[Hypotensive action of 0.5% carteolol versus 0.1% timolol in patients with intraocular hypertension].

PURPOSE: The aim of the study was to compare 2 beta-blocker eye drops at a low concentration: 0.5% carteolol and 0.1% timolol. METHODS: The study was designed as a random-order, double-blinded comparison of 2 parallel treatment groups. Fifty patients with early primary open angle glaucoma or high intraocular pressure were included. The treatment lasted 4 weeks, on the basis of 1 drop twice daily. Diurnal I.O.P. curve was assessed with 4 measurements from 8.30 a.m. to 4.30 p.m. before and after treatment. The 8.30 a.m. measure of the final assessment of I.O.P. curve was established prior to morning medication. The mean values of the 4 measures were compared. RESULTS: Both treatments reduced IOP by a comparable amount: 4.25 +/- 1.2 mmHg (mean +/- SD) for carteolol and 4.69 +/- 1.9 mmHg for timolol. The decrease of IOP was found at every time of assessment, without any significant difference between treatments. Both eye drops were very well tolerated. CONCLUSION: The results of this study show that the new beta-blocker eye drop solution 0.5% carteolol is effective for initial management of high intraocular pressure.

[Efficacy and tolerability of 2 presentations of eyedrops combining carteolol 2% and pilocarpine 2% in primary open-angle glaucoma and simple ocular hypertension].

PURPOSE: The aim of the study was to compare 2 combinations of eye drops containing 2% carteolol and 2% pilocarpine: LCM 1010: ready to use eye drops CBS 341A: eye drops to be reconstituted (freeze-dried powder + solvent). METHODS: Ninety-seven patients with primary open angle glaucoma or simple ocular hypertension were included in a randomized, double-blind multicentric study comparing 2 parallel groups of treatment. Intra-ocular pressure was greater than 21 mmHg with beta-blocker alone. One instillation of 2% carteolol-2% pilocarpine combination was given twice a day for one month. Before and after this treatment, intra-ocular pressure was measured at 9 am (12 hours after evening instillation) and at 11 am (2 hours after morning instillation). RESULTS: Both treatments reduced intra-ocular pressure by a comparable amount and there was no significant difference between groups at either measure: at 9 am: 2.11 +/- 2.39 mmHg (mean +/- SD) for LCM 1010 1.79 +/- 1.73 mmHg for CBS 341 A p = 0.25 at 11 am: 3.75 +/- 3.83 mmHg for LCM 1010 3.40 +/- 1.69 mmHg for CBS 341 A p = 0.42. Both eye drops were generally well tolerated. CONCLUSION: Efficacy and safety of ready to use eye drops 2% carteolol-2% pilocarpine combination proved to be comparable to that of eye drops to be reconstituted in the treatment of ocular hypertension poorly controlled by beta-blocker eye drops alone.

Two topical carbonic anhydrase inhibitors sezolamide and dorzolamide in Gelrite vehicle: a multiple-dose efficacy study.

The ocular hypotensive activities of the two potent topical carbonic anhydrase inhibitors sezolamide (previously known as MK-417) and dorzolamide (previously known as MK-507 and L-671,152) were compared formulated in Gelrite vehicle, a novel ophthalmic drug delivery system. This was a four-center, double-blind, randomized, placebo-controlled, parallel study in 73 patients with a diagnosis of bilateral primary open-angle glaucoma or ocular hypertension and a morning intraocular pressure (IOP) of greater than 23 mmHg in both eyes following washout of ocular hypotensive medications. Parallel 12-h modified diurnal curves were performed prestudy and on day 6, with a 4-h IOP curve on day 1. On day 6 the peak mean percentage decrease in IOP from baseline occurred 4 h after the dose of dorzolamide (22.1%) and 6 h after the dose of sezolamide (21.3%). There were no significant differences between 2% dorzolamide and 1.8% sezolamide at any time point, although the decrease in IOP for sezolamide tended to be slightly greater than that for dorzolamide. Duration of action of both compounds was, at most, slightly prolonged by the use of Gelrite vehicle when compared with former studies on sezolamide and dorzolamide.

[Risk of infection after cataract surgery with intraocular implant].

A personal series of 10 patients with post surgical bacterial endophthalmitis is reviewed. Clinical signs, risk factors and diagnostic modalities are analysed and compared to other reports in literature. The role of intraocular lens is discussed. While it does not seem to be a risk factor in acute endophthalmitis, it plays a major role in chronic bacterial endophthalmitis. Endophthalmitis remains a rare but serious complication after cataract surgery and prevention modalities, especially using prophylactic antibiotherapy, have still to be evaluated.

Multiple-dose efficacy comparison of the two topical carbonic anhydrase inhibitors sezolamide and MK-927.

The multiple-dose twice-daily efficacy of the topical carbonic anhydrase inhibitor MK-927, a racemic compound, was compared with that of its pharmacologically more active S-enantiomer in a four-center, double-masked, randomized, placebo-controlled, parallel study of 1.8% sezolamide hydrochloride (MK-417), 2% MK-927, and placebo, given twice daily to 48 patients with bilateral primary open angle glaucoma or ocular hypertension and morning intraocular pressure greater than 24 mm Hg in both eyes following washout of ocular hypotensive medications. Parallel 10-hour modified diurnal curves were performed before the study and on day 14, with 4-hour curves on days 1 and 4. Both compounds demonstrated significant lowering of intraocular pressure at 8 AM, 12 hours following the evening dose, and through 10 and 6 hours following the 8 AM dose for sezolamide and MK-927, respectively. Morning trough (evening) activity as measured by mean percent change in intraocular pressure from prestudy was -9.2% for sezolamide and -11.1% for MK-927 (-13.5% and -9.6%); peak effect occurred 2 hours after dose administration and was -19.4% and -19.2% for sezolamide and MK-927, respectively. From 2 hours after dose administration, sezolamide consistently demonstrated a slightly greater decrease in intraocular pressure than MK-927; however, these differences were not statistically significant.

[Submacular neovascular membrane and drusen of the optic disk].

A case of unilateral serous hemorrhagic maculopathy with a very acute course is reported in an eight year old girl fluorescein angiography revealed the presence of a subretinal neovascular membrane extending from the inferotemporal part of the disc to the macula; CT scan identified calcified drusen of the optic disc. Ophthalmoscopy and angiography one year later showed a glial macular scar, absence of hemorrhage and serous detachment. Visual acuity was reduced to counting fingers. This unusual type of maculopathy combining optic disc drusen and a neovascular membrane contradicts the classically benign prognosis of optic disc drusen.

[Our experience with myopic implants. Initial optical results].

28 patients with high myopia (over-10 dioptries) were implanted in anterior chamber with a myopic IOL. 3 months postoperatively the visual acuity without correction was equal to the preoperative acuity. With a light correction, especially of residual astigmatism, we noticed a gain of acuity of 0.2.

[Corneal abscess caused by Pasteurella following cat scratch injury].

Authors report a Pasteurella Multocida corneal abscess after a scratch from a cat. The evolution was torpid and needed a transfixiant keratoplasty. They recall bacterial tanks, infestation ways, clinical manifestations and bacteriological identification. They note the low occurrence of ophthalmological manifestations and the sensibility of Pasteurella to various antibiotics, especially Beta lactams, phenicol and cyclins.

[Penetrating eye injuries caused by firearms. Apropos of 17 cases].

The authors present 17 cases of perforating ocular injuries caused by fire arms. They successively describe circumstances of trauma, initial injuries, surgical techniques employed and their results. The mean follow-up of patients is about one year. They particularly insist on severity of this type of trauma and legislative problems caused by free sale of some of these arms, more specially so called self defence pellets guns.

[Treatment of lacrimal canal stenoses by canaliculoplasty].

An attempt to make a canaliculoplasty for 53 cases with canalicular stenosis gave a functional favorable result for 40 cases; Microsurgery is essential. By pass procedures were performed only in case of failure in several cases. Our rules for canaliculoplasty were: a large DCR osteotomy systematically associated, direct exposure of the stenosis (modified DCR cutaneous incision), interrupted sutures with extramucosal not, restitution of a mucosal continuity between the punctum and the nose without stretch, long term (one year) or permanent bicanalicular silicone tubing, the different procedures are discussed: reimplantation of the common canaliculus in the lacrimal sac (9 cases); resection + direct suture for minimal stenosis (13 cases), Turning inside out of the lacrimal sac + large nasal mucosal flap (22 cases)... We emphasize the third procedure which provides in our hands much satisfactory results (18 successes and 4 failures) for 4 or 6 millimeters (sometimes 8 or more) canalicular stenosis when the natural punctum is still present: the lacrimal mucosa of the sac is turned inside out and provides the posterior flap of the new pathway. The anterior flap is provided by a very large nasal mucosal flap. Both flaps are sutured to the safe canalicular extremity.
