Extreme diffuse adenomatous hyperplasia of Brunner's glands: case report.

A case of extreme diffuse adenomatous hyperplasia of Brunner's glands was clinically manifested by melena and anemia. Diagnosis was established by barium studies, endoscopy, ultrasonography, computerized tomography, and histology. The radiologic, clinical, pathologic, and differential diagnostic features are reviewed.

Bilateral primary breast carcinoma in a man.

The case history is presented of a 56-year-old man, who had a second primary carcinoma in the contralateral breast after 13 years. Both times, he underwent a modified radical mastectomy. The axillary lymph nodes were free from tumour. The incidence in men is about 1 per cent being bilateral in 14 out of 1,000 of these cases. The therapy for men is the same as for women. Because of the smaller breast volume, tumours in men are earlier discovered and show infiltration. If metastases cannot be detected, the first choice of treatment is surgical. Radiotherapy is indicated in case surgical intervention has not been radical.

Spontaneous rupture of the spleen in third trimester of pregnancy. Report of a case and review of the literature.

A case of idiopathic spontaneous rupture of the spleen in the third trimester of pregnancy is presented. The cause remains an enigma; the diagnosis is difficult, but echography may prove to be helpful. The treatment requires early exploration to enhance fetal as well as maternal survival. In this case mother and child did not survive. The etiology and clinical picture are discussed. The literature relating to rupture of the spleen is reviewed, including one particular case of a rupture in the third trimester of pregnancy.

Conjunctival sarcoidosis.

A biopsy from the lower conjunctival fornix, whether or not taken from follicles, is a method which yields a positive result in +/- 25% of cases of suspected sarcoidosis. We saw a patient with chronic recurrent follicular conjunctivitis of the upper eyelid, without further ocular symptoms. He was known to suffer from allergy and chronic respiratory disease. As the conjunctivitis did not improve a biopsy was taken from the conjunctiva of the upper eyelid. The specimen showed non-necrotizing epithelioid-cell granulomata with multinucleate giant cells and asteroid bodies, and an increase in reticular fibres. The diagnosis of sarcoidosis was confirmed by finding parabronchial granulomata and a raised level of angiotensin-converting enzyme in the serum. Prolonged local corticoid therapy then gradually cleared up the conjunctival condition.
