Representations of the anatomic surface location of cutaneous lesions and the surgical procedures performed on these lesions can be transferred to the medical charts using simple anatomic rubber stamps (ARS) of the body. The technique is exemplified with symbols to represent surgical excision, chemical face peel, dermabrasion, micrographic surgery, and the harvesting of flaps and grafts. ARS are also useful in liposuction surgery, punch grafts for pitted facial scars, hair transplantation, sclerotherapy, laser surgery, and other cosmetic dermatologic procedures. ARS are particularly valuable in micrographic surgery for skin cancer because they help document the depth and breadth of cancer invasion and aid in the follow-up of recurrent skin cancer, especially when defects are reconstructed.
The history and possible adverse sequelae of polidocanol (Aethoxysklerol) and the sclerotherapy technique used on 1500 patients in 5000 procedures is reviewed.
Questionnaires were mailed to six physicians experienced in chemical peels regarding common problems encountered in the procedure. They are: Thomas H. Alt, M.D., Minneapolis, MN; Samuel J. Stegman, M.D., San Francisco, CA; James J. Stagnone, M.D., Albuquerque, NM; Robert Kotler, M.D., Los Angeles, CA; William H. Beeson, M.D., Indianapolis, IN; and Paul S. Collins, M.D., San Luis Obispo, CA. Their answers are presented in a panel format.
Sclerotherapy remains the best single treatment for telangiectasias and superficial venules of the lower extremities. It has been used successfully for decades. It is, however, a somewhat tedious and time-consuming procedure and requires great patience on the part of the physician. With careful attention to the details of the procedure and with necessary practice and experience on the part of the physician, a good result will be obtained. To summarize: Use one of the safe, modern sclerosing solutions: hypertonic sodium chloride 18 to 25 per cent, Aethoxysklerol 0.25 to 1.0 per cent, or Sclerodex. Good illumination, liberal use of alcohol on the skin, and 2 to 3 X magnification will increase the visibility of the tiny vessels. Use 3-ml Luer-Lok syringes and 30-gauge needles and thread the needle carefully into the vessel while using slow and steady injection with light pressure. Always provide post-treatment compression with cotton balls, tape, and compression hosiery. Because the majority of patients are most gratified, serious risks are minimal, and most side effects are of a transitory nature, sclerotherapy is a most worthwhile technique to master and include in one's armamentarium of simple, office-based cosmetic dermatologic procedures.
UNLABELLED: Glucocorticoid cytosol and whole cell receptors from human PMN's have been quantified, and compared to those of human MN leukocytes on the same blood sample. The normal cytosol PMN receptor density (N = 15) averaged 1,254 +/- 105 (SE) molecules bound/cell at 0900 h and increased significantly to 1,497 +/- 98 at 2,100 h (P less than 0.02). MN cell cytosol receptor density was 1,198 +/- 145 at 0900 h and increased significantly to 1,551 +/- 117 molecules bound/cell at 2,100 h (P less than 0.01). Corresponding whole cell receptor densities at 0900 h were 2,845 +/- 273 (PMN) and 3,547 +/- 290 (MN) and these did not change significantly at 2,100 h. CONCLUSIONS: Cytosol receptors in normal human PMN and MN cells increased significantly at 2,100 h from the 0900 h level while serum cortisol levels were dropping. Whole cell receptors in the same PMN and MN cell samples did not change significantly between 0900 and 2,100 h. The normal circadian variation in serum cortisol influences the distribution of the glucocorticoid receptor between the cytosol and the nucleus, but does not influence the amount of receptor available to the whole cell. This is the first time that an endogenous physiological variation of cortisol concentration has been utilized to demonstrate a corresponding change in receptor capacity in vivo.
Several different methods exist for surgical hair replacement of the vertex region of the scalp. Despite the more recent addition of alopecia reduction and flaps, small areas of crown baldness frequently, can best be handled cosmetically by well-planned and technically well-executed punch autografts. The methodology of hair transplantation of the vertex region of the scalp and the avoidance of cosmetic problems are discussed.
An inguinal lymph node was excised from a 23-year-old woman. Histologic examination revealed necrotic foci with cytologic changes indicative of herpes simplex virus; immunohistochemistry and electron microscopy confirmed the presence of virus. Typical cutaneous lesions appeared on the vulva three days after surgery. Focally necrotizing lymphadenitis and cytologic changes of viral transformation are unusual in herpes group infections.
