[Drug induced fatal anaphylactic shock in Denmark 1968-1990].

Based on notifications to the Danish Committee on Adverse Drug Reactions and to the Central Death Register, 30 cases of fatal drug-induced anaphylactic shock were identified during the period 1968-1990. The most frequent causes were contrast media for X-ray examinations, antibiotics, and extracts of allergens. Nine cases occurred outside hospital; they were mainly caused by the use of allergen extracts of moulds for desensitization in general practice. Fatal drug-induced anaphylactic shock is rare, estimated in the present study at 0.3 cases per million inhabitants per year.

alpha-Amylase in resectable lung cancer.

Biochemical analysis and immunohistochemical techniques support the theory that hyperamylasaemia in lung cancer is due to amylase production in carcinoma cells. The vast majority of amylase-producing carcinomas are adenocarcinomas with amylase isoenzyme similar to the salivary type. This prospective study assesses alpha-amylase expression in resectable lung cancer. Seventy four patients with resectable lung cancer were studied. Amylase activity in tumour tissue was analysed and isoamylase identification performed. Immunohistochemical analysis was performed using a polyclonal rabbit antibody against human salivary amylase. Hyperamylasaemia occurred in 13 out of 70 patients. Increased amylase activity in tumour tissue was found in 10 out of 52 cases, of which only two were associated with hyperamylasaemia. With the exception of one large cell carcinoma and one squamous cell carcinoma, the tumours were adenocarcinomas. Immunohistochemical analysis revealed amylase expression in seven adenocarcinomas and two adenosquamous carcinomas. In conclusion, immunohistochemical amylase expression was restricted to carcinomas with adenomatous differentiation. Biochemical analysis confirmed amylase production in 5 of 7 cases examined, the tissue amylase isoenzymes being of salivary type. However, hyperamylasaemia and a slightly increased amylase activity in tumour tissue may be caused by factors other than amylase-producing carcinoma cells.

[Preoperative adrenergic beta receptor blockade and anesthesia. II. Effect on peroperative anxiety and cardiovascular response in epidural anesthesia].

The effects of a preoperative beta-adrenoceptor antagonist, metoprolol, on peroperative anxiety, relaxation, general feeling, blood pressures and heart rate, were studied in patients undergoing orthopaedic surgery during epidural anesthesia. The study was double-blind and placebo controlled. Twenty non-consecutive patients were randomized to receive either metoprolol 100 mg or placebo orally 1-2 hours before anaesthesia. Patients receiving metoprolol scored statistically significantly better on anxiety, and general feeling. The average height of the epidural block was ThVIII. The average blood pressure was reduced by 5% during the first 45 minutes in both groups. The average maximum falls in mean blood pressure from preanaesthetic to the lowest measured after the epidural were 14% (metoprolol) and 7% (placebo). The heart rates were significantly lower after metoprolol. In conclusion, the patients felt subjectively better after metoprolol and no significant differences were found in blood pressure after metoprolol compared to placebo.

[Perioperative hypothermia in adults].

In perioperative hypothermia, a central temperature of less than 36 degrees C develops in connection with anaesthesia and operation. Perioperative hypothermia constitutes a daily problem which results in increased morbidity and mortality in risk groups. The influence of anaesthetic agents on temperature regulation is reviewed. The postoperative hypothermic phase with increased oxygen requirement on rewarming increases the risk of development of complications, including cardiac complications. The risk groups and prophylactic methods for hypothermia are mentioned. Higher temperatures in the anaesthetic room, prewarming of infusion fluids and employment of infusion warmers should be employed with all anaesthetics. In patients in risk groups, extensive employment of combined methods of prevention of hypothermia is recommended.

[The heating effect of two infusion warmers].

An investigation was performed to compare the Animec infusion warmer with the Fenwall infusion warmer. The maximal increase in temperature for the Animec infusion warmer was 8.9 degrees C and this was recorded with a flow of 395 ml/hour. The values for the Fenwall infusion warmer were 12.0 degrees C with a flow of 1,527 ml/hour. An estimate is made of the reduced energy deficit with employment of the two infusion warmers. It is concluded that supply of energy to the infusion warmers. It is concluded that supply of energy to the infusion fluid depends on the flow of fluid through the infusion warmer and the capacity of the infusion warmer. Great differences were observed in the capacities of the two infusion warmers to compensate for the peroperative energy deficit at the rates of flow normally recommended.

A rapidly progressing lethal case of neuroleptic malignant syndrome.

A fast progressing lethal case of neuroleptic malignant syndrome (NMS) complicated by disseminated intravascular coagulation (DIC) is presented. The fulminant course is infrequent and relates NMS to malignant hyperthermia. Muscular relaxation in combination with fluid load dramatically decreased the temperature, but the catastrophic course of this case was so advanced that the currently recommended treatment of NMS was impossible.

Effect of metoprolol and diazepam on pre-operative anxiety.

A double-blind study of 98 patients who underwent elective hysterectomy or orthopaedic surgery was conducted to evaluate the effect of metoprolol or placebo combined with diazepam given 1-3 hours before surgery. Evaluation was by anaesthetists and by visual analogue scoring by the patients. The anaesthetists found that patients who received metoprolol were significantly less anxious (p less than 0.005) and better sedated (p less than 0.001) before induction of anaesthesia. The patients who received metoprolol found themselves more calm compared with placebo patients. Arterial blood pressure and heart rate were reduced by metoprolol compared to placebo. Metoprolol may be a valuable drug for premedication.

Familial spontaneous pneumothorax.

In a three generation family with 27 members we examined a relationship could be found between spontaneous pneumothorax and HLA-haplotypes, alpha 1-antitrypsin phenotypes or concentration or lung volumes and ventilatory capacity. Eight individuals in the family suffered from spontaneous pneumothorax. No relationship with the investigated markers could be found in this informative family. All patients showed normal lung volumes and ventilatory capacity after recovery.

[Vascular surgery in Denmark].

This report presents vascular surgical operations performed in Denmark 1983-1987, the first five years after the establishment of the speciality. The number of vascular surgical operations increased by 18 per cent during the five years to nearly 2,000 operations in 1987. Approximately 50 per cent of all vascular procedures were intraabdominal operations, while 30 per cent were peripheral reconstructions. In 1987 40 operations/100,000 inhabitants were made, a level considerably below the activity performed in the other Scandinavian countries. We conclude that vascular surgical activity will increase in Denmark in the years to come.

[Amylase-producing lung tumors].

Raised serum amylase associated with pulmonary tumours is a rare phenomenon. On the basis of two cases, the literature is reviewed and the case histories are compared. The majority of tumours associated with raised serum amylase are adenocarcinomata and, in all of the cases in which determinations of isoenzymes were undertaken, the amylase-isoenzyme was of salivary gland type. In one of the authors' cases, immunhistochemical investigation revealed that the tumour cells contained amylase. In cases of hyperamylasaemia of unknown origin with increased amylase of salivary gland type, adenocarcinoma in the lungs should be considered as a possible diagnosis.

Galactose tolerance test and methotrexate-induced liver fibrosis and cirrhosis in patients with psoriasis.

A total of 151 galactose tolerance tests (GTT) and liver biopsies were performed in a consecutive study of 45 psoriatic patients with methotrexate-induced liver fibrosis; of these, 23 had cirrhosis. Most patients with an abnormal liver histology had a normal GTT. We conclude that an oral GTT is not sensitive enough to reveal methotrexate-induced liver fibrosis or cirrhosis. The results indicate that the histological changes in methotrexate-induced liver fibrosis and cirrhosis may be of a rather non-aggressive nature.
