Hypoxia modulates rapid effects of aldosterone on oxidative metabolism in human calf muscle.

Non-genomic effects of aldosterone on oxidative metabolism of skeletal muscle have been shown, recently. To further characterize these rapid effects on the increase of phosphocreatine (PCr) in the recovery period after isometric exercise, a randomized cross-over placebo-controlled study was conducted on 9 healthy volunteers. Hypoxia was chosen to test the dependence of the effect on oxygen supply and thus its relation to oxidative vs non-oxidative metabolism. Parameters related to the energy metabolism of calf muscles were measured by 31P magnetic resonance spectroscopy during four repetitive contractions in four different tests (hypoxia [FiO2=0.13] vs normoxia [FiO2=0.21], +/- 0.5 mg aldosterone). The area-under-curves of post-exercise PCr levels after the 4th contraction were significantly increased by aldosterone vs placebo during normoxia (875.5 +/- 5.1 vs 857.2 +/- 8.3%-min; p=0.02). In addition, aldosterone induced an undershoot of inorganic phosphate (Pi) in the recovery after isometric exercise (77.5 +/- 5.4 vs 88.9 +/- 5.1 mmol/l x min; p=0.05). Hypoxia blocked effects of aldosterone on PCr overshoot and Pi undershoot. Concentrations of ATP, ADP, phosphomonoesters, and intracellular pH were not affected by those interventions. In conclusion, these data demonstrate rapid actions of aldosterone on post-excercise PCr and Pi levels in human calf muscle, which are blocked by hypoxia. We hypothesize that aldosterone rapidly interferes with oxidative metabolism probably by direct modulation of ATP-turnover, which is induced by an oxygen-dependent imbalance of the ATP-synthesis and utilization rate.

[New European medical flight safety guidelines. Their significance for the ophthalmologists].

In 1996 the new European Flight Crew Medical Requirements were finalized. The ophthalmological changes compared to the existing German standards range from the organization of evaluations to the necessary frequency of examinations. They also cover different examination techniques and different criteria for fitness. The changes in the requirements for ophthalmological fitness primarily affect ametropia, anisometropia, color vision and phoria. Knowledge of the European requirements is very important to advise pilots and any patient interested in becoming a pilot.

Intranasal application of atrial natriuretic peptide and 1-deamino-d-arginine vasopressin in healthy volunteers. Hemodynamic, hormonal, and renal excretory effects.

In the present study we investigated the effects of an intranasal administration of 25 micrograms alpha-human atrial natriuretic peptide (alpha-hANP) dissolved in 0.2 mL 0.9% saline on renal excretory function, blood pressure (BP), heart rate (HR), and also its interaction with the renal effects of 20 micrograms deamino-d-arginine vasopressin (d-DAVP) in 10 healthy volunteers. After two 30-min control periods plasma concentrations of ANP and cGMP rose significantly from 14.8 +/- 1.8 pmol/L and 5.9 +/- 0.3 pmol/mL to 23.3 +/- 2.3 pmol/L and 6.6 +/- 0.4 pmol/mL (P less than .05), respectively within 30 min after ANP administration. The levels returned to basal values after 60 min. Urinary cGMP excretion initially rose from 17.9 +/- 3.6 to 46.8 +/- 3.7 pmol/30 min and then returned to control values, whereas plasma renin activity and plasma aldosterone concentration decreased significantly after 30 and 60 min (P less than .05). Systolic and diastolic BP declined slightly by 8% and 7%, respectively, while HR remained unaltered. Urine flow rate and sodium excretion increased by 321% and 190%, respectively (P less than .05). These changes were also significant when data were compared with a time-matched placebo study performed on the preceding day with intranasal administration of 0.2 mL 0.9% saline alone. After 48 h the protocol was repeated but 20 micrograms d-DAVP was administered intranasally 120 min before an intranasal application of 25 micrograms alpha-hANP. Within 120 min d-DAVP had reduced urine flow by approximately 50% (P less than .05), while sodium excretion remained unaltered.(ABSTRACT TRUNCATED AT 250 WORDS)

Inhibition by ketotifen of idiosyncratic reactions to aspirin.

Ketotifen administered prior to aspirin offered protection against bronchoconstriction in 13 of 14 patients with aspirin-sensitive asthma. In four other subjects, suffering from urticaria/angioedema following ingestion of aspirin-like drugs, pretreatment with ketotifen resulted in total prevention of the adverse reactions. These results support the suggestion of a common pathogenetic mechanism operating in two distinct clinical patterns of idiosyncrasy to aspirin and other cyclo-oxygenase inhibitors. They also indicate that ketotifen might find application in treatment of adverse reactions to aspirin.

Athletic endurance training - advantage for space flight?: The significance of physical fitness for selection and training of spacelab crews.

While intensive physical exercise has been part of the conditioning of astronauts and consmonauts for spaceflights, its benefits have been questioned. After reviewing the pertinent literature, it is concluded that the morphological and functional changes obtained with athletic endurance training are rather specific and of no general advantage for the tolerance to space stresses. Particularly during gravitational loads, in the relaxed subject, these changes allow a more pronounced shift of fluid into the lower extremities, with the possible consequence of a reduced tolerance. This unfavourable response, obviously, is accentuated through immersion and weightlessness. The aerobic work capacity is also more impaired in athletes. Based on these conclusions, recommendations for crews and passengers of future Spacelab missions are given with respect to selection and pre-and in-flight physical exercise.

Air operations and circadian performance rhythms.

This paper reviews experimental results and pertinent data from the literature on circadian behavioural rhythms and their modifications through various factors. It relates them to the operation of aircrews "round the clock" and on transmeridian routes and discusses some possibilities of an appropriate scheduling.
