Irritable bowel syndrome: a test of the colonic hyperalgesia hypothesis.

This study tested the hypothesis that, patients with irritable bowel syndrome (IBS), there is a primary hyperalgesia of the colon. Previous work, which examined these patients and normals, has not included subjects who provide a control for relevant psychological characteristics. We compared ratings of pain, following varying degrees of distension of the sigmoid colon, in normals, patients with IBS, and patients who were psychologically disturbed but without bowel symptoms. Psychological characteristics were assessed by a psychiatric interview and psychometric inventories; response to distension was tested by placing a tube in the rectosigmoid colon and successively inflating a nd deflating a balloon at its tip at 10 cm3 increments up to 50 cm3. Ratings of pain were recorded at each volume. The results indicated that the two patient groups were psychologically similar and both were more disturbed than normals. A linear relation was found between reports of pain and volume of distension in all three groups. There were no significant differences between the proportions of subjects experiencing pain in each group or the average of the ratings. There were no significant associations between the pain ratings and measures of anxiety, depression, neuroticism, and extraversion. The data do not support the hypothesis that colonic hyperalgesia is an important contributory factor in the etiology of the irritable bowel syndrome.

Light scattering vs. microscopy for measuring average cell size and shape.

Light-scattering photometry is compared with electron and light microscopy as a source of information about the average size and shape of cells in population. Examined are the effects of limited instrument resolution, necessary experimental procedures, and cell heterogeneity. Information theory is used to survey the relative amounts of information provided by photometric and microscopic measurements. Then in model exploratory experiments, cell size and shape and changes therein are determined both by microscopy and by photometry for spherical and spheroidal cells. Scattering theory is used to calculate photometrically observed light from cell parameters. It is found that if certain other appropriate information about the morphological property of interest is available, then visible light photometry is the preferred method for obtaining quantitative information. It has good absolute sensitivity (0.01-0.10 micrometers resolution), its results are relatively unaffected by sample heterogeneity, it is nondestructive and compatible with many other techniques, it requires no sample preparation, and it provides its information in real time.

Psychophysiologic disorders: a critical appraisal of concept and theory illustrated with reference to the irritable bowel syndrome (IBS).

The concept of psychophysiologic disorders and the major theories invented to account for such disorders are critically reviewed. The Irritable Bowel Syndrome (IBS) serves to illustrate the application of each theory and provides a vehicle for their appraisal. The tendency to think of 'physical' and 'psychological' as separate entities rather than separate languages has led to attempts to make a categorical distinction between disorders caused by 'psychological' factors and those caused by 'physical' factors. Some of the theories developed to account for psychophysiologic disorders are unscientific and none can adequately account for all the features of IBS. It is concluded that the concept of psychophysiologic or psychosomatic disorder is outmoded.

A case of homosexuality treated by in vivo desensitization and assertive training.

Sexual deviation is not a unitary condition, amenable to a single treatment approach. Deviant sexual arousal has been overemphasized while inadequate attention has been given to associated behavioural problems such as deficient heterosexual arousal, deficient heterosexual skills and gender role deviation. A client with homosexual arousal and requesting aversion therapy was treated successfully and rapidly using in vivo desensitization and assertive training. The treatment is described and the importance of the behavioural analysis in directing the course of therapy is stressed.
