[Induction of fetal lung-maturity. Part III: surfactant-parameters in amniotic-fluid and fetal lung].

In animal experiments we determined surfactant-specific parameters of premature fetal lungs and of samples of amniotic fluid taken fromthe same animal, and compared them with each other. These parameters were, among others: total phospholipids, lecithine and palmitic acid. In these experiments, 75 gravid Wistar rats were treated with NaCl (controls) or drugs in clinically relevant doses for stimulating the fetal synthesis of phospholipids like betamethasone, bromhexine metabolite VIII and D-L-carnitine hydrochloride. A relation between the amniotic fluid and the fetal lung with regard to the quantity of surfactant substances was found - with a security of 95% - with the controls only. The value of amniotic fluid analysis is discussed under this aspect.

[Pharmacologic influencing of fetal phospholipide synthesis. Part II: Carnitin - a new way of dyspnea syndrome prophylaxis? (author's transl)].

Carnitine is a body-owned Betaine which is wide-spread in living nature in the form of a physiologic metabolit. On account of its central metabolic function, Carnitine has various effects in the intermediary metabolism. Toxicity of Carnitine is very low. DL-Carnitine hydrochloride (30 mg/kg body weight) was injected into 25 gravid Wistar rats and 24 gravid rabbits, 4 days before the Cesarean section. Compared to the group of controls, the group treated with Bromhexine metabolit VIII, as well as to the group treated with Betamethason, significantly higher values with regard to the content of total phospholipids, lecithine and palmitic acid could be found in the fetal rat lungs. Effects of Carnitine on the phospholipid metabolism are discussed.

[Pharmacologic influencing of fetal phospholipide synthesis. Part I: Effect of betamethasone and bromohexine metabolit VIII (Ambroxol) (author's transl)].

The effect of Betamethasone and Bromhexine-Metabolite VIII, commonly used in RDS-prevention, was studied in pregnant rats and rabbits. 73 rats and 78 rabbits were treated for 4 days with doses, similar to those used in men. Cesarean section was performed in rats from the 18th-21th day of gestation (23 days), in rabbits from 27th-30th day (32 days). In the amniotic fluid, in fetal and maternal lungs the total phospholipid content, percentage of lecithins in total phospholipids and palmitic acid were determined and used as parameters for the surfactant content. There was no sign of induction with regard to synthesis. Premature deliveries and malformed fetusses were frequently observed under steroid-therapy. Clinical application should therefore be observed more critically.

[The practicability of the gravlee-jet-wash-technique--results of a laboratory (author's transl)].

The matter of examination were 171 jet-wash-proofs of 166 patients. In working up the material we aimed to get "sedimentsmears" and "specimens" by the aid of the cellbloc-technique. In the case of seventeen patients there was no material to be evaluated. Counting the 13 cases which were found necessary to be repeated because the quantity of material was too little, 17,5% of the cases finally could not be diagnosed. Using the histology of curettage, or the surgery preparations of 148 patients, we had something to compare with the results of the jet-wash-proofs. 11 out of 13 endometrial carcinomata (84,6%) were registered by the aid of the jet-wash-technique; this was contrasted with two false positive results out of 148 cases. The reasons of false positive and false negative results are being discussed. The efficiency of the jet-wash-technique depends on the prooftaking method, the way the material is worked up, and the kind of the dammage of the endometrium. The results show, that the use of a jet-washer is now, after a phase of learning, a diagnostical completion for the recognition of the endometrial carcinoma in high risk patients.

Prenatal prediction of respiratory distress syndrome. Measurement of surface properties and Lecithin/Sphingomyelin ratio in human amniotic fluid.

118 samples of amniotic fluid were taken from 102 patients in the 25th to the 43th week of pregnancy by amniotomy or by transabdominal amniocentesis. 76 samples were obtained from patients with normal pregnancies without evidence for either induction or retardation of lung maturity; they served to establish normal values. In 37 cases samples were taken within 72 hours prior to delivery, the decisive period for the prediction of a respiratory distress syndrome. In seven cases a respiratory distress syndrome occured which was diagnosed by an independent pediatrician. The following tests were performed for all samples: 1. Surface tension was measured biomechanically using the Wilhelmy balance and a surface tension area diagram (Fig. 1) was made: A platimum plate is immersed into the liquid placed in the trough; the surface tension is measured via a transducer. The surface film of th liquid to be studied is compressed cyclically from an initial area of 100% to 20% by a moveable barrier and is expanded afterward. Platinum plate and barrier are connected to an X-Y recorder. The surface tension area diagrams are interpreted on the basis of the following criteria: see article 2. The determination of the L/S ratio (simultaneous comparison with the standard method): The method of GLUCK as modified by BORER(planimetric interpretation of the the spots) was employed. Both methods produced normal values. For the biomechanical measurements a classification into three groups was made (25th-30th, 31st-35th, 36th-43rd week of pregnancy) (Tab.I);the L/S ratio showed the usual steep increase from the 36th week of pregnancy onwards. In cases with RDS (Tab.II) surface tension-area-diagrams show high values for gamma max and a slight decrease in surface tension on compression (high gamma min values); the stability index is low, the hysteresis is reduced. Up to the 34th week of pregnancy the 2:1L/S ratio assumed as the threshold for a sufficient surfactant concentration does not permit a clear interpretation; in subsequent weeks all cases fail to coincide with the normal pattern of distribution (Fig.2). Both the L/S ratio and the criteria of the surface tension area diagrams show significant differences in cases with and without RDS. The prognostic validity of both methods was tested for the probability of predicting occurrence of RDS, probability of RDS occurence and probability of non-occurrence of FDS. The prognostic value seems equal to the L/S Ratio.

Amniotic Fluid

[Biomechanic examination of prenatal lung maturity. Testing of amniotic fluid specimens and lung homogenates on Wilhelmy's scale].

In all traditional methods of determining fetal lung maturity the content of phospholipids in the amniotic fluid is used as parameter. The function of the other parts of the surfactant is not taken into account. The dynamic surface tension measurements using the Wilhelmy balance simulate the interplay of the whole biological system. We have tested 76 specimens of amniotic fluid and 17 lung extracts from 25 weeks' gestation onwards using the biomechanical procedure. In addition, the L/S ratio in the amniotic fluid was determined. There was a correspondence between the values of the amniotic fluid and the lung measured in the same week of pregnancy. The results of the surface tension measurements show a good correlation with the standard method. The following are the advantages of the Wilhelmy balance: results show a small range of dispersion, procedure can be easily learned and carried out quickly and simply.

[Opportunities for the application of the prostaglandins in gynecology. Interruption of pregnancy in acute leukosis. Description of a case].

Prostaglandin is being used increasingly as an abortifacient, especially in infans mortuus, fetus mortuus, interruption of pregnancy, missed abortion and hydatidiform mole. The most commonly used form of application is extraamniotic instillation. Interruption of pregnancy during acute leukosis of pregnancy is an extension of the list of indications. One such case is reported. The reliable and rapid onset of action and the small loss of blood suggest that prostaglandins may be the drug of choice.
