Epidemiologic Methods

Insulin-like growth factor (IGF) binding protein complementary deoxyribonucleic acid from human HEP G2 hepatoma cells: predicted protein sequence suggests an IGF binding domain different from those of the IGF-I and IGF-II receptors.

The primary structure of an insulin-like growth factor (IGF) binding protein produced by human HEP G2 hepatoma cells has been deduced from the cDNA sequence. The 234 amino acid protein has a predicted molecular mass of 25,274 and contains a single, distinctive cysteine-rich region. The N-terminal sequence of this protein is quite similar to the limited sequence data available for a rat IGF binding protein produced by BRL-3A cells and suggests a common ancestral origin. In contrast, the HEP G2 IGF binding protein sequence bears no similarity to the N-terminal 15 amino acids of a 53 kilodalton binding protein purified from human plasma. Comparison of full-length protein sequences for the IGF-I and IGF-II receptors with that of the HEP G2 IGF binding protein also fails to demonstrate any significant similarities among these three proteins, and suggests that each contains a unique binding domain for the IGF peptides.

Amino Acid Sequence

Expanded microporous polytetrafluoroethylene as canine arterial bypass or replacement graft.

Expanded microporous polytetrafluoroethylene (PTFE) has been used experimentally and clinically for small-vessel bypass or replacement grafting; the results vary widely. This study assessed short (4--cm) femoral and carotid artery replacement PTFE grafts and longer (12--cm) femoral artery bypass PTFE grafts that crossed a flexion crease. Pore size at the blood-graft interface ranged from 10 to 30 mu. At the end of three months, overall patency rate was 22%. No long bypass grafts remained patent. The pore size and type of anastomosis did not affect patency. The occlusion was always thrombotic, associated with fibrin at the suture line. Nine of the 20 short carotid grafts (45%) and five of 22 short femoral grafts (22%) remained patent. These poor results indicate that further experimental studies are needed before PTFE is used clinically when an alternative exists.

Isolation, cloning and characterization of argF gene DNA from Escherichia coli K-12.

A 1650 base pair (BP) fragment carrying the entire argF structural gene with its associated control regions was isolated from an EcoRI/BamHI digest of phi80argFilambda cI857 DNA. This segment was cloned using the EcoRI and BamHI cleavage sites in the plasmid pBR322. A preliminary restriction map of the argF region was prepared. RNA polymerase binding studies indicated that the argF promoter is located approx. 30 base pairs from the EcoRI terminus of the cloned DNA segment.

The relationship between transfusion and hypoxemia in combat casualties.

The relationship between transfusion and subsequent hypoxemia was examined retrospectively in the records of combat casualties studied by the first three U.S. Army Surgical Research Teams in Vietnam. There was no evident relationship in 425 casualties studied before anesthesia and operation. In 199 casualties studied preoperatively and on at least two of the first three postoperative days, there was no evident relationship in those with injuries not involving the chest or abdomen. Eighteen such casualties received over ten units of blood each (average 24.5) and exhibited subsequent changes in arterial oxygen tension (PaO2) which were indistinguishable from those transfused lesser amounts or not all. Similar observations were made in casualties with injuries to the abdomen, although there was a tendency to lower PaO2 two days after injury in those heavily transfused. In those with thoracic injury, there was statistically significantly lower PaO2 on the first two postoperative days in those heavily transfused. Two possible interpretations are considered, one that blood transfusion contributed to hypoxemia, and alternatively, that a greater magnitude of the injuries accounted for both the worsened hypoxemia and the need for more transfusions. The latter was thought more likely. The differences in PaO2 related to the type of injury exceeded the differences associated with transfusion.

A 3-year clinical trial of the effect on dental caries of a dentifrice containing 2% sodium monoflurophosphate.

A clinical trial of a dentifrice containing 2% sodium monofluorophosphate was carried out on 782 schoolchildren in Shropshire, England. After 3 years, a reduction of 23.8% was found in the DMFS increment obtained from clinical examination. The reduction was 38.8% using radiographs. The children were divided into three groups according to baseline DMFS. Those with a "medium" caries experience were found to benefit most from using the dentifrice. There was a difference in the effect on individual tooth sites; the approximal surfaces of the posterior teeth received the most benefit and the occlusal surfaces the least.

Canine kidney preservation: comparison of "intracellular", "extracellular," and high molecular weight dextran flushing solutions.

The effects of modified Collins-2 solution, adjusted lactated Ringer's solution, and a solution containing dextran 70 on kidney preservation and red blood cell (RBC) washout in dogs were evaluated. Excised kidneys were stored on ice for 24 hours and then reimplanted, at which time the contralateral kidney was removed. RBC washout from the preserved kidneys was measured at the time of initial cold flush. For 20 days after transplantation, serial measurements were made of serum creatinine, blood urea nitrogen, creatinine clearance, serum and urine osmolality, blood pH, and dog weight. No one solution cleared RBCs from the kidneys better than the other solutions. Renal function was significantly better in transplanted kidneys flushed and preserved with the Collins-2 solution than that in kidneys flushed and preserved with the other two solutions. There was generally no significant difference in function between kidneys preserved with dextran and those preserved with lactated Ringer's solution. Our findings suggest that the electrolyte composition of the flush solution may be more important than maintaining a high osmolality in the flush solution in the preservation of renal function during 24 hours' cold storage.

Some problems of sexual growth in adolescent underprivileged unwed black girls.

The adolescent female experiences many problems associated with sexual growth. These are greatly compounded in underprivileged black girls, particularly the unwed. Meharry Medical College has established comprehensive programs for the management of special health problems involving adolescent girls. This study presents an overview and statistical study of the psychological and psychosocial forces that confront these underprivileged girls.

Evaluation of catheter placement in the treatment of venous air embolism.

Venous air embolism is a potential complication of many surgical, therapeutic, and diagnostic procedures. Aspiration of air via a catheter placed in the superior vena cava or right atrium or placed in the pulmonary outflow tract and pulled through the right heart chambers had been advocated for the treatment of venous air embolism. In this study, three catheter positions were analyzed to determine which was best for removal of gas after induction of massive venous air embolism in dogs. In 18 dogs, 9 of which were suspended by their forelegs to simulate the sitting position used in posterior fossa exploration and 9 of which were supine, a Swan-Ganz catheter was placed in the right atrium, right ventricle, or pulmonary artery. A measured amount of air was injected into the left jugular vein and syringe aspiration of the air was attempted through the catheter. In the group with the catheter in the pulmonary artery, aspiration was continuous while the catheter was withdrawn through the right heart chambers. The amount of air aspirated varied widely among the three catheter positions, and no one catheter position proved superior to the other two.

Use of bovine heterografts in intravenous therapy.

Thirteen bovine heterografts were placed in nine patients: three for renal dialysis, four for treatment of diabetes mellitus, and six for cancer chemotherapy. One graft became occluded on the night of operation. One graft caused dilation of distal veins sufficient for dialysis and was never used; this graft was occluded by a blow ten months later. Two grafts were thrombosed by poor puncture technic, one after three months' use and one after seven months' use. One graft became infected, then thrombosed, and was never used. The remaining eight have served as intravenous conduits for 6 to 19 months. Despite the problems we have encountered with these grafts, we believe that their use is the procedure of choice in cancer patients with limited life spans, even when such patients have available and satisfactory saphenous veins. The establishment of a bovine heterograft requires less time than establishment of a saphenous vein graft and requires only regional or local anesthesia--both advantages in seriously ill patients.

Isolation and characterization of the specialized transducing bacteriophages phi80dargF and lambdah80cI857 dargF: specific cleavage of arginine transducing deoxyribonucleic acid by the endonucleases EcoRI and SmaR.

The directed transposition of argF to the tonB locus of the Escherichia coli chromosome and the subsequent isolation of the specialized transducing phage phi80dargF is described. The structure of this phage has been has been determined. A hybrid lambdah80cI857dargF phage has been constructed. Deoxyribonucleic acid isolated from these and their parent bacteriophages has been specifically cleaved by the endonucleases EcoRI and SmaR; the unique deoxyribonucleic acid fragments thus obtained have been resolved and analyzed by electrophoresis in agarose gel.
