An evaluation of a behavioural treatment approach to hearing impairment.

Twenty-four elderly hearing impaired Ss participated in an experimental treatment study and received either behavioural group treatment or served as untreated controls. The treatment package included applied relaxation, video self-modelling, exposure, information and various coping skills. Assessments (pre-post) were conducted using a structured video-interview measuring coping behaviour. In order to evoke behavioural compensation small acoustic provocations were included in the interview. The edited videos were then rated blindly by two trained observers. Pre-post assessments also included daily registered hearing problems on visual analogue scales and a questionnaire. Finally, a one month follow-up blind telephone interview was conducted. Results showed significant beneficial effects in favour of the treatment package and support the implementation of a behavioural approach in audiological rehabilitation research.

Lung function after open versus laparoscopic cholecystectomy.

Postoperative lung function and gas exchange were studied in 36 patients after cholecystectomy. Twenty-four of the patients underwent laparoscopic cholecystectomy while the remaining twelve were operated with open technique. Before surgery all patients had normal ventilatory volumes (forced vital capacity, FVC and forced expired volume in 1 s, FEV1) and normal gas exchange. Two hours postoperatively FVC was reduced to 64 +/- 16% (P < 0.05) of the preoperative level in the laparoscopic group and to 45 +/- 23% (P < 0.05) after open cholecystectomy. On the first postoperative day FVC was virtually normal in the laparoscopic patients (77 +/- 17% of preoperative level, NS), whereas the open surgery patients still had a decreased FVC (56 +/- 13% of preoperative, P < 0.05). FEV1 in the postoperative period followed the same course as FVC. Gas exchange was significantly impaired in the early postoperative period in all patients but no difference between the two groups was found. Two hours postoperatively PaO2 was reduced to 85% (P < 0.05) of preoperative value and PaCO2 had increased by 0.5 kPa (P < 0.05). The alveolo-arterial oxygen tension difference (PA-aO2) had increased by approximately 45% to a mean of 3.7 kPa (P < 0.05). On the first postoperative day gas exchange was still significantly impaired in the open surgery patients. Atelectasis detected by computed X-ray tomography of the lungs were found in both groups. However, the amount of atelectasis tended to be smaller in the laparoscopic group than in the open surgery patients.(ABSTRACT TRUNCATED AT 250 WORDS)

Development of a short scale for self-assessment of experiences of hearing loss. The hearing coping assessment.

A short scale for self-assessment of experiences of hearing impairment--the Hearing Coping Assessment (HCA) was developed and administered to 114 consecutive hearing-impaired patients at a Swedish hearing centre. The scale was evaluated in terms of descriptive statistics, reliability, principal components analysis, and validity. The results showed high internal consistency, high split-half correlation, and high item-total correlations. Significant correlations were found between the HCA questionnaire and measures of optimism, depressive syndrome, and audiogram (PTA). The principal component analysis showed two meaningful factors. The first mainly represents disability, and the second emotional reactions due to hearing loss. Aspects of handicap were present in both factors. The subjects in this study were no less optimistic nor did they show more signs of depressive syndrome than comparable norm groups. Still, optimists reported fewer hearing problems as measured by the HCA. The HCA is proposed as a suitable assessment scale in studies on the effects of counselling.

Behavioral counselling for subjects with acquired hearing loss. A new approach to hearing tactics.

Effects of behavioral counselling of subjects with an acquired hearing loss were evaluated in an experimental design. The study included a group of 20 hearing-impaired subjects who were assessed and randomly allocated to a treatment or control group. Behavioural counselling, individualized according to the results of functional analysis, was then given to 10 subjects and included the teaching of 'hearing tactics'. The groups were measured three times, pre-counselling and post-counselling and at a 15-month follow-up, after which treatment results were analysed. Significant (time x treatment) interactions were followed by post-hoc testing revealing that the treated group had improved in several areas of functioning while controls had not. These results were in agreement with interview data. The findings indicate that a behavioural counselling approach to hearing tactics is beneficial in the understanding and rehabilitation of hearing loss, especially over time.

A behavioural approach to individually designed hearing tactics training.

In an experimental study involving 20 subjects functional consequences of hearing disability were assessed and counselled in accordance with behavioural principles. The aims were to develop psychological methods for analysing and relieving hearing problems. The results indicated that treated subjects improved significantly in three functional areas as compared to controls. Subjects also developed more knowledge and ability to handle hearing problems than controls. Although these findings suggest that behavioural methods can be beneficial in the understanding and rehabilitation of handicaps related to hearing disability, they must be interpreted with caution given the small sample size.

Atelectasis and lung function in the postoperative period.

Thirteen patients with healthy hearts and lungs, and with a mean age of 68 years, who were scheduled for lower abdominal surgery during isoflurane anaesthesia with muscular paralysis, were investigated with arterial blood gases, spirometry, pulmonary x-ray and computed tomography (CT) of the chest before and during anaesthesia, as well as during the first 4 postoperative days. Before anaesthesia, lung function and gas exchange were normal in all patients. Pulmonary x-ray and CT scans of the lungs were also normal. During anaesthesia, 6 of 13 patients developed atelectasis (mean 1.0% of intrathoracic transverse area in all patients). Two hours postoperatively, 11 of 13 patients had atelectasis and the mean atelectatic area was 1.8%. Pao2 was significantly reduced by 2.1 kPa to 9.8 kPa. On the first postoperative day, the mean atelectasis was unaltered (1.8%). None of the atelectasis found on CT scanning could be detected on standard pulmonary x-ray. Forced vital capacity (FVC) and forced expired volume in 1 s (FEV1) were significantly decreased to 2/3 of preoperative level. Pao2 was significantly reduced to less than 80% of the preoperative level (mean 9.4 kPa). There were significant correlations between the atelectatic area and the impairment in FVC, FEV1, and Pao2. Spirometry and blood gases improved during the succeeding postoperative days, and atelectasis decreased. No patient suffered from pulmonary complications, as judged from clinical criteria and pulmonary x-ray, in contrast to the findings of atelectasis in 85% of the patients by computed tomography.

Resolution of the enantiomers of omeprazole and some of its analogues by liquid chromatography on a trisphenylcarbamoylcellulose-based stationary phase. The effect of the enantiomers of omeprazole on gastric glands.

The enantiomers of omeprazole and some of its analogues have been separated on a chiral stationary phase comprising trisphenylcarbamoylcellulose coated on 3-aminopropyl silica. The nature of the supporting silica has a crucial effect on the separations obtained. The racemisation half-life of omeprazole was estimated to be 1.3.10(2) h at 37 degrees C. In vitro tests on isolated gastric glands from rabbits showed that both enantiomers of omeprazole had an inhibitory effect on acid formation.

Predictors of tinnitus discomfort, adaptation and subjective loudness.

In a nation-wide investigation, covering all the hearing centres in Sweden, a study was made of adaptation processes, subjective discomfort from tinnitus, subjective loudness of tinnitus and psychological complaints in 3372 subjects by means of a questionnaire. The most important predictors of discomfort from and adaptation to tinnitus were found to be the controllability and the degree of maskability by external sounds, i.e. the subject's coping abilities or internal-external locus of control. Increased control and masking effects from the environment imply a decrease in discomfort and better adaptation. The most important predictor of worsened subjective loudness of tinnitus was the duration of the tinnitus. That is, subjects who had had tinnitus for a longer time perceived the loudness as more intense. The psychosomatic factors which most strongly predicted increased discomfort from and decreased tolerance to tinnitus were depression and insomnia. These findings have theoretical and practical implications for the management and treatment of tinnitus.

The psychological treatment of tinnitus: an experimental evaluation.

Disabling tinnitus in 27 patients was treated in an experimental setting with two different type of therapy aiming at the development of coping behaviour. One was focused on behavioural control procedures, while the other aimed at more cognitive control methods such as distraction. The results of the two forms of therapy were compared with each other and with observations of the untreated control subjects. Assessments of subjective loudness, discomfort from tinnitus and controllability were made on analogue scales and at a questionnaire follow-up. Psychoacoustic measures such as personal loudness units (PLU) were used in connection with these self-recordings and for evaluation of treatment effects. The results confirmed favourable reports on behavioural methods in tinnitus. However, no differences were found between therapies. The results are discussed in the light of the coping and adaptation theory.

N,N-Dialkylated monophenolic trans-2-phenylcyclopropylamines: novel central 5-hydroxytryptamine receptor agonists.

N,N-Dialkylated monophenolic derivatives of trans-2-phenylcyclopropylamine were synthesized and tested for central 5-hydroxytryptamine (5-HT) and dopamine (DA) receptor stimulating activity by use of a biochemical test method in rats. A hydroxy substituent in the 2- or 3-position of the phenyl ring was required for 5-HT-receptor stimulation. N,N-Diethyl or N,N-di-n-propyl substitution gave the most potent 5-HT-receptor agonists. The 4-hydroxy and 3,4-dihydroxy derivatives of trans-2-phenyl-N,N-di-n-propylcyclopropylamine were inactive at central DA and 5-HT receptors. In contrast, the corresponding 3-hydroxy derivative 18 and some of its derivatives weakly affected both DA and NE synthesis. Two of the most potent 5-HT-receptor agonists, trans-2-(2-hydroxyphenyl)-N,N-di-n-propylcyclopropylamine (8) and the 3-hydroxy isomer 18 were resolved into the enantiomers. The 1R,2S enantiomers of 8 and 18 displayed 5-HT activity, while the 1S,2R enantiomers were inactive. Compound (1R,2S)-18, but not (1R,2S)-8, weakly affected rat brain DA and NE synthesis.

Behavioural therapy in the clinical management of tinnitus.

Behavioural therapy was given to reduce discomfort and annoyance associated with severe tinnitus in 75 consecutive patients. Treatment was conducted within the routine clinical management. It included information and discussion about tinnitus, behavioural analysis and relaxation training combined with various cognitive strategies. The method of treatment was thus a form of coping technique. Self-recordings showed significant reductions of 'discomfort from tinnitus' as well as significant elevations in 'general mood' over a 3-month follow-up period. Interview findings indicated strong overall positive effects of treatment, as well as other positive side-effects on complaints such as tension headaches, dizziness, muscle tension and sleep disturbances. Implications for the clinical management of tinnitus sufferers are discussed.

Carbamate ester derivatives as potential prodrugs of the presynaptic dopamine autoreceptor agonist (-)-3-(3-hydroxyphenyl)-N-propylpiperidine.

Twenty derivatives bearing substituents on the phenolic function of (-)-3-(3-hydroxyphenyl)-N-propylpiperidine [(-)-3-PPP] were synthesized and tested as prodrugs. The carbamate ester derivatives were found to be the most suitable prodrugs, and especially the 4-isopropylphenylcarbamate 20 was capable of escaping the first-pass metabolism and still generating high plasma levels of the parent compound. Four hours after an oral dose of 100 mumol/kg to rats, a plasma level of 2400 nmol/L of (-)-3-PPP was detected by an HPLC method. This was 90 times the level reached after 4 h (27 nmol/L) when (-)-3-PPP itself was given orally at the same dose.

Long-term effects of psychological treatment of tinnitus.

Twenty patients with severe tinnitus who had undergone behavioural treatment, including applied relaxation and perceptual restructuring, were re-assessed 9 months after completion of treatment. Among the self-recorded variables, tinnitus loudness, discomfort from tinnitus, depression, and irritation, discomfort from tinnitus was the only variable which was still significantly reduced. As part of the 9-month follow-up assessment, the patients' recall of the loudness and discomfort from their tinnitus was studied. Correlations between original recordings and recall data were low.

Behavior Therapy

Hearing aids and tinnitus--an experimental group study.

From a waiting list consisting of hearing-impaired patients waiting for hearing aids to be fitted, 39 subjects who at interview also stated that they had tinnitus took part in an experimental group study with the aim of investigating the effect of a hearing aid on tinnitus. No subject had any previous experience of hearing aids. The subjects were randomly allocated to a treatment and a waiting list control group. After an initial interview, the routine programme for the fitting of hearing aids started in the treatment group, while the waiting list control group had to wait for 6 weeks before starting the same hearing aid rehabilitation programme. The hearing aids were fitted exclusively for hearing purposes. As expected, the hearing aids improved the hearing capacity, but they did not reduce tinnitus as recorded on a visual analogue scale. According to information obtained at the final interview, there were significant differences in tinnitus between subjects who used their aid for more than 2 hours daily and those who used it for less than 2 hours. However, the results of scaling (pre- and post-fitting) did not support this finding. The discrepancy between the scaling and interview data is probably due to demand characteristics.

Effect of Se status on some haematological parameters in neonatal piglets.

Blood samples were taken from 74 1-3 day old piglets, the offspring of sows maintained during pregnancy on a commercial ration with 0.1 ppm of added Se as sodium selenite. At the time of blood sampling, all piglets were treated subcutaneously with 200 mg Fe+3 from iron dextran. Additional blood samples were taken 14 days after the iron treatment. Piglets with 14 day whole blood Se concentrations greater than 74.0 micrograms/l (n = 43) had significantly higher mean numbers of red blood cells than those with whole blood Se less than or equal to 74.0 micrograms/l (p = 0.016). The mean increase in erythrocyte numbers during the 14 day period was significantly higher in the higher than in the lower Se group (p = 0.007) as also in the piglets with plasma GSH-Px activity greater than 799 U/l at 1-3 days of age when compared to the piglets with the GSH-Px activity less than or equal to 799 U/l (p = 0.017).
