Does punishment hurt? The impact of aversives on the clinician.

This paper explored the extent to which the use of mild versus strong aversives in programs for clients with autism affected the degree of job stress reported by direct care staff on the Maslach Burnout Inventory. The subjects included 56 staff from programs using only mild aversives and 60 staff from programs using strong aversives. Subjects anonymously completed the inventory and mailed it directly back to the researchers. The most significant finding was that direct care staff in facilities which permitted the use of strong aversives reported more intense feelings of personal accomplishment on the inventory than did subjects whose programs were limited to the use of mild aversives. Furthermore, there was a significant relationship among the personal use of strong aversives, number of years in the field, and the intensity of Personal Accomplishment. These findings suggest that allowing staff to use a wide range of interventions including strong aversives may diminish job stress and enhance one's sense of personal efficacy.

Cognitive appraisals in high- and low-stress mothers of adolescents with autism.

The present study examined cognitive appraisals in 8 high-stress and 8 low-stress mothers of autistic adolescents. Subjects' cognitive appraisals, based on videotaped scenes of autistic adolescents engaged in everyday activities, were assessed. High-stress mothers reported more threatening appraisals than did low-stress mothers; low-stress mothers were more likely to reappraise threatening perceptions in benign terms. Furthermore, high-stress mothers reported greater negative emotional reactions to the scenes. Trends toward fewer resources and higher levels of maladaptive behaviors in the autistic adolescents of high-stress mothers were noted. The implications of these findings on stress in the day-to-day lives of mothers with autistic adolescents are discussed.

Percutaneous transhepatic internal drainage using polyurethane double-pigtail endoprostheses.

Relief of obstructive jaundice by the percutaneous transhepatic insertion of an endoprosthesis is now a well recognised and common radiological procedure. We report the successful insertion of polyurethane double-pigtail stents (made for transpapillary endoscopic insertion) by the percutaneous transhepatic route in five patients. Our experience indicates that these stents are as effective in relieving jaundice as, and less traumatic and easier to insert than, the larger varieties in common use.

Traumatic renal arteriovenous fistula.

A case of a 29-year-old man who developed a renal arteriovenous fistula after a gunshot injury is reported. The patient presented with renovascular hypertension associated with high putput cardiac failure, both of which were reversed by surgery.

Effect of hypertransfusion on bone marrow restoration after localized depletion.

The reconstitution of marrow in a mechanically depleted medullary cavity was compared in normal and hypertransfused mice. Polycythemia (hematocrit of approximately 70%) prevented the emergence of recognizable erythroid cells but not of erythropoietin-responsive cells as revealed by autoradiographic studies with 55Fe after erythropoietin treatment. The similar regeneration of erythropoietin-responsive cells in normal and hypertransfused mice was shown by comparison of 55Fe labeling in the repopulating relative to the contralateral marrow; in each case, labeling in the repopulating marrow was about 20% of that in the intact contralateral marrow. Polycythemia did not influence the recovery of granulocytic elements in the regenerating marrow even though the myeloblast-metamyelocyte population of the intact contralateral marrow was increased. The findings are intrepreted to support the concept that hematopoietic stem cell commitment to one or another pathway of differentiation is basically a local phenomenon.
