[Importance of embryo cryopreservation in a program of in vitro fertilization: experience and results of 550 transfers of thawed embryos].

Results of 550 transfers of cryopreserved and thawed embryos were analyzed. A total number of 47 pregnancies was achieved (8.5% per embryo transfer). Significantly better results were obtained when no selection of embryos before cryopreservation was made (i.e. pronucleate stage embryos and patients with all embryos frozen) compared to the "spare embryos" group-14 pregnancies per 99 transfers (14%) vs. 33 pregnancies per 451 transfers (7%) respectively. Cultivation of thawed embryos before embryo transfer has a beneficial effect on implantation-19 pregnancies per 121 transfers (16%) in the group of cultured embryos vs. 28 pregnancies per 429 transfers (7%) in the group without cultivation. Cryopreservation significantly contributes to the effectivity of IVF and provides further possibilities for conception in addition to those achieved from the fresh cycle.

[Helicobacter pylori eradication: modified triple therapy with lansoprazole].

For the eradication of Helicobacter pylori in H.p.-positive patients with peptic ulcer, preference is presently being given worldwide to the modified triple therapy-comprising an acid suppressant and two antibiotics. This relatively new form of treatment is highly effective, relatively well tolerated, patient-friendly (5 to 6 tablets daily), and inexpensive. In 12 studies involving more than 600 patients, the new proton pump inhibitor, lansoprazole, has proved to be an effective component of this eradication regimen. In combination with clarithromycin and metronidazole or amoxicillin and clarithromycin, consistently high eradication rates of more than 85% are obtained, with treatment of only seven days duration. Since in terms of clinical efficacy 30 mg lansoprazole daily presumably correspond to 40 mg omeprazole daily, the new proton pump inhibitor represents a more economic alternative for H.p. eradication.

Prolonged allograft survival by the inhibition of costimulatory CD2 signals but not by modulation of CD48 (CD2 ligand) in the rat.

The CD2 receptor is an important costimulatory molecule in T cell activation. Its ligand CD48 in rodents is supposed to be a homologue of human CD58, because of its similarities in structure and distribution. We evaluated the immunosuppressive activity of CD2/CD48-directed therapy in vitro and in vivo for the efficacy in prolonging rat heart allograft survival in a high responder transplant model. CD2-directed monoclonal antibody (mAb) therapy significantly prolonged median survival time to 45 days (P < 0.001). Suppression was mediated by down-modulation of CD2 below 20% on lymph node cells without considerable cell depletion. In contrast, CD48 mAb could not prolong graft survival. Rejection occurred in the presence of complete CD48 modulation and, therefore, despite disruption of the CD2-CD48 interaction. CD48 mAb failed to inhibit lymphocyte activation via a mitogenic pair of CD2 mAbs and inhibited mixed lymphocyte reaction (MLR) only by an unspecific mechanism. In conclusion, our results suggest a negative regulatory signal transduction by inhibitory CD2 mAbs and argue against a pivotal role of mere disruption of the CD2-CD48 interaction in CD2-mediated immunosuppression.

ATP-dependent redistribution of phosphatidylethanolamine in the plasma membrane of an epithelial and a hepatocytic cell line.

The redistribution of spin-labeled phospholipid analogues of sphingomyelin (SM) and phosphatidylethanolamine (PE) from the outer to the inner leaflet of the plasma membrane of an epithelial (CaCo-2) and a hepatocytic cell line (HepG2) was investigated. The amount of analogues in the outer leaflet was determined by their back-exchange to bovine serum albumin (BSA). For both cell lines a fast ATP-dependent inward movement of spinlabeled PE (SL-PE) was found while SL-SM redistributed only slowly by a passive mechanism. After depletion of intracellular ATP transverse diffusion of SL-PE was similar to that of SL-SM. The data are compatible with the presence of an aminophospholipid translocase in both cell lines.

Transverse mobility of aminophospholipids in the plasma membrane of ram sperm cells.

The transbilayer movement of aminophospholipids in the plasma membrane of ram sperm cells was investigated using spin-labeled lipid analogues. After incorporation, spinlabeled phosphatidylserine (SL-PS) and phosphatidyl-ethanolamine (SL-PE) rapidly disappeared from the exoplasmic monolayer. Even at lower temperatures (10 degrees C) the inward movement of SL-PE is fast. The initial velocities of the internalization of SL-PS and SL-PE were compared with those of ram and human erythrocytes.

Bioavailability of estradiol from a new matrix and a conventional reservoir-type transdermal therapeutic system.

OBJECTIVE: Bioavailability of estradiol delivered from a newly developed matrix-type transdermal therapeutic system (MTTS) was compared with that of the conventional reservoir-type system (RTTS). Both formulations have a nominal delivery rate of 50 micrograms per day of 17 beta-estradiol (E2). Plasma concentrations of E2 and estrone (E1) were determined at steady state during a 96-h application of each formulation to 34 postmenopausal volunteers, using a two-stage randomized two-period crossover design. RESULTS: The MTTS proved to be equivalent to the RTTS with respect to the extent of E2 absorption. Due to differences in patch design and composition, the rate of absorption was different between the two systems, with less fluctuating E2 plasma levels during application of the matrix system. Local tolerability and adhesion of MTTS appeared to be better than those of the reservoir system.

Detection of genetic alterations in micrometastatic cells in bone marrow of cancer patients by fluorescence in situ hybridization.

Detection of micrometastatic tumor cells in bone marrow of cancer patients has been shown to be of prognostic significance. To further characterize these cells, we combined antibody labeling and fluorescence in situ hybridization (FISH). For detection of numerical changes of chromosome 17, nine patients with proven breast cancer whose bone marrow contained epithelial tumor cells were evaluated. Epithelial cells were stained by anticytokeratin antibody. Afterwards FISH was performed using an alpha-satellite probe specific for chromosome 17. In a second series bone marrow epithelial cells of eight patients with breast cancer and of six with prostatic cancer were evaluated for the amplification of HER-2/neu by using a gene-specific DNA probe. In the first series four patients had only single epithelial cells in their bone marrow. Only one single cell showed five hybridization signals, whereas all other single cells showed two or less. Five patients had clusters of epithelial cells in bone marrow with or without additional single cells. One hundred four cells had three or more hybridization signals and 103 of these polysomic cells were located in tumor cell clusters. In the second series we could detect HER-2/neu amplification in bone marrow epithelial tumor cells in two of eight patients with breast cancer but in none of the prostatic cancer patients. These results show that it is possible to detect numerical chromosomal changes and oncogene amplification in bone marrow micrometastatic epithelial cells of cancer patients by combining immunophenotyping and FISH.
