[Prenatal diagnosis of pulmonary hypoplasia camptodactyly syndrome. Hypothesis concerning the origin of hydramnios].

Camptodactyly and pulmonary hypoplasia syndrome was described for the first time in 1974. Today 15 cases have been found. None were subject to a prenatal diagnosis as any ultrasonography. The case we observe is a 27 years old woman, third gestation, showing a polyhydramnios at 25 weeks of gestation. The ultrasonographic examination showed deformation with a rounded abdomen, a narrow thoracic frame slightly concave, no gastric picture and hands in constant flexion. After elimination of the V.A.T.E.R.' syndrome the camptodactyly's syndrome with pulmonary hypoplasia was proposed. Because of the lethal character of this syndrome, the abortion was realised. The photos of the fetus were the same as the ultrasonographic pictures during the prenatal diagnosis. The absence of intrinsic (paralysis) or extrinsic (compression) movements of the fetus is very important in the genesis of "deformations". The severity of the deformities depends on the importance of akinesia.

Bilateral phaeochromocytomas in pregnancy: anaesthetic management of combined caesarean section and tumour removal.

A 25-year-old primigravida with bilateral phaeochromocytomas was managed from 6 weeks' gestation until 2.5 years following combined Caesarean section and removal of adrenal tumours. Pre-operative alpha-blockade was achieved by oral prazosin without deleterious effects. Intra-operative management comprised neuroleptanalgesia, arterial and pulmonary pressure monitoring, adequate heart loading with fluid replacement and sodium nitroprusside. Post-operatively, mother and baby remained normotensive.

Influence of IgG subclasses on automated anti-D (Rho) quantification.

Anti-D quantification by both an automated Polybrene method and an automated trypsin-albumin-dextran (TAD) method gave discrepant results in certain cases. These discrepancies, expressed as the polybrene TAD ratio of reactivity (PTR), were related to the IgG subclass of anti-D. Anti-D of the IgG3 subclass showed a higher PTR than IgG1 (0.94 vs 1.65). No difference was shown between G1m(1) and G1m(3) (0.93 and 0.95, respectively) or between G3m(11) and G3m(21) (1.40 and 1.81, respectively) allotypes. The simultaneous use of our automated Polybrene and TAD methods provides information about the anti-D subclass composition.

Electromyographical study of uterine activity in the human during labour induced by prostaglandin F2 alpha.

In full-term pregnant women, electrical and mechanical activity of the uterus was monitored throughout the course of labour promoted by intravenous infusion of Pg F2 alpha. The recorded potentials were mostly biphasic and characterized by their long duration ranging from 1 to 2s. A wide range of potential amplitudes (100 microV to 1.8 mV) was observed according to the various patients. Early at the beginning of labour induction, the electrical complexes firing at various uterine sites were proved to be in close relationship and also well correlated with the mechanical events. This feature remained unchanged during labour. Potential amplitudes also remained unchanged during the same period of time. Under these conditions, improvement of uterine coordination does not appear to be the mechanism by which the increase of uterine contractile strength, necessary to expel the fetus, is obtained at the end of gestation.

[Contraception using an intrauterine device inserted 1 month after delivery].

The authors report their experience of mechanical contraception using an intra-uterine device placed one month after delivery. The fact that perforation did not occur and there were no serious infectious complications, as well as that the method was well tolerated, that there was only 4.7% per year expulsions, and few failures (Pearl 2.96% per year) shows that this is a worth-while contraceptive method to recommend to women who have just delivered.

Upper limb exercise effect on tilt tolerance during orthostatic training of patients with spinal cord injury.

Twelve male patients with traumatic spinal cord injury were randomly divided into a group of six experimental and six control subjects. All subjects were tilted from 0 degree to 70 degrees by 10 degrees increments at five-minute intervals until blood pressure dropped below 70/40, hypotensive symptoms appeared, or 70 degrees of tilt was achieved. The experimental subjects performed 60 active bilateral full-range forearm flexion and extension movements per minute during the first and third minute of each tilt angle. The control patients did not perform upper limb exercises during the same orthostatic training procedure. Blood pressure and pulse rate of all subjects were recorded prior to orthostatic training, during the training at specified intervals and posttraining on each day of testing and training. Total group response was quantitated by indicating the tilting protocol step at which subjects experienced orthostatic hypotension requiring termination of the test. All subjects significantly increased their tolerance of higher tilt angles by using this protocol, but there was no significant difference between the exercise group and nonexercise group with reference to tolerance to progressive vertical tilt (Mann-Whitney U test).

[The onset of human labor].

The authors reviewed the literature in order to try to work out the different mechanisms that are responsible for labour starting. A role seems to be played by the decidua of the uterus, by the placenta and by the fetal membranes as well as the very important role of prostaglandin synthesis. It does seem as though there is no single factor that is important for starting labour. If we knew more about the physiology of human labour it would perhaps be easier to control premature labour.

[Hydramnios as evidence of Steinert's congenital myotonic distrophy (author's transl)].

The authors describe a case of Steinert's disease. It was tracked down because of three anomalies of pregnancy: a lessening in active movements of the fetus, breech presentation and hydramnios, and it was diagnosed by neurological and electromyography of the mother. The existence of hydramnios should make it possible to avoid maternal trauma during the delivery, knowing that the child will be born with a very severe form of the illness, which is likely to be lethal. Those few cases that survive are severely mentally handicapped.

[The use of prostaglandins to perform terminations of pregnancy in the second trimester (author's transl)].

The authors report 48 cases of termination of pregnancy between the 12th and the 20th week using prostaglandins. Two methods were used: An injection of 40 mg of Prostaglandin F2 alpha (1) intra-amniotically: used on 38 women in group i. 10 patients had 5 mg of 15 (S) 15 methylprostaglandin F2 alpha administered vaginally in group ii. In the first group all women aborted with a mean induction expulsion time of 16 hours and 20 minutes. In the second group 9 women aborted with a mean time of 14 hours and 2 minutes. Side effects were very marked in the second group, whereas they seemed to be acceptable in the first group.

[Effect of intravenous administration of spasmaverine on uterine activity during labor].

The authors have studied the action of an intravenous injection of Spasmaverine (Bis gamma phenyl-3-propylethylamine) on the intra-uterine pressure in labour in 20 cases. This study has shown that there is a definite rise in amplitude, in the frequency of contractions, in the uterine activity as expressed in montevideo units and in the basal tone of the uterus in the ten minutes following an injection of 80 mg of Spasmavérine intravenously. The fetal cardiac rhythm was never altered. The authors recommend care in using this substance intravenously when sedation of uterine activity is sought.

[The induction of labor by prostaglandin F2alpha Clinical, tocographic and electrohysterographic study].

Induction of labor was performed in 65 patients by intravenous infusion of PgF2alpha. 55 patients were near term and induction made electively. 5 cases failed. No correlation was found between the stade of the cervix and the probability of success nor the length of the induction time. PgF2alpha induced labor needs to be closely monitored (electronical pumps and monitoring) because of the risk of hypertonus. With such a technique the side effects are minimal and no undesirable consequences were observed concerning either the mother or the child. During the course of this investigation a study was performed for the first time of the electrical activity of the human myometrium subjected to PgF2alpha stimulation. (26 recordings through the extra-amniotic intrauterine technique). The pattern of this activity is quite similar to spontaneously occuring alectrical activity.
