Assessment of appropriate laboratory measurements to supplement the Crohn's disease activity index.

The ability of 11 laboratory parameters to reflect the degree of activity of Crohn's disease, using a clinical index as reference point was compared by means of multiple stepwise regression analysis. Activity was best defined in decreasing order by orosomucoid, sedimentation rate, C reactive protein, alpha-1-antitrypsin, albumin, haematocrit, IgM, circulating immune complexes, serum iron, IgG, and IgA. The haematocrit, the only laboratory measurement in the Crohn's disease activity index developed by the National Cooperative Study Group in the USA, is less discriminant than acute phase reactants. Only three parameters-namely, orosomucoid, sedimentation rate, and C reactive protein-have a significant weight and should be complementary to a simple clinical index.

ARCANE. A new medical patient information system.

ARCANE is a new medical patient information system developed at Necker Enfants-malades University Hospital. It was initially dedicated to pediatric nephrology, using microcomputers directly accessible to the physicians. It is however designed for general use and could be directly adapted to many domains in Medicine. ARCANE's referential is based on conceptual slicing close to SNOMED's. The thesaurus is built on keywords or key-expressions. Combination with adjectives and modifiers reduces the volume of the thesaurus to about 1000 entries. Multipurpose use of the data is made available by automatic transcodification to ICD classification and SNOMED nomenclature. External formatting by semantic fields allows the physicians to attribute medical expressions dynamically to concepts of the thesaurus. This system, devoted to daily practice, is also conceived as a clinical research aid and an activity analysis tool.

[Evaluation of the diagnostic value of a test. Main information indices].

Evaluating a new diagnostic test is an essential step required before using a new examination technique. The information concerning the disease provided by the test must be examined in terms of diagnostic value and cost. A test can be evaluated using several indexes. For tests giving binary results, the main information indexes were defined and the calculation of the index was illustrated by an example. The sensitivity, specificity, the Youden index, and the positive and negative predictive values were presented together with the likelihood and odds-ratio. For tests giving quantitative results, the analysis of the ROC curves was discussed. Indexes were interpreted in terms of disease prevalence, the modalities (diagnosis, screening) for applying the test, the sampling technique for the tested population, the gain in diagnosis, and cost of the examination.
