patient were closely correlated with the susceptibility to infection they seem to be rather the expression of the decreased resistance than direct consequences of the loss of the spleen.

Adolescent

[Long-term sequelae of splenic loss].

Beside the increased risk of infection there are further long lasting consequences of splenectomy. The hematologic changes include morphological alterations of red cells, lymphocytosis, monocytosis and increased blood viscosity. There are complaints about general disturbances like physical weakness, abdominal pain or intolerance of alcohol. The response to certain vaccinations is impaired especially in the early postoperative period. Findings concerning the influence on the growth of cancer are controversial. There seems to be an increased risk of disturbed wound healing.

Follow-Up Studies

Incidental splenectomy: early and late postoperative complications.

Of 542 splenectomies performed from 1964 to 1977, 242 were incidental to gastrointestinal surgery. The highest rate of complications was observed in these patients as opposed to those who underwent splenectomy for trauma or Hodgkin's disease. Eighteen laboratory studies did not show significant changes 1.5 to 10 years postoperatively in 96 patients. Ninety-three patients questioned because of late symptoms had the highest rate of complaints after incidental splenectomy.

Transcutaneous PO2 measurement on skin transplants.

Trnascutaneous PO2 measurements are suitable for an objective control of free skin transplants and pedicle flaps if used with standardized and controlled hyperemia. They are also suitable for monitoring the rejection of skin allografts and to investigate the influence of drugs on skin transplants in general.

[Changes in ionized calcium and citrate levels in dogs during mechanical autotransfusion with heparin, ACD and CPD].

The control of ionized calcium (Ca++), total calcium, and citrate levels in serum were determined in dogs during autotransfusion (AT) of blood stabilized with heparin, ACD (formula B) and CPD. Blood samples were taken according to the changes of aortic pressure (AOP), which was continuously monitored. Taking the values during the stable phase of AOP preceding the AT as baseline, Ca++ dropped by 27% with ACD and by 34% with CPD at the maximum decrease of AOP immediately after the AT. The corresponding increase of citrate was 174% with ACD and 521% with CPD, while total calcium remained stable. Thus cardiac depression after AT of citrated blood seems to be mainly caused by the drop of Ca++, which is significantly more pronounced with CPD, corresponding to the higher content of citrate.

Transcutaneous PO2 measurement for evaluating the oxygen supply of skin allo- and autografts. Theory and practical application.

The condition of skin autografts and allografts in rats and rabbits was checked by transcutaneous PO2 measurement according to R. HUCH, D. W. LUBBERS and A. HUCH. The tcPO2 values obtained with standardized maximal hyperemia allow conclusions concerning the oxygen supply and thus the vascularization of a transplant. This method seems suitable to monitor the healing of autografts and to register quantitatively the progress of rejection in allografts.
