Group psychotherapy for adolescents comorbid for substance abuse and psychiatric problems: a relational constructionist approach.

The group psychotherapy of adolescents comorbid for psychiatric and substance abuse problems is fraught with inconsistent attendance, frequent verbal and physical disruptions by the adolescents, and intense countertransference toward patients and their parents on the part of treating staff. A relational constructionist approach assists staff and patients in overcoming these obstacles and in focusing on the adolescents' search for personal meaning. Multiple group psychotherapy conducted in a combined day treatment and high school program is described and illustrated by case examples during the course of a year of treatment. These examples illustrate the importance of a relational non-hierarchical approach to adolescent patients by treating staff.

Anger management for adolescents: efficacy of brief group therapy.

OBJECTIVES: To investigate the efficacy of a brief, manual-based group therapy for adolescents with poor anger control. A previously developed anger management treatment package of 10 to 12 sessions was condensed to a 4-session package to be given within 2 weeks. Immediate effectiveness and the transfer of skills were investigated; anger management skills not only had to be acquired, they also had to be used in the adolescents' natural social interactions. METHOD: Fifty adolescent psychiatric inpatients were selected for high levels of anger and randomly assigned to treatment or control conditions. Pre- and posttreatment measures were administered to subjects and adults who rated the subjects' behaviors. RESULTS: Pre/post self-report measures, as well as behavior ratings by adults, indicated that the patients who went through the anger management series exhibited significantly improved skills. CONCLUSIONS: These results suggested that the intervention for adolescents was effective even though it was condensed, but it should not be further abbreviated.

Relationship between high fasting blood sugar and depression in a mental hygiene clinic population.

The relationship between diagnosis and abnormally high fasting blood sugar levels was reviewed for 4,994 patients in a mental hygiene clinic. Fifty-four patients exceeded the criterion level of fasting blood sugar abnormality, FBS greater than or equal to 145 mg%. There was no evidence of relationship between psychiatric diagnosis and the presence of an abnormally high fasting blood sugar level. This held true for the diagnosis of depression despite observations made elsewhere that a significant association exists between the depressions and diabetes.

Clinical laboratory abnormalities and their follow-up in a mental hygiene clinic.

In a retrospective study, there were clearly abnormal laboratory findings in 4.8 per cent of 4,994 adults who attended for psychiatric evaluation at a mental hygiene clinic. In particular high fasting blood sugars, abnormal blood counts, eosinophilia, VDRL and abnormalities of chest X-ray or urinalysis were found. Just under half of these abnormalities led to further medical follow-up. Old and young, schizophrenic and neurotic were equally likely to be followed up, but tests which were clinically not alarming might be ignored. The implications are discussed.
