SPECT neuropsychological activation procedure with the Verbal Fluency Test in attempted suicide patients.

Performance on the Verbal Fluency Test, as a measure of the ability of initiating processes, is reduced in depressed suicidal patients. The hampered results in this prefrontal executive task parallel the reduction in prefrontal blood perfusion and metabolism in depressed subjects. A neuropsychological activation study with the verbal fluency paradigm could evaluate a possible blunted increase in perfusion in the prefrontal cortex in depressed suicidal patients. Twenty clinically depressed patients who had recently attempted suicide and 20 healthy volunteers were included in a single photon emission computed tomography (SPECT) split-dose activation study following a verbal fluency paradigm. Statistical parametric mapping was used to determine voxelwise significant changes. Differences in regional cortical activation between the letter fluency and category fluency tasks in attempted suicide patients were found. These patients showed a blunted increase in perfusion in the prefrontal cortex. Methodological restrictions concerning group uniformity, medication bias and subjective effort of the participants are discussed. Our findings indicate a blunted increase in prefrontal blood perfusion as a possible biological reason for reduced drive and loss of initiative in attempted suicide patients.

Characterisation and disintegration properties of irradiated starch.

Irradiation treatment could provide a quick and simple way to modify the physical, chemical and pharmaceutical properties of biopolymers such as starch. Corn, potato and drum dried corn starch were exposed to X-ray and electron beam (e-beam) irradiation treatment at doses of 10, 50 and 100 kGy. The disintegration properties of these starches were compared using alpha-lactose monohydrate tablets containing 5% (w/w) starch as disintegrant. Starch solubility increased, while its swelling capacity decreased with increasing irradiation dose. The irradiation treatment caused fragmentation of the amylopectin fraction. Irradiation modified the different starches thoroughly, showing remarkable differences in disintegration properties after X-ray treatment and e-beam modification. The e-beam modification resulted in significantly higher disintegration times of the tablets.

Regional brain activity during different paradigms of mental rotation in healthy volunteers: a positron emission tomography study.

Positron emission tomography (PET) was used to observe changes in regional cerebral blood flow (rCBF) in 10 right-handed healthy volunteers performing two paradigms of mental rotation. In one paradigm, subjects mentally rotated a single alphanumeric stimulus to determine whether it was shown in a normal or mirror-image position. In a second paradigm, subjects mentally rotated and compared pairs of figurative stimuli to determine whether the stimuli were identical or mirror-images. In both paradigms, rCBF was compared with a control task that used identical stimuli, but required no mental rotation. Mental rotation of single alphanumeric stimuli engendered activation in the primary somatomotor area in the left precentral gyrus. Mental rotation of paired figures engendered activation in the left superior parietal lobule and the right frontal medial gyrus. A deactivated area was located in the medial part of the left superior frontal gyrus. Comparison of both paradigms revealed that the left gyrus precentralis was activated significantly during the alphanumeric condition and that the left gyrus lingualis was significantly activated during the paired figures condition. Motor processes may be an inherent part of every mental rotation but the type of motor involvement appears strongly dependent on the specific task or the specific stimuli. Similar paradigms, designed to isolate the same cognitive function, in the same subjects, using the same imaging technology and methodology, but differing only in stimulus material, lead to different areas of neural activation. Task specificity determines the most significant changes in cerebral blood flow in different mental rotation paradigms.

The classical stroop interference task as a prefrontal activation probe: a validation study using 99Tcm-ECD brain SPECT.

This study investigated the feasibility of brain single photon emission computed tomography (SPECT) functional imaging in a neuropsychological test setting, following a single-day protocol with a split-dose paradigm. The Stroop Color Word Test (SCWT) is an example of a well-documented prefrontal activation task. In a split-dose protocol, ten right-handed healthy volunteers were injected twice with 370 MBq 99Tcm-ethyl cysteinate dimer while performing consecutively both series of card-reading of the SCWT. Images were reconstructed using filtered back-projection and normalized to a standard template in Talairach coordinates. Statistical Parametric Mapping (SPM96) was used to determine voxelwise significant changes. A first activation cluster was found in the left medial prefrontal cortex, consisting of the gyrus cinguli anterior and the gyrus frontalis medius and superior. A second activation cluster included the right gyrus frontalis dorsalis and medius. These findings confirm to a large extent the results of previous functional magnetic resonance imaging, positron emission tomography studies of Stroop-like tasks. The choice and validity of various methodological characteristics of the experimental design leading to these results is critically discussed. It is concluded that brain SPECT activation with the Stroop Color Word Test under standard neuropsychological conditions in healthy volunteers, is both technically and practically feasible.

Assessing the performance of SPM analyses of spect neuroactivation studies. Statistical Parametric Mapping.

Several simulations of SPECT neuroactivation studies have been performed in order to determine the influence of both study size and activation focus characteristics on the detection of brain activation foci following a pixel-based statistical analysis. This was achieved by developing a methodology based on the Hoffman software brain phantom, SPECT acquisition simulation software, standard reconstruction software, and the Statistical Parametric Mapping (SPM96) package. We present results on the minimal activation levels required for focus detection. Furthermore, the improved sensitivity of the analysis resulting from the use of an iterative reconstruction technique (OSEM) with regard to the classical filtered backprojection (FBP) is assessed quantitatively, and the various physical, processing, and physiological parameters that potentially influence the detection of foci are discussed. Finally, the influence is investigated of the height threshold as implemented in SPM96 upon the size of the detected foci. Practical guidelines are proposed with regard to the number of subjects per group for SPECT activation studies following the split-dose design.

Verbal fluency as a prefrontal activation probe: a validation study using 99mTc-ECD brain SPET.

This study aimed to investigate the feasibility of brain single-photon emission tomography (SPET) in the letter and category fluency paradigm of the Controlled Oral Word Association (COWA) test in healthy volunteers. Two groups each comprising ten right-handed healthy volunteers were injected twice with 370 MBq technetium-99m ethyl cysteinate dimer following a split-dose paradigm (resting and activation condition). Statistical parametric mapping (SPM96) was used to determine voxelwise significant changes. The letter fluency and the category fluency activation paradigm had a differential brain activation pattern. The posterior part of the left inferior prefrontal cortex (LIPC) was activated in both paradigms, with the category fluency task having an extra activation in the anterior LIPC. In the category fluency task, but not the letter fluency task, an activation in the right inferior prefrontal cortex was found. These findings confirm to a large extent the results of previous functional magnetic resonance imaging and positron emission tomography studies in semantic and phonological activation paradigms. The choice and validity of various methodological characteristics of the experimental design leading to these results are critically discussed. It is concluded that brain SPET activation with the letter fluency and category fluency paradigm under standard neuropsychological conditions in healthy volunteers is both technically and practically feasible.

Theoretical study of the relation between radon and its long-lived progeny in a room.

We present a theoretical study of the complex relation between radon and its long-lived progeny implanted in glass surfaces. The well known (extended) Jacobi room model, which is normally used to describe radon and its progeny in a room, was transformed into a two-parameter model revealing a linear correlation between long term radon exposure and surface activity due to implanted radon decay products. Furthermore, this new approach made integration into a Monte Carlo simulation possible so that the large variation of different room model parameters could be taken into account. This allowed the calculation of a probability distribution for radon exposure from the measurement of the implanted 210Po activity. The availability of a 95% confidence interval for the radon exposure is valuable in the application of retrospective radon assessment in epidemiological studies.

European Medical Device Directive: impact on nuclear medicine.

The European Council Directive 93/42/EEC concerning medical devices (14 June 1993) assigns new responsibilities and imposes technical requirements both to the manufacturer and user of medical devices. In this paper the general outlines of the directive are discussed with a particular emphasis on the risk classification of products, the compliance and evaluation process and the CE-marking regulations. Furthermore, some practical implications are highlighted for devices and tools relevant to the field of nuclear medicine such as radiation detectors, gamma- and PET-cameras and software.
