[The value of 2- and 3-dimensional computed tomography in the diagnosis and classification of midfacial and orbital fractures].

The value of 2D- and 3D-computed assisted tomography in detecting and classifying midfacial fractures and craniofacial deformities acquired by gunshot injuries was assessed in 123 resp. 26 patients and compared with the postoperative results. In a comparative study 37 patients were prospectively examined by pluridirectional tomography. Examinations of a macerated skull were performed to optimize 3D investigation technique. High resolution CT proved to be the most reliable technique in detecting midfacial fractures and associated soft tissue injuries. 3D-imaging of bony surfaces allowed clear demonstration of fraction extent and fragment displacement without offering additional detail information. 3D-based computer-assisted manufacturing of individual models facilitated surgical planning of complex midfacial injuries.

P6vices for hydrodynamic simulation are required in a variety of studies such as device evaluation, cardiovascular modeling and for student training. Most studies today use different, incompatible circuits, which must be redesigned for every new application. To obtain a universal apparatus, a unitized system with standard connectors was developed. Three types of connectors were selected: 1" flange connectors, 1/2" tubing connectors and Luer-connectors with a 2 mm lumen. The complete system consists of reservoirs, throttles, valve holders, adapters for Doppler ultrasound probes, and converters to link these basic diameters. The apparatus can be driven by membrane, centrifugal and geared pumps. The system has successfully been used in echocardiographic studies of stenosis and valvular insufficiency, for pulse propagation in vascular grafts, and to test the hydraulic performance of cardiac assist devices. Flow rates between 0.1 and 30 l/min and pressure gradients up to 250 mmHg were achieved. In practical use, the system can be adapted to suit various investigations, with minimal expense. Standardization of the parts and connectors results in simple documentation and good reproducibility.

[Hyper-HDL-cholesterolemia: prevalence and clinical aspects].

The prevalence and clinical aspects of high-HDL-cholesterolemia were investigated in 2000 patients (1215 males and 785 females) aged 14-88 years in our medical outpatient department. The incidence of high-HDL-cholesterolemia was 7% (greater than 1.68 mmol/l) in males and 9.9% (greater than 1.94 mmol/l) in females and was associated in 80% of cases with a normal lipid profile according to the Fredrickson classification. Compared with patients with low-HDL-cholesterolemia, high-HDL-cholesterolemia distinguished by a lower incidence of obesity, of hyperlipidemia, of diabetes and of cigarette smoking. In contrast, daily alcohol intake was significantly higher. Fewer patients had a history of ischemic cardiovascular events. High-HDL-cholesterolemia appears to arise from both environmental and genetic factors.

[Wound infection following appendectomy. Metronidazole vs ornidazole as single-dose prophylaxis in non-perforated appendix].

In an open prospective randomized study, the postoperative wound infection rate following removal of an unperforated appendix was evaluated in 187 patients who received either metronidazole (1 g suppositories) or ornidazole (500 mg iv or 500 mg suppositories) in a single dose preoperatively. The overall postoperative infection rate was 2.1%. Metronidazole and ornidazole suppositories are a cheap method of preventing postoperative infection in cases of unperforated appendix.
