[Rehabilitation in asthmatic diseases].

The early recognition of an asthmatic disease is important for optimal treatment and rehabilitation with a long lasting success. The knowledge, that bronchial asthma is always accompanied by a chronic inflammatory process of the airways, necessitates the early application of antiphlogistic substances like glucocorticoids as well as weaker substances like DNCG and nedocromil. Therefore, the basic therapy of patients with asthma of the severity II to III should be a combination of beta-2-agonists and antiphlogistic drugs. The pharmacological therapy has to be accompanied by a practical health training (ashtma and antiobstructive training). This is the only way to accomplish an optimal treatment with a correct inhalation, professional usage of ventilation aids, and a convincing recording of peak-flow-values. Rehabilitation of patients with asthma is team work. Beside physicians, co-therapists like psychologists, dietitians, and trained nurses have to be employed.

[Effects of cimetidine and ranitidine on blood coagulation].

The influence of cimetidine and ranitidine on blood coagulation and plasmatic fibrinolysis were assessed in vitro and in vivo. After an intravenous bolus of 200 mg cimetidine a marked prolongation of plasmatic thrombin time was observed, which however did not occur after 50 mg of ranitidine. In vitro addition of cimetidine (0.12 mg/ml) and ranitidine (0.06 mg/ml) again led to prolongation of plasmatic thrombin time only after cimetidine, reaching double the initial value. This requires application of cimetidine in acute intestinal haemorrhage not as a bolus, but only by drip infusion. Continuous application of 600 mg over 12 hours did not result in changes of coagulation values. In contrast, ranitidine may be administered as a bolus. Bolus injections of 50 to 100 mg did not result in any prolongation of plasmatic thrombin time. The antifibrinolytic effect of both H2-receptor antagonists was investigated in the streptokinase-induced fibrinolysis spindle of the thrombelastogram. Clear-cut antifibrinolytic properties were not demonstrable for either preparation with this method.

[Thrombolytic treatment of venous thrombi with combined use of streptokinase and human plasminogen (author's transl)].

Thirteen patients with up to two-week-old phlebothromboses were treated with streptokinase and human plasminogen according to a fixed dosage scheme. In six cases reopening of the vein was complete, in three cases partially complete. In four cases thrombolysis had to be stopped due to macrohaematuria. Haemorrhagic complications occurred mainly after the third day of treatment. Substitution of human plasminogen during continuous streptokinase administration is, in the case of demonstrable lowering of the plasma plasminogen level, a valuable addition to the thrombolytic concept.

[Atypical course of human salmonellosis (author's transl)].

During the last few years atypical forms of human salmonellosis were observed in 27 patients. Most frequent were acute gastroenteritis with bacteraemia or septicaemia, abscess formations in various organs and septic infections with gastroenteritis Salmonellae but no demonstrable gastroenteritis. In addition there were cases of osteomyelitis, peritonitis, one case of infected hip replacement, one of infected adrenal cortical tumour, and one of infected aortic aneurysm. In 24 of the 27 patients there was an underlying disease likely to have favoured the development of such infections. Diabetes mellitus, cholelithiasis and malignant tumours were the most frequent condition. Isolation of the positive organism was obtained from various materials (blood, abscess pus, gallbladder smear, operative specimen). Chemotherapy is definitely indicated in such Salmonella infections. Often additional surgical measures, e.g. to control spread of septic foci, were necessary. Prophylactic chemotherapy is recommended for patients with risk factors in order to prevent bacteraemic-metastatic events.

[Phlebography for pre- and posttherapeutic control of fibrinolysis and thrombectomy (author's transl)].

The indications for fibrinolysis or thrombectomy when there is suspected fresh ilio-femoral venous thrombosis should be based on accurate phlebography at the earliest possible moment. Early posttherapeutic phlebography under standardised conditions should be used to control further treatment. Later examinations document the final outcome. Early phlebography, when treatment has been discontinued and has been unsuccessful, shows the so-called "Rischhalte phenomenon", which may be an indication for further thrombectomy. Seven cases are reported in which primary fibrinolysis with streptokinase was followed by thrombectomy with a Fogarty catheter. 3-Step-phlebography provides the essential information which governs treatment and demonstrates its results.

[Results of pacing after acute myocardial infarction (author's transl)].

Indications for electrical pacing were present in 58 of 665 patients with acute myocardial infarction (8.7%). Posterior-wall infarction had occurred in 34, anterior-wall infarction in 20, while two had combined infarction and in a further two precise localisation was not possible. There were 16 deaths (27.6%), six of them with posterior-wall and nine with anterior-wall infarction. Bradycardic arrhythmias with A-V block predominated among posterior-wall infarctions, hemiblocks and bifascicular block in anterior-wall infarctions. Permanent pacing was practised in 14 patients, 11 with posterior and three with anterior-wall infarctions, i.e. permanent pacing was four times as common after posterior than anterior-wall infarctions, the proportion being 2:1 for temporary pacing, largely due to a higher mortality-rate after anterior-wall infarction. Old anterior-wall scars were present in nine of eleven patients with permanent pacing. Because of the danger of late bradycardia or A-V block patients with posterior-wall infarction should be carefully followed in the late phase (from about the third week onwards), especially if it is known that they have an old anterior-wall infarct.
